Background: Circulating markers are attractive molecules for prognosis and management of cancer that allow sequential monitoring of patients during and after treatment. Based on previous protein profiling data, circulating interleukin 1 receptor antagonist (IL-1Ra) was evaluated as a potential diagnostic and prognostic marker for squamous cell carcinomas of the head and neck (SCCHN). In this study, we aimed at confirming the clinical relevance of plasma IL-1Ra in SCCHN and exploring its potential as a prediction marker for SCCHN.

Despite a remarkable increase in the genomic profiling of cancer, integration of genomic discoveries into clinical care has lagged behind. We report the feasibility of rapid identification of targetable mutations in 153 pediatric patients with relapsed/refractory or high-risk leukemias enrolled on a prospective clinical trial conducted by the LEAP Consortium. Eighteen percent of patients had a high confidence Tier 1 or 2 recommendation.

"Chronic Disseminated Aspergillosis," a Novel Fungal Immune Reconstitution Inflammatory Syndrome.

Open Forum Infect Dis

November 2020

Paris University, Infectious Diseases and Tropical Medicine Department, Necker-Enfants Malades Hospital, AP-HP, Paris, France.

We report a case of chronic hepatosplenic aspergillosis following immune reconstitution complicating colic aspergillosis in an AIDS patient with multicentric Castleman disease. Symptoms mimicked the clinical presentation of chronic disseminated candidiasis and responded to corticosteroid. This emerging entity enlarges the spectrum of fungal immune reconstitution inflammatory syndrome in the HIV setting.

  • Transfer-RNA-derived fragments (tRFs) are small non-coding RNAs that differ from their tRNA origins and can regulate gene expression and impact various diseases.
  • A study focused on tRFs in squamous cell carcinoma of the head and neck (SCCHN) analyzed data from 523 patients to explore their abundance and potential as prognostic indicators.
  • The research identified 15 tRFs linked to overall survival rates, with tRF-20-S998LO9D showing significant promise as a marker, being upregulated in cancerous tissue compared to healthy samples.

Esophageal cancer in Aleppo, Syria 2010-2020: a rare cancer in a war zone.

Ann Transl Med

September 2020

Division of Hematology/Oncology, The Warren Alpert Medical School of Brown University, Lifespan Cancer Institute, Rhode Island Hospital, Providence, RI, USA.

The management of diseases such as cancer in developing countries are often suboptimal given a lack of resources and access to specialists and therapeutics. In March 2020, Syria descended into its ninth year of the war with a rising death toll and millions of Syrian refugees. Aside from the inherent dangers of war, cancer care during war is especially difficult with partially or non-functional infrastructure due to destruction, inconsistent electrical power, inaccessibility, or the inherent dangers of living in a war zone.

Coronavirus disease 2019 in patients with inborn errors of immunity: An international study.

J Allergy Clin Immunol

February 2021

Garvan Institute of Medical Research, Darlinghurst, Australia; St Vincent's Clinical School, UNSW Sydney, Darlinghurst, Australia. Electronic address:

Background: There is uncertainty about the impact of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in individuals with rare inborn errors of immunity (IEI), a population at risk of developing severe coronavirus disease 2019. This is relevant not only for these patients but also for the general population, because studies of IEIs can unveil key requirements for host defense.

Objective: We sought to describe the presentation, manifestations, and outcome of SARS-CoV-2 infection in IEI to inform physicians and enhance understanding of host defense against SARS-CoV-2.

Efficacy of generic meropenem products in combination with colistin in carbapenemase-producing Klebsiella pneumoniae experimental osteomyelitis.

Int J Antimicrob Agents

November 2020

UMR 1173, Versailles Saint-Quentin Université, Versailles, France; St Louis Hospital AP-HP, Université, Paris 7, France.

Guidelines for the management of carbapenemase-producing Enterobacterales (CPE) infections recommend a combination of two active agents, including meropenem if the minimum inhibitory concentration (MIC) is ≤8 mg/L. The therapeutic equivalence of meropenem generics has been challenged. We compared the bactericidal activity of meropenem innovator (AstraZeneca) and four generic products (Actavis, Kabi, Mylan and Panpharma), both in vitro and in vivo, in association with colistin.

Article Synopsis
  • Researchers are studying how metabolic changes, particularly in amino acid pathways linked to the folate cycle, affect the effectiveness of cancer treatments in acute myeloid leukemia (AML).
  • They found that lower levels of folate and a specific gene variant affecting the MTHFR enzyme can lead to resistance against certain cancer therapies targeting MYC in both lab models and patient samples.
  • Supplementing with CH-THF, a product of the MTHFR enzyme, can potentially overcome this resistance, suggesting that assessing individual folate cycle status may help identify patients who could benefit most from MYC-targeting treatments.

Background: There is little evidence to support the management of severe COVID-19 patients.

Methods: To document this variation in practices, we performed an online survey (April 30-May 25, 2020) on behalf of the European Society of Intensive Care Medicine (ESICM). A case vignette was sent to ESICM members.

Mucin 1 (MUC1) is a membrane-bound and secreted glycoprotein that has a protective role in surface epithelia. We recently demonstrated that MUC1 mRNA expression was upregulated in tumour-free tongue tissues adjacent to squamous cell carcinoma of the oral tongue (SCCOT) compared with that in the tumour tissues. The present study investigated MUC1 protein in SCCOT tissue and serum from patients with squamous cell carcinoma of the head and neck (SCCHN) at different sub-sites.

After heart transplant, adding everolimus (EVL) to standard immunosuppressive regimen mostly relies on converting calcineurin inhibitors (CNIs) into EVL. The aim of this study was to describe the effects of combining low-dose EVL and CNIs in maintenance immunosuppression regimen (quadritherapy) and compare it with standard tritherapy associating standard-dose CNIs, mycophenolate mofetil, and corticosteroids. In the 3-year registry cohort of heart transplanted patients, those who received quadritherapy were compared with those who received tritherapy.

Utility of anaerobic bottles for the diagnosis of bloodstream infections.

BMC Infect Dis

February 2020

Department of Infectious Diseases, Saint-Louis Hospital, Paris, University of Paris Diderot Paris 7, Sorbonne Paris Cité, INSERM U941, Paris, France.

Article Synopsis
  • The study examines the effectiveness of anaerobic blood culture bottles in diagnosing bloodstream infections (BSIs), noting that obligate anaerobes are usually not a significant part of bacteria found in blood cultures.
  • Over a 6-month period, 19,739 blood cultures were analyzed; anaerobic bottles showed a high positive rate, but many of these were contaminants rather than true infections.
  • The findings suggest that while anaerobic bottles can be useful, most identified BSIs were associated with non-obligate anaerobes, and genuine BSIs from obligate anaerobes were rare, primarily occurring in patients with gastrointestinal issues or immunocompromised conditions.
