Introduction: The degree of improvement in serum creatinine (SCr) has previously been suggested as a sensitive indicator of treatment response in patients with hepatorenal syndrome-acute kidney injury (HRS-AKI), while HRS reversal remains the primary endpoint in clinical trials.

Methods: A total of ≥ 30% SCr improvement was analyzed as an exploratory prespecified endpoint in the CONFIRM trial. In this post hoc analysis, intent-to-treat population data from three Phase 3 studies (OT-0401, REVERSE, and CONFIRM) conducted in North America in patients with HRS-AKI were pooled to assess the incidence of > 30% improvement in SCr and its association with clinical outcomes.

Article Synopsis
  • * It typically occurs in males and is characterized by extra-nodal masses, often found in the oral cavity or gastrointestinal tract, but a unique case involved a 93-year-old female without HIV showing pericardial effusion instead.
  • * Diagnosis of PBL can be challenging due to atypical presentations; thus, awareness, thorough immunohistochemistry, and clinical correlation are critical for accurate identification.

Cephalosporins for Outpatient Pyelonephritis in the Emergency Department: COPY-ED Study.

Ann Emerg Med

November 2024

Department of Emergency Medicine and Pharmacy, University of Iowa, Iowa City, IA. Electronic address:

Study Objective: The primary objective of our study was to compare the effectiveness of oral cephalosporins versus fluroquinolones and trimethoprim/sulfamethoxazole (TMP-SMX) for the treatment of pyelonephritis in patients discharged home from the emergency department (ED).

Methods: This was a multicenter, retrospective, observational cohort study of 11 geographically diverse US EDs. Patients aged ≥18 years diagnosed with pyelonephritis and discharged home from the ED between January 1, 2021 and October 31, 2023 were included.

This case report and literature review explore Crohn's disease (CD), a chronic inflammatory disorder of the gastrointestinal tract characterized by unpredictable flare-ups and remissions. The study highlights the diverse clinical manifestations, including abdominal pain, diarrhea, and weight loss, which significantly affect quality of life. It examines the interplay of genetic, environmental, and immune factors in CD's pathogenesis and the complexities in managing the disease.

Differences in Intraoperative Fluoroscopic Radiation Exposure During Ankle Fracture Open Reduction and Internal Fixation Between Orthopaedic Surgery and Podiatry.

J Am Acad Orthop Surg Glob Res Rev

June 2024

Department of Orthopaedics and Sports Medicine, Temple University Hospital, Philadelphia, PA (Dr. DiFiori and Dr. Bires); Lancaster Orthopaedic Group, Lancaster General Hospital, Lancaster, PA (Dr. Luginbuhl); Department of Orthopaedics, St. Josephs University Medical Center, Paterson, NJ (Dr. Rallis); Trinity Health Mid-Atlantic, St. Mary Medical Center, Langhorne, PA (Dr. Gokcen).

Surgical ankle fractures pose a unique situation because both podiatrists and orthopaedic surgeons manage these injuries. Intraoperative fluoroscopy is routinely used; however, excessive radiation can be harmful to both the patient and the surgical team. The primary goal of this study was to determine whether there is a difference in the amount of intraoperative radiation exposure during ankle fracture open reduction and internal fixation (ORIF) when performed by orthopaedic surgeons versus podiatrists.

Background And Aims: Perianal fistuliing Crohn's disease [PFCD] is an aggressive phenotype of Crohn's disease defined by frequent relapses and disabling symptoms. A novel consensus classification system was recently outlined by the TOpCLASS consortium, which seeks to unify disease severity with patient-centred goals but has not yet been validated. We aimed to apply this to a real-world cohort and to identify factors that predict transition between classes over time.

Carmustine is an intravenous alkylating agent that inhibits DNA synthesis and protein synthesis by forming cross-links in DNA and RNA and ultimately causing cell death. We report a rare case of Takotsubo cardiomyopathy that is thought to be related to carmustine, where the patient presented with chest pain, and ST depression within 12 hours of carmustine therapy. Workup, including echocardiography, showed global hypokinesis of the left ventricle with regional variations; mid and apical anterior septum and apex were akinetic with left ventricular ejection fraction (EF) of 30%.

Unplanned extubations (UE) frequently occur in critical care units. These events are precipitated by many risk factors and are associated with adverse outcomes for patients. We reviewed the current literature to examine factors related to UE and presented the analysis of 41 articles critical to the topic.

Background: Task specific tremor (TST) is a poorly understood entity without any standard treatments, that may subsequently develop tremor during additional tasks, later develop postural/kinetic tremor (essential tremor criteria), and later develop Parkinson's disease. The pathophysiology is not understood as it has features of tremor, dystonia, and parkinsonism.

Objectives: To assess response of TST to apomorphine and thus infer pathophysiology.

Alcohol and Cardiovascular Disease: Helpful or Hurtful.

Rev Cardiovasc Med

April 2023

Department of Cardiology, St Mary Medical Center, Langhorne, PA 19047, USA.

Alcohol has been considered throughout history as both a tonic and a poison. The answer as to which likely depends on one's current health, the amount one consumes, and with what regularity. In examining the relationship of alcohol and cardiovascular health, most, but not all, epidemiological studies suggest that light to moderate alcohol consumption can reduce the incidence of coronary artery disease (CAD), ischemic stroke, and peripheral arterial disease events.

Chronic kidney disease (CKD) is a debilitating progressive illness that affects more than 10% of the world's population. In this literature review, we discussed the roles of nutritional interventions, lifestyle modifications, hypertension (HTN) and diabetes mellitus (DM) control, and medications in delaying the progression of CKD. Walking, weight loss, low-protein diet (LPD), adherence to the alternate Mediterranean (aMed) diet, and Alternative Healthy Eating Index (AHEI)-2010 slow the progression of CKD.

Behcet disease: an undifferentiating and complex vasculitis.

Postgrad Med

January 2023

Internal Medicine, Lankenau Medical Center, Wynnewood, Pennsylvania, USA.

Behçet Disease is a relapsing and remitting variable vessel vasculitis characterized by recurrent mucocutaneous ulcers that can involve almost every organ system in the body. Indeed, the presence of recurrent oral or genital ulcers with other auto-inflammatory symptoms should raise suspicion for this elusive disease. It is unique among the vasculitides in that it can affect vessels of small, medium, and large size and tends to involve venous rather than arterial circulation, and its effects on the pulmonary venous circulation are particularly notable for their role in disease mortality.
