Background: Cervical laminoplasty is a motion-preserving procedure that addresses spinal cord compression and avoids postlaminectomy kyphosis associated with cervical laminectomy. The most common complications include C5 nerve palsy, axial neck pain, hinge nonunion, and premature closure. Plating is a relatively newer method of laminoplasty fixation that may provide greater stabilization postoperatively and reduce the risk of laminoplasty closure compared with less rigid (e.

Stem cells carry the remarkable ability to differentiate into different cell types while retaining the capability to self-replicate and maintain the characteristics of their parent cells, referred to as potency. Stem cells have been studied extensively to better understand human development and organogenesis. Because of advances in stem cell-based therapies, regenerative medicine has seen significant growth.

Purpose: A case of vancomycin enzyme immunoassay (EIA) interference confirmed by high-performance liquid chromatography (HPLC) is described.

Summary: Therapeutic drug monitoring is standard of practice in vancomycin dosing and monitoring in order to maximize the pharmacodynamic effects and minimize toxicity. After a 52-year-old woman received 5 doses of vancomycin, serum concentrations continued to rise for several days in the absence of ongoing vancomycin administration.

Article Synopsis
  • NSAIDs are commonly used pain relievers but are linked to significant side effects, including hospitalizations and deaths, prompting increased warnings from health authorities about related cardiovascular risks.
  • Randomization in studies helps control confounding factors, making trials necessary to obtain reliable data, but current research remains inconclusive about the full benefits and risks of NSAIDs.
  • Healthcare providers must weigh the potential benefits of pain relief and improved quality of life against cardiovascular and gastrointestinal risks when deciding on NSAID prescriptions, favoring balanced individual clinical decisions.

Previous studies have shown that virtual reality (VR) is effective in reducing acute and chronic pain both in adults and in children. Given the emergence of new VR technology, and the growing body of research surrounding VR and pain management, an updated systematic review is warranted. The purpose of this systematic review is to compare the effectiveness of VR in reducing acute and chronic pain in adults.

Objective: Pyogenic spinal infections account for 2%-4% of orthopaedic infections. They are often difficult to diagnose, resulting in a delay in diagnosis. Risk factors for orthopaedic and spinal infection are well-documented in the literature, yet there is a paucity of studies examining risk factors specifically for multifocal spinal infections.

Wilderness Medical Society Clinical Practice Guidelines for the Prevention and Treatment of Acute Altitude Illness: 2019 Update.

Wilderness Environ Med

December 2019

Altitude Research Center, Division of Pulmonary Sciences and Critical Care Medicine, Department of Medicine, University of Colorado Anschutz Medical Campus, Aurora, CO.

To provide guidance to clinicians about best preventive and therapeutic practices, the Wilderness Medical Society (WMS) convened an expert panel to develop evidence-based guidelines for prevention and treatment of acute mountain sickness, high altitude cerebral edema, and high altitude pulmonary edema. Recommendations are graded based on the quality of supporting evidence and the balance between the benefits and risks/burdens according to criteria put forth by the American College of Chest Physicians. The guidelines also provide suggested approaches to prevention and management of each form of acute altitude illness that incorporate these recommendations.

Objectives: To utilize the RSSearch Patient Registry (RSSPR) to examine local control (LC), overall survival (OS), and toxicities following stereotactic body radiation therapy (SBRT) for stage I (T1-T2/N0) medically inoperable small cell lung carcinoma (SCLC).

Materials And Methods: We searched the RSSPR for medically inoperable stage I SCLC patients treated with definitive SBRT. Potential predictive factors of OS were estimated using the Kaplan-Meier method as well as a Cox proportional hazards model.

Technical Tips: Surviving a Joint Commission Visit in the Neurodiagnostic Department.

Neurodiagn J

December 2019

a Neurophysiology Department , St. Mary's Medical Center, Huntington , West Virginia.

An accreditation site visit from The Joint Commission (TJC) can strike fear in the heart of staff, but knowing what surveyors expect can go a long way toward reducing anxiety. Embedding TJC standards into our everyday routine increases the likelihood of a positive survey. Although TJC accredits many different types of organizations (and specialty services within organizations), the focus of this article will be on hospital-based, neurodiagnostic (ND) departments and includes information specific to infection prevention; medication management; provision of care, treatment, and services; rights and responsibilities of the individual; environment of care; fire safety; hazardous materials; human resources; performance improvement; and record of care, treatment, and services.

  • The use of ultra-portable, mechanically powered disposable negative pressure wound therapy (dNPWT) is increasing for managing wounds in outpatient care and transitioning inpatients to home care, showing similar effectiveness to electric versions.
  • Very few studies exist on best practices for dNPWT across different wound types, indicating a need for comprehensive clinical guidelines and knowledge on reimbursement processes.
  • A panel of experts has developed recommendations for using dNPWT, including optimal patient selection, wound preparation, and reimbursement guidelines, aimed at improving clinical outcomes and economic value.
