Article Synopsis
  • The study examined how cognitive specialists diagnose and manage functional cognitive disorder (FCD) compared to neurodegenerative diseases, using clinical vignettes for assessment.
  • Forty-five experts from 12 countries effectively distinguished between FCD and neurodegeneration, accurately diagnosing all neurodegenerative cases, but only 75%-88% of FCD cases were correctly classified due to varied terminology.
  • The findings highlight the need for improved agreement on FCD terminology and more research into validation and diagnostic tools, while suggesting that management strategies should include diagnostic communication and referrals for psychotherapy.

Integration of neuroscience into psychiatric training and practice: suggestions for implementation.

BJPsych Bull

April 2024

South West London and St George's Mental Health NHS Trust, London, UK.

Mainstream psychiatric practice requires a solid grounding in neuroscience, an important part of the biopsychosocial model, allowing for holistic person-centred care. There have been repeated calls for better integration of neuroscience into training, although so far with less focus on implementation for life-long learning. We suggest that such training should be accessible and utilised by all psychiatrists, not solely those with a special interest in neuropsychiatry.

Article Synopsis
  • Streptococcus pneumoniae is a key cause of pneumonia in children, and a study (CAP-IT trial) examined antibiotic treatment effects on serotype prevalence and resistance, finding no significant differences based on dosage or duration of amoxicillin.
  • In-depth genomic analysis of 390 pneumococcal isolates revealed that serotypes 15B/C, 11A, 15A, and 23B1 were the most common, with varying levels of resistance to penicillin and amoxicillin, but overall low resistance rates.
  • The findings suggest that current amoxicillin treatment does not influence serotype prevalence, indicating a need for vaccines covering a broader range of pneumococcal serotypes.

Pregnant women and measles: we need to be vigilant during outbreaks.

EClinicalMedicine

June 2024

Immunisation and Countermeasures Division, UK Health Security Agency, UK.

A number of countries including the UK are currently experiencing large outbreaks of measles affecting mainly young children but also adolescents and young adults. Women of childbearing age are a particular group of concern because the 1988 Wakefield Lancet paper, which falsely asserted a connection between the MMR vaccine and autism, was associated with a large and sharp decline in childhood MMR uptake over several years. This has left large cohorts of non-immune adolescents and young adults (born between 1998 and 2004), including young women who are now of childbearing age and remain susceptible to measles as well as rubella.

Introduction: Medullary thyroid carcinoma (MTC) is a rare thyroid cancer arising from the calcitonin-secreting parafollicular cells. Previous studies suggested a preoperative calcitonin level >200ng/l is an indication for prophylactic lateral neck dissection (LND) to remove micrometastases and improve survival outcomes.

Methods: This retrospective single-centre study assessed the efficacy of preoperative calcitonin levels as a marker for determining need for prophylactic LND in MTC.

Professional identity and GP trainers as educational leaders.

Educ Prim Care

September 2024

Institute of Medical and Biomedical Education, St George's University of London, London, UK.

GP training in the UK has a long history of success, however that is measured. That success is in part due to the formality and credentialling that underlies preparation to take on that role of a GP teacher, which is somewhat under current threat due to workforce pressures. We identify three important factors associated with the GP trainer function [leadership, professional identity and clinical care improvement] that are not often analysed but are at some risk if preparation for the GP trainer role is reduced or devalued.

Introduction: To examine the impact of the COVID-19 pandemic on mortality, we estimated excess all-cause mortality in 24 countries for 2020 and 2021, overall and stratified by sex and age.

Methods: Total, age-specific and sex-specific weekly all-cause mortality was collected for 2015-2021 and excess mortality for 2020 and 2021 was calculated by comparing weekly 2020 and 2021 age-standardised mortality rates against expected mortality, estimated based on historical data (2015-2019), accounting for seasonality, and long-term and short-term trends. Age-specific weekly excess mortality was similarly calculated using crude mortality rates.
