Article Synopsis
  • High-risk nonmuscle invasive bladder cancer patients who progress to muscle-invasive bladder cancer indicate treatment failure, and this study explores how progression impacts overall survival (OS).
  • The study analyzed data from 1,510 patients across multiple institutions, using random survival forest and Cox regression models to assess factors influencing OS.
  • Results show that the time to progression is the most significant predictor of OS, suggesting it could be used as a primary endpoint in clinical trials and for patient stratification in treatment planning.

Introduction: The study aimed to evaluate the effectiveness and safety of brivaracetam (BRV) as conversion monotherapy in adults with focal epilepsy treated in the context of real-world clinical practice.

Methods: This was a retrospective, observational, non-interventional study in adults with focal epilepsy who converted to BRV monotherapy following the withdrawal of background antiseizure medications (ASMs). Primary effectiveness outcome was the retention rate of BRV as single ASM at 6 and 12 months.

Objectives: Seizures (SZ) are one of the main complications occurring in infants undergoing therapeutic hypothermia (TH) due to perinatal asphyxia (PA) and hypoxic ischemic encephalopathy (HIE). Phenobarbital (PB) is the first-line therapeutic strategy, although data on its potential side-effects need elucidation. We investigated whether: i) PB administration in PA-HIE TH-treated infants affects S100B urine levels, and ii) S100B could be a reliable early predictor of SZ.

Peripheral artery disease is a common condition in patients on chronic dialysis treatment, end-stage kidney failure itself being a risk factor. The most severe stage of peripheral artery disease, critical limb ischemia, causes marked chronic pain and is associated with risk of limb loss. Despite improvements in revascularization procedures, the results of limb salvage procedures among dialysis patients remains poor, and lower extremity amputation is associated with high mortality and grim socio-economic implications.

Current definitions of migraine that are based mainly on clinical characteristics do not account for other patient's features such as those related to an impaired quality of life, due to loss of social life and productivity, and the differences related to the geographical distribution of the disease and cultural misconceptions which tend to underestimate migraine as a psychosocial rather than neurobiological disorder.Global differences definition, care access, and health equity for headache disorders, especially migraine are reported in this paper from a collaborative group of the editorial board members of the Journal of Headache and Pain. Other components that affect patients with migraine, in addition to the impact promoted by the migraine symptoms such as stigma and social determinants, are also reported.

Transcranial Doppler (TCD) is a repeatable, at-the-bedside, helpful tool for confirming cerebral circulatory arrest (CCA). Despite its variable accuracy, TCD is increasingly used during brain death determination, and it is considered among the optional ancillary tests in several countries. Among its limitations, the need for skilled operators with appropriate knowledge of typical CCA patterns and the lack of adequate acoustic bone windows for intracranial arteries assessment are critical.

Article Synopsis
  • Postoperative atrial fibrillation (POAF) is a frequent complication after heart surgery and is linked to worse health outcomes, highlighting the need for early identification of at-risk patients.* -
  • The study examined left atrial strain parameters through preoperative echocardiography in patients undergoing coronary artery bypass graft surgery, finding that a global peak atrial longitudinal strain (PALS) value below 28% significantly indicates a higher risk of developing POAF.* -
  • Among 310 patients, those with PALS <28% had a 51% incidence of POAF, compared to 14% for those with PALS ≥28%, suggesting that assessing PALS can help target patients for preventive measures and closer monitoring.*

Late-onset multiple sclerosis: disability trajectories in relapsing-remitting patients of the Italian MS Registry.

J Neurol

April 2024

Department of Medical Sciences and Public Health, Multiple Sclerosis Center, Binaghi Hospital, University of Cagliari, ASL Cagliari, via Is Guadazzonis 2, PO Binaghi, 01916, Cagliari, Italy.

Background: Generally infrequent, multiple sclerosis (MS) with late onset (LOMS) is characterized by an onset over the age of 50 and a mainly progressive course, while relapsing-remitting (RR) forms are less frequently observed and explored. This study aimed to characterize a large cohort of MS patients with RRMS at onset to assess the baseline factors related to the worst disability trajectories and explore the role of LOMS.

Methods: The data were extracted from the Italian MS Register (IMSR).

Article Synopsis
  • The Italian Society of Echography and Cardiovascular Imaging (SIECVI) conducted a national survey to investigate stress echocardiography (SE) practices across Italy, collecting data from 228 laboratories in November 2022.
  • The survey revealed that out of 179 centers performing SE, most were located in northern Italy, and the study categorized them into low, moderate, and high volume of activity based on the number of SE examinations.
  • Key findings indicated differences in the use of stressors, with a tendency for high-volume centers to employ multiple stress techniques and incorporate advanced evaluations like coronary flow velocity reserve (CFVR) more frequently than low and moderate volume centers.

  • Pulmonary embolism (PE) is a serious health issue that can be life-threatening, especially in patients at high risk, who need immediate treatment to avoid worsening conditions or death.
  • Recovery from PE can lead to long-term complications, including recurrent cases, bleeding, and psychological issues, which necessitates better management strategies.
  • The paper emphasizes the importance of catheter-directed therapies for unstable PE patients, as these offer quicker and safer recovery options compared to traditional treatments, highlighting the need for a patient-centered approach in managing risk and treatment.

Article Synopsis
  • The study examines how general anesthesia (GA) compares to conscious sedation/local anesthesia (CS/LA) in patients with minor strokes undergoing immediate mechanical thrombectomy (iMT).
  • The research involved analyzing data from 16 stroke centers, focusing on patients with isolated M2 occlusions and mild stroke severity, utilizing propensity score matching for a fair comparison.
  • Results showed no significant difference in patient outcomes or complications between those receiving GA or CS/LA, indicating that the choice of anesthesia does not impact recovery or procedural safety in these cases.

Background: The aim of our study was to find predictors of parenchymal hematoma (PH) and clinical outcome after mechanical thrombectomy (MT) in patients with large vessel occlusion (LVO) and baseline large infarct.

Methods: The databases of 16 stroke centers were retrospectively screened for patients with anterior circulation LVO and baseline Alberta Stroke Program Early CT Score (ASPECTS) ≤5 that received MT. Procedural parameters, including the number of passes during first and second technique of MT, were recorded.

The aim of this study is to present the first Italian experience with robotic-assisted retrograde intrarenal surgery (rRIRS) using the Ily platform. Procedures were performed for renal stones using the Ily Robot (STERLAB, Vallauris, France), which is a ureteroscope holder with multiple degrees of freedom that can be controlled remotely through a wireless controller. In March 2023, consecutive patients with indications for rRIRS were included in the study.

Background: Despite the feasibility and promising activity data on intensity-modulated RT and simultaneous integrated boost (IMRT-SIB) dose escalation in preoperative chemoradiation (CRT) for locally advanced rectal cancer (LARC), few data are currently available on long-term outcomes.

Patients And Methods: A cohort of 288 LARC patients with cT3-T4, cN0-2, cM0 treated with IMRT-SIB and capecitabine from March 2013 to December 2019, followed by a total mesorectal excision (TME) or an organ-preserving strategy, was collected from a prospective database of 10 Italian institutions. A dose of 45 Gy in 25 fractions was prescribed to the tumor and elective nodes, while the SIB dose was prescribed according to the clinical practice of each institution on the gross tumor volume (GTV).
