Although a subspecialty-trained transfusion medicine (TM) physician brings value to the clinical bedside, hospital transfusion service oversight often falls under the responsibility of pathologists primarily focused on surgical pathology. Yet, pathologists who lack TM fellowship training may not be quite as confident in their role as the TM physician in-charge, especially when the need to communicate with another clinician arises. Given that blood is a resource subject to frequent shortages, there is a need for constant monitoring of blood utilization such that those responsible for transfusion service oversight need to handle challenging clinical interactions when transfusion guidelines are breeched.

Background/aims: Diagnostic abdominal paracentesis has been described in literature to have variable sensitivity of 50%-75% for the detection of peritoneal carcinomatosis (PC). We believe that random needle aspirates from the omentum, even in the absence of obvious deposits by endoscopic ultrasound (EUS), could prove malignancy in patients with PC.
Methods: Consecutive patients who underwent EUS for diagnosis and staging of cancer and found to have ascites were included after obtaining informed consent.

Pulmonary tuberculosis can have a wide variety of presentations including hematological manifestations. We report a case of a young male patient who presented with complaints of generalized petechiae, gum bleeding, systemic lymphadenopathy, and severe thrombocytopenia. His bone marrow revealed normal megakaryocytes, and in the absence of hepatosplenomegaly, a diagnosis of immune thrombocytopenic purpura (ITP) was made.

Context: Subclinical hypothyroidism (SCH) is stated as mild thyroid failure, is more common as compared to overt hypothyroidism, is associated with different biochemical abnormalities such as dyslipidemia, and is also having high conversion rate into overt hypothyroidism in patients having thyroid peroxidase (TPO) antibody positive. Lipid abnormalities are controversial in SCH and there is lack of Indian studies showing correlation between lipid abnormalities and TPO positivity in SCH. Hence, we did this study to find the TPO positivity and associated dyslipidemia in SCH patients.
