The coexistence of pleural and pericardial effusions in frail patients with or without confirmed neoplasia necessitates the use of a minimally invasive technique that has a minor impact on the patient's general status and allows for fast fluid evacuation and biopsy sampling if necessary. We present a subxiphoid mediastinoscopic autonomous (simultaneous noncommunicating) double fenestration approach for these patients with both diagnostic and therapeutic advantages in selected cases. Using the mediastinoscope alone through the subxiphoid incision can considerably reduce the duration of operation, allow for fluid evacuation, and significantly alleviate the patient's symptoms.

It is bilateral in 20% of the cases. Most often it appears due to severe occlusion of the carotid arteries (ICA, MCA>ECA), described in 1963 by Kearns and Hollenhorst.

Introduction: Malignant pleuro-pericardial effusions (MPPEs) are a common problem in the treatment of patients with cancer and may occur with any malignancy.
Methods: Between 01.01.
Purpose: to evaluate the efficiency of new torsional phacoemulsification software (Ozil IP system) in hard nucleus cataract extraction.
Methods: 45 eyes with hard senile cataract (degree III and IV) underwent phacoemulsification performed by the same surgeon, using the same technique (stop and chop). Infiniti (Alcon) platform was used, with Ozil IP software and Kelman phaco tip miniflared, 45 degrees.
Rigid Gas-Permeable contact lenses is a less risky option for improving the quality of vision in corneal ectasias such as keratoconus. They reshape the corneal surface (flattens the cornea) so that in most cases, with a proper lens, the patient can reach a visual acuity of 20/20.

Ophthalmic solutions are the main therapeutic methods used in the treatment of ocular diseases, including glaucoma. Preservatives are added in multidose solutions to reduce the risk of microbial contamination. Repeated use of topical medication, especially in patients treated for glaucoma, has many adverse effects especially on the ocular surface.

Adjustment of the ocular flow has clear implications in glaucomatous pathology. It cannot be drawn a clear barrier between causality and effect of ocular vascular factor in glaucoma but altered hemodynamics leads clearly to node loss. Unstable ocular perfusion creates a low blood flow to the optic nerve directly involved in glaucomatous optic neuropathy , ocular perfusion pressure (PPO) being a risk factor predictor of progression associated with impairment of visual function in glaucoma.

Topical eye-drops remain the cornerstone of treatment for ocular diseases, including glaucoma. Preservatives are added to multi-dose medication bottles to reduce the risk of microbial contamination, to extend the validity of the product and to allow an easily use of the bottles. However the repetitious use of ophthalmic medications containing preservatives, particularly when treating chronic diseases, has been linked to adverse effects and ocular surface disease (OSD).

Methods: prospective study; 76 patients, 102 eyes; preoperative assessment: visual acuity, intraocular pressure, fundus biomicroscopy fluoresceine angiography, sometimes OCT; postoperative assessment: visual acuity, intraocular pressure, fundus biomicroscopy, fluoresceine angiography, sometimes OCT.
Results: Visual acuity improvement in 42% cases; subjective improvement of visual acuity in 72% of cases
Conclusions: Triamcinolon acetonide may play a role in treatment of diabetic macular oedema.
"Red eye" is a sign and the same time a symptom frequently met in the ophthalmological practice. The "red eye" is not always the clinical manifestation of a common conjunctivitis. We will present a case report of a patient who accused conjunctival hyperemia, lacrimation, foreign--body sensation; she was also describing recurrent conjunctivitis for the past year.
