Recombinant Tissue Plasminogen Activator: The New Concept for Dissolving Septae in Loculated Peri-pancreatic Fluid Collection prior to Endoscopic Drainage. A Case Report.

Peri-pancreatic fluid collections are late complications of acute pancreatitis. Loculated peri-pancreatic fluid collections, even rare, remain the "black sheep" in terms of drainage, due to difficulty to puncture all compartments, thus prohibiting proper drainage of all compartments. Recombined tissue plasminogen activator (r-tPA) has been advocated as treatment of the loculated collections, due to its ability to dissolve the fibrinous strands and thus facilitate proper drainage.

[The use of iontophoresis in corneal crosslinking technique].

Iontophoresis is a method of facilitating the penetration of a drug through an intact tissue in the presence of an low intensity electrical current. In corneal crosslinking technique, iontophoresis is used for transepitelial impregnation of cornea with riboflavin. Compared to passive technique of corneal impregnation, iontophoresis shortens the time needed for impregnation, the time of exposure to UVA radiation and does not require de-epithelialisation.

[Unique duodenal hamartomatous polyp--case report].

Duodenal tumors are very rare tumors, with the lower incidence among the tumors of the small bowel, whose frequence is less than 5 % of all digestive tumors. In most of the cases these tumors remain asymptomatic, sometimes the entire life. When they become symptomatic, their first manifestation is the loss of digested blood (melena), secondary anemia and obstructive symptomatology.

[Hypopyon].

The hypopyon represents an anterior chamber exudative syndrome which has important clinical and prognostic implications. It is very important to be able to differentiate between the sterile and infectiouos hypopyon and also to separate it by pseudohypopyon. A clear view over the etiology is strongly correlated with the therapeutical approach and the patient evolution.

[Gastrointestinal stromal tumor with primary hepatic unique location--clinical case].

The gastrointestinal stromal tumors are mesenchymal tumors whose primary extradigestive location is very rare (less than 10% primary liver localization). We present a clinical case of primary hepatic location of GIST in a 28 year-old patient. The discovery of this tumor is a chance, the patient presenting for non-specific dyspeptic syndrome and epigastralgia.

[Adult onset foveomacular vitelliform dystrophy].

We present the case report of a 43 years old patient with adult-onset foveomacular vitelliform dystrophy, a rare disease described for the first time by Gass in 1974. The differential diagnosis was made particularly with Best disease, based on the aspect, the progression of the macular lesions, and the EOG.

[Macular syndrome].

The syndrome consists of signs and symptoms associated in a similar clinical picture, yet induced by different causes. The macular syndrome represents a complex of signs (changes in macular architecture) and symptoms associated with central vision disturbances, obviously induced by different causes. Our paper reviews the macula alteration causes, the clinical elements of this dysfunction and the clinical and paraclinical methods of diagnosis in macular diseases.

[Choroidal neovascularization---physiopathologic theories].

The Age-Related Macular Degeneration is an extremely serious health problem for the elderly people. The therapeutic options are not so effective, but there are a plenty of physiopathologic theories and hypothesis for logical epidemiology and etiology. We are looking through this theoretical aspects and try to identify what is necessary for clinical practice.

[Mini invasive treatment of common biliary duct lithiasis].

The aim of the paper is to show modern techniques of treatment in common biliary duct lithiasis, in the context of the remarkable development of laparoscopic surgery.
