Article Synopsis
  • Anterior-only multilevel cervical decompression and fusion surgery (AMCS) is complex due to potential complications and unclear outcome predictors, particularly in patients with mild to moderate cervical kyphosis.
  • The study analyzed 244 patients undergoing AMCS, identifying key measurements like cervical lordosis (CL) and sagittal vertical axis (cSVA) to assess clinical outcomes.
  • Results showed that improving fusion angle (FA) and cSVA were linked to better outcomes, with higher rates of non-union in patients with worse preoperative alignment; overall, 41% of patients achieved the best outcomes.

Extracellular chromatin, for example in the form of neutrophil extracellular traps (NETs), is an important element that propels the pathological progression of a plethora of diseases. DNA drives the interferon system, serves as autoantigen, and forms the extracellular scaffold for proteins of the innate immune system. An insufficient clearance of extruded chromatin after the release of DNA from the nucleus into the extracellular milieu can perform a secret task of moonlighting in immune-inflammatory and occlusive disorders.

Objective: In double and triple major adolescent idiopathic scoliosis curves it is still controversial whether the lowest instrumented vertebra (LIV) should be L3 or L4. Too short a fusion can impede postoperative distal curve compensation and promote adding on (AON). Longer fusions lower the chance of compensation by alignment changes of the lumbosacral curve (LSC).

Introduction: Dorsal root ganglion (DRG) stimulation demonstrated superiority over traditional spinal cord stimulation with better pain relief and greater improvement of quality of life. However, leads specifically designed for DRG stimulation are difficult to implant in patients who previously underwent spinal surgery and show epidural scarring at the desired site of implantation because of the reduced stiffness of the lead. Nevertheless, recurrent leg or arm pain after spinal surgery usually manifests as a single level radiculopathy, which should theoretically be amenable to DRG stimulation.

Hydrogels are commonly used for the 3D culture of musculoskeletal cells. Sulfated hydrogels, which have seen a growing interest over the past years, provide a microenvironment that help maintain the phenotype of chondrocytes and chondrocyte-like cells and can be used for sustained delivery of growth factors and other drugs. Sulfated hydrogels are hence valuable tools to improve cartilage and intervertebral disc tissue engineering.

BMC Musculoskelet Disord

November 2021

Department of Trauma and Orthopedic Surgery, Klinikum Traunstein, Germany.

Article Synopsis
  • Osteoporotic fractures of the pelvis (OFP) are becoming a significant concern in orthopedics, prompting the need for a new, simpler, and more reliable classification system (CS) that uses both CT and MRI, as existing systems have limited reliability.
  • A consensus among trauma and orthopedic experts in German-speaking countries led to the creation of the new OF-Pelvis CS over five years, which was validated by having experienced surgeons categorize 25 anonymized cases using imaging scans.
  • The new classification has five subgroups indicating increasing instability and three modifiers that can be used alone or in combination; the inter-rater reliability scores are notably high, showing strong agreement among both the developers and the
