Introduction: T cells are involved in the early identification and clearance of viral infections and also support the development of antibodies by B cells. This central role for T cells makes them a desirable target for assessing the immune response to SARS-CoV-2 infection.

Methods: Here, we combined two high-throughput immune profiling methods to create a quantitative picture of the T-cell response to SARS-CoV-2.

Article Synopsis
  • The natural history of occult hepatitis B virus infection (OBI) and its reactivation mechanisms remain partially understood, particularly in people living with HIV (PLWH).
  • A case study of a co-infected patient illustrated that, despite initial normal liver enzymes, OBI can reactivate after antiretroviral therapy (ART) reintroduction, leading to immune reconstitution inflammatory syndrome (IRIS).
  • This highlights the need for heightened vigilance and regular monitoring of HBV markers in immunocompromised patients, even when no hepatitis flare is present.

Antibody-dependent enhancement (ADE) is a complex phenomenon mediated by antibodies, frequently pre-existing non-neutralizing or sub-neutralizing antibodies. In the course of infectious diseases, ADE may be responsible for worsening the clinical course of the disease by increasing the virulence of pathogens (ADE of infection) or enhancing disease severity (ADE of disease). Here we reviewed the mechanisms thought to be behind the ADE phenomenon and its potential relationship with COVID-19 severity.

Article Synopsis
  • Human cytomegalovirus (HCMV) is a major cause of congenital disabilities, prompting a study to assess the rate and risk factors of congenital CMV (cCMV) infection in newborns of immune women.
  • The study involved two parts: the first part analyzed the prevalence of cCMV without any hygiene recommendations, with findings showing a cCMV rate of 0.19% among 9,661 newborns tested.
  • Results indicated that specific factors like twin pregnancies and maternal health conditions are linked to higher cCMV risks, but overall, the low cCMV rate in immune women suggests serological screening might be important to identify those at lower risk for future pregnancies.

Ultrasound Obstet Gynecol

August 2020

Imperial College School of Medicine, Imperial College London and Department of Fetal Medicine, Queen Charlotte's and Chelsea Hospital, Imperial College NHS trust, London, UK.

Objectives: To explore the association between fetal umbilical and middle cerebral artery (MCA) Doppler abnormalities and outcome in late preterm pregnancies at risk of fetal growth restriction.

Methods: This was a prospective cohort study of singleton pregnancies at risk of fetal growth restriction at 32 + 0 to 36 + 6 weeks of gestation, enrolled in 33 European centers between 2017 and 2018, in which umbilical and fetal MCA Doppler velocimetry was performed. Pregnancies were considered at risk of fetal growth restriction if they had estimated fetal weight and/or abdominal circumference (AC) < 10 percentile, abnormal arterial Doppler and/or a fall in AC growth velocity of more than 40 percentile points from the 20-week scan.

Management of acute heart failure in elderly patients.

Arch Cardiovasc Dis

January 2017

INSERM UMR-S 942, Université Paris Diderot, PRES Sorbonne Paris Cité, Paris, France; Department of Anaesthesiology and Critical Care Medicine, AP-HP, Saint Louis Lariboisière University Hospitals, Paris, France.

Acute heart failure (AHF) is the most common cause of unplanned hospital admissions, and is associated with high mortality rates. Over the next few decades, the combination of improved cardiovascular disease survival and progressive ageing of the population will further increase the prevalence of AHF in developed countries. New recommendations on the management of AHF have been published recently, but as elderly patients are under-represented in clinical trials, and scientific evidence is often lacking, the diagnosis and management of AHF in this population is challenging.

In the late 1970s, calcitriol was introduced into clinical practice for the management of secondary renal hyperparathyroidism in chronic kidney disease (CKD). Since then, the use of calcifediol or other native forms of vitamin D was largely ignored until the publication of the 2009 Kidney Disease Improving Global Outcomes (KDIGO) recommendations. The guidelines suggested that measurement of circulating levels of 25(OH)D (calcifediol) and its supplementation were to be performed on the same basis as for the general population.
