Spectrum Health/Michigan State University, General Surgery Residency, Grand Rapids, MI, USA; Spectrum Health Trauma Research Institute, Grand Rapids, MI, USA; Spectrum Health Medical Group, Division of Acute Care Surgery, Grand Rapids, MI, USA.

Introduction: The impact of a visual pain medication schedule on opioid use among hospitalized trauma patients is unknown. We examined whether removal of this displayed schedule would decrease oral morphine equivalent (OME) use.

Methods: This retrospective cohort study compared OME use in trauma patients in the inpatient setting before and after removing the patient-facing pain medication schedule that is typically displayed on the patient's white board for all trauma admissions.

Background: Dog bite injuries are common within the pediatric population. Currently, there are inconclusive data on best sedation practice, antibiotic regimen, and need for plastic surgery referrals for treatment of dog bite injuries in the emergency department (ED) versus operating room (OR). This study set out to determine sedation practice, infection management, and necessity for plastic surgery referral at a level I pediatric trauma center.

Article Synopsis
  • Vanishing lung syndrome (VLS) is a rare form of chronic obstructive pulmonary disease (COPD) primarily affecting middle-aged smokers, as illustrated by a case of a 58-year-old female smoker who developed spontaneous pneumothorax.
  • This patient experienced worsening shortness of breath, leading to the discovery of a right-sided pneumothorax and extensive bullous disease, which was treated with a pigtail catheter.
  • Following treatment, she faced complications including massive subcutaneous emphysema, which was resolved by repositioning the catheter, emphasizing the need for quick diagnosis and management of VLS-related complications.

Bachmann-Bupp syndrome (OMIM #619075) is a novel autosomal dominant disorder caused by variants in the c-terminus of the ornithine decarboxylase 1 gene, resulting in increased levels of ornithine decarboxylase. This case report includes two patients diagnosed with Bachmann-Bupp syndrome who were treated with difluoromethylornithine through compassionate use approval from the United States Food and Drug Administration. In both patients, treatment with difluoromethylornithine has resulted in improved dermatologic signs, including regrowth of eyebrow and scalp hair and cessation of recurrent follicular cyst development.

Article Synopsis
  • Cardiac metastasis from melanoma is uncommon, found in only 1.8% of patients studied, and typically diagnosed only after death.
  • The study analyzed data from patients at the University of Michigan and revealed that most heart metastases affected the right ventricle and led to complications like arrhythmias and heart failure.
  • Patients with cardiac metastasis had a significantly higher 2-year mortality rate (59%) compared to those without, with cancer progression being the main cause of death.

Background: The precise cause of and treatment for capsular contracture remains uncertain, at least partially because there is no reliable quantitative measurement tool. To address this, it is postulated that the surface area of an implant as defined by the surrounding pocket may provide a quantifiable variable that can be measured to evaluate the degree of capsular contracture.

Methods: A bench model for capsular contracture was developed.

Objectives: To evaluate the use of temporary-permanent pacemaker (TPP) in patients with right bundle branch block (RBBB) undergoing transcatheter aortic valve replacement (TAVR). We also sought to identify key predictors of permanent pacemaker (PPM) within 30 days of TAVR in this population.

Background: RBBB is a well-recognized risk factor for PPM post TAVR.

BMI: does it predict the need for component separation?

Hernia

April 2023

Department of Surgery, Spectrum Health Medical Group, 1900 Wealthy St SE Suite 180, Grand Rapids, MI, 49506, USA.

Purpose: Patient optimization and selecting the proper technique to repair large incisional hernias is a multifaceted challenge. Body mass index (BMI) is a modifiable variable that may infer higher intra-abdominal pressures and, thus, predict the need for component separation (CS) at the time of surgery, but no data exist to support this. This paper assesses if the ratio of anterior-posterior (AP): transverse (TRSV) abdominal diameter, from pre-operative CT imaging, indicates a larger proportion of intra-abdominal fat and correlates with a hernia defect requiring a component separation for successful tension-free closure.
