Background: Founder variants are ancestral variants shared by individuals who are not closely related. The large effect size of some of these variants in the context of Mendelian disorders offers numerous precision medicine opportunities.

Methods: Using one of the largest datasets on Mendelian disorders in the Middle East, we identified 2,908 medically relevant founder variants derived from 18,360 exomes and genomes and investigated their contribution to the clinical annotation of the human genome.

This editorial explores the recent advancements in generative artificial intelligence with the newly-released OpenAI o1-Preview, comparing its capabilities to the traditional ChatGPT (GPT-4) model, particularly in the context of healthcare. While ChatGPT has shown many applications for general medical advice and patient interactions, OpenAI o1-Preview introduces new features with advanced reasoning skills using a  processes that could enable users to tackle more complex medical queries such as genetic disease discovery, multi-system or complex disease care, and medical research support. The article explores some of the new model's potential and other aspects that may affect its usage, like slower response times due to its extensive reasoning approach yet highlights its potential for reducing hallucinations and offering more accurate outputs for complex medical problems.

Post-vaccination immune thrombocytopenia (ITP) is a rare but recognized adverse event believed to result from an autoimmune reaction triggered by the vaccine. This case report presents the fourth documented instance of severe ITP requiring splenectomy following the administration of a COVID-19 vaccine. The patient, a 54-year-old previously healthy female with no familial history of autoimmune or hematological disorders, developed ITP two weeks after the first dose of the COVID-19 vaccine.
