Radiologist shortages and lack of access to radiology services are common issues in low- and middle-income countries around the world. Teleradiology offers radiologists an opportunity to contribute to global health and support hospital systems in low-resource regions remotely. Challenges can occur when determining how to integrate the new remote worklist, how radiologists will view and report exams, and how a US host site can ensure safety and privacy across the different systems.

Article Synopsis
  • A myxopapillary ependymoma (MPE) is a rare variant of ependymoma typically found in the conus medullaris or filum terminale, with very few cases documented in the brain, particularly in the fourth ventricle.
  • A 31-year-old woman experienced balance issues and headaches, leading to MRI scans that revealed a tumor in the fourth ventricle, which resulted in obstructive hydrocephalus; surgical removal confirmed the tumor as MPE, classified as WHO grade 2.
  • This case highlights the challenges posed by MPEs due to their tendency to adhere to and infiltrate surrounding neural tissue, impacting treatment outcomes, especially in cases located in the fourth ventricle.

Introduction: Biportal spinal endoscopy is a safe and cost-effective methodology for the management of lumbar pathology in rural underserved hospitals that have standard orthopaedic arthroscopic equipment, but lack access to microscopy.

Methods: This is a case series noting 1-year outcomes from 5 patients managed with biportal spinal endoscopy during an 11-day mission trip to Soddo, Ethiopia in November 2022. Surgical complications, postoperative assessments, and patient-reported outcomes (PROs) were retrospectively collected and analyzed.

Collaborations between orthopaedic training programs in developed countries and international sites in austere environments offer abundant benefits and mutual enrichment. It is often assumed that the exchange is one-sided and we hope to dispel that assumption. Despite the logistical challenges inherent in these partnerships, our experience has been unanimously reviewed as "greatly beneficial" to visiting residents/faculty and surgeons/trainees at the host location.
