Congenital internal carotid artery hypoplasia is a rare condition characterized by underdevelopment or reduced caliber of the internal carotid artery during embryonic development. This anomaly presents significant challenges in management, particularly in neurosurgical interventions for cerebrovascular events. We present a case report of a 67-year-old male who presented with subarachnoid hemorrhage and intraparenchymal hemorrhage extending as intraventricular hemorrhage due to a ruptured left true posterior communicating artery aneurysm, associated with intraoperative findings of left internal carotid artery aneurysm, accompanied by incidental findings of bilateral congenital hypoplasia of the internal carotid artery on computed tomography angiography.

The International Subarachnoid Aneurysm Trial led to a shift from clipping to endovascular coiling as the primary therapy for cerebral aneurysm particularly in the management of posterior circulation aneurysm. However, endovascular therapy is often unavailable in low-resource settings, emphasizing the importance of maintaining surgical skill sets in resource-poor countries. This article presents a detailed case report on the successful microneurosurgical management of a 65-year-old female with a history of headache and weakness with past history of hypertension and a right posterior cerebral artery territory infarct who was diagnosed with a ruptured aneurysm situated within the intracranial vertebral artery.

Clinical documentation of patient visits has changed over the last 20 years, with the increasing use of electronic records causing a seismic shift in how notes are taken. Electronic note-taking aims at reducing the time taken to document a visit, and the introduction of dot phrases, or DotPhrases, in electronic medical records is a step toward reducing the time required to update patients' charts, which might allow doctors to spend more time with their patients. DotPhrases, abbreviated phrases used in the electronic medical record, help in the simplification of note-taking and the standardization of notes.

This study represents the first meta-analysis assessing the impact of symptomatic status on post-ERCP complications and provides critical data to optimize treatment strategies.
