Background: The occurrence of post-traumatic stress disorder (PTSD) following a traumatic event is associated with biological differences that can represent the susceptibility to PTSD, the impact of trauma, or the sequelae of PTSD itself. These effects include differences in DNA methylation (DNAm), an important form of epigenetic gene regulation, at multiple CpG loci across the genome. Moreover, these effects can be shared or specific to both central and peripheral tissues.

Effect of continuous positive airway pressure on non-fatal stroke and paroxysmal atrial fibrillation recurrence in obstructive sleep apnoea elderly patients.

Eur J Intern Med

December 2024

Geriatrics Division, "Renato Dulbecco" University Hospital of Catanzaro, Catanzaro 88100, Italy; Department of Medical and Surgical Sciences, University Magna Græcia of Catanzaro, Catanzaro, Italy.

Background: Obstructive sleep apnoea (OSA) is the most common and clinically significant sleep breathing disorder, with a high prevalence in elderly with cardiovascular diseases . OSA is often under-recognised and under-treated in clinical practice. The aim of this study is to investigate possible differences in major cardiovascular events (MACE) incidence and Paroxysmal Atrial Fibrillation (PAF) recurrence between patients receiving Continuous positive airway pressure (CPAP) treatment versus no CPAP treatment, in a cohort of elderly OSA patients with several comorbidities and history of PAF.

Background: Obstructive sleep apnea is associated with increased dementia risk, yet its role in postoperative neurocognitive disorders is unclear. Here, we studied whether the severity of untreated obstructive sleep apnea is associated with the severity of postoperative neurocognitive disorder.

Methods: In this single-center prospective cohort study, older noncardiac surgery patients aged 60 years and above underwent preoperative home sleep apnea testing, and pre- and postoperative delirium assessments and cognitive testing.

Background: Fatigue is a prevalent yet under-recognized non-motor symptom (NMS) of Parkinson's disease (PD), significantly impacting patients' quality of life. Despite its clinical importance, the relationship between fatigue and other motor and non-motor symptoms remains poorly understood. Its frequent co-occurrence with other NMS further complicates both diagnosis and management, often leading to underdiagnosis and suboptimal treatment.

Background: Sleep pattern (sleep duration, trouble sleeping, sleep disorder) is associated with both energy density dietary inflammatory index (EDII) and depression. However, whether depression mediates the EDII-sleep pattern relationship is unclear.

Methods: 14,305 participants from the National Health and Nutrition Examination Survey (NHANES) from 2005 to 2014 were included in this study.

Article Synopsis
  • The study investigates how sleep disturbances vary among different racial and ethnic groups and their relationship to dementia risk in older adults.
  • It analyzes data from a large cohort including non-Hispanic White, Black, Hispanic, and Asian participants over ten years to track sleep issues and dementia onset.
  • Findings show that Black, Hispanic, and Asian older adults face more sleep problems but use fewer sleep medications, with varying impacts on dementia risk, highlighting the need for more research on these disparities.

Article Synopsis
  • The study aims to examine how factors like physician age, sex, clinical and communication skills, and cultural background influence the prescribing of benzodiazepines (BDZ) to older adults with insomnia.
  • A cohort of international medical graduates (IMGs) treated Medicare patients from 2014-2015, and the analysis showed that U.S.-born IMGs were more likely to prescribe BDZs, particularly those from the U.K. being the least likely.
  • Findings indicate that older physicians tend to prescribe BDZs more often, suggesting that addressing these cultural and age-related factors could help reduce inappropriate medication use.

Article Synopsis
  • - Glucosylceramidase beta 1 (GBA1) mutations increase the risk of Parkinson's disease (PD), but many carriers don't develop symptoms; this study explores factors that may influence PD development in GBA1 carriers.
  • - The research used a database with participants divided into non-manifesting GBA1 carriers, controls, and those with GBA1-associated PD, analyzing exposure to pesticides, smoking, and other factors in relation to PD risk and symptoms.
  • - Findings indicated that pesticide exposure was linked to a higher risk of PD in GBA1 carriers, while smoking seemed to lower that risk; however, early-stage GBA1-associated PD had more severe symptoms than non-manifesting carriers,
