Study Question: Do early- and mid-luteal serum progesterone (P4) levels impact ongoing pregnancy rates (OPRs) in fresh blastocyst transfer cycles using standard luteal phase support (LPS)?

Summary Answer: A drop in serum P4 level from oocyte pick-up (OPU) + 3 days to OPU + 5 days (negative ΔP4) is associated with a ∼2-fold decrease in OPRs.

What Is Known Already: In fresh embryo transfer cycles, significant inter-individual variation occurs in serum P4 levels during the luteal phase, possibly due to differences in endogenous P4 production after hCG trigger and/or differences in bioavailability of exogenously administered progesterone (P) via different routes. Although exogenous P may alleviate this drop in serum P4 in fresh transfer cycles, there is a paucity of data exploring the possible impact on reproductive outcomes of a reduction in serum P4 levels.

Indices of asthma among atopic and non-atopic woodworkers.

Aims: To investigate the relation between wood dust exposure and different indices of asthma among woodworkers and non-exposed subjects.

Methods: A total of 302 woodworkers and 71 non-exposed subjects answered a respiratory health questionnaire, underwent a non-specific bronchial provocation test using the Yan method, and received a skin prick test with 12 common inhalant allergens. Subgroups performed repeated peak flow monitoring and underwent a reversibility test.

Transfer of embryos at the blastocyst stage has been associated with exceptionally high implantation rates. There are, however, only a few prospective randomized studies comparing day 3 versus day 5 embryo transfer. Furthermore, the number of embryos replaced in the day 3 group transfer is often higher than the number of blastocysts replaced, thereby affecting implantation rates.

Musculoskeletal disorders among floor layers: is prevention possible?

Respiratory symptoms and lung function among Danish woodworkers.

In this study, the authors describe a method for evaluation of material emissions. The study was based on chemical analysis of emissions from 23 materials representing solid wood and wood-based materials commonly used in furniture, interior furnishings, and building products in Denmark in the 1990s. The authors used the emission chamber testing method to examine the selected materials with a qualitative screening and quantitative determination of volatile organic compounds.

In semen samples from 94 men from infertile couples and 42 control patients proven to be fertile the occurrence of different kinds of nonspermatozoal cells (NSC) was examined. The findings were evaluated in relation to other seminal parameters and to the fertility status of the couple. The ratios of NSC to sperm and primary spermatocytes to spermatids revealed increasing values with decreasing sperm counts.

Spindle cell lipoma is a distinct type of lipoma characterized by replacement of mature fat by collagen forming spindle cells. It is a benign lesion occurring chiefly in males older than 45 years and affects the subcutaneous tissues in the regions of the shoulder and posterior neck. Deep-seated (intramuscular) spindle cell lipomas are probably uncommon tumors and to date only two cases have been reported in the literature.

Extraocular sebaceous gland carcinomas of the skin are uncommon neoplasms that may recur locally, but in contrast to their ocular counterparts infrequently metastasize. A case is reported in which a 62-year-old woman had a 1 cm scalp tumor incompletely excised. Microscopic examination revealed a sebaceous carcinoma.

Familial spontaneous pneumothorax.

In a three generation family with 27 members we examined a relationship could be found between spontaneous pneumothorax and HLA-haplotypes, alpha 1-antitrypsin phenotypes or concentration or lung volumes and ventilatory capacity. Eight individuals in the family suffered from spontaneous pneumothorax. No relationship with the investigated markers could be found in this informative family.

Rare tumorous lesions involving both parotid glands were found in a 90-year-old man at autopsy. The clinical course held together with the histopathological features of the parotid alterations, make it likely that they present a multifocal adenomatous oncocytic hyperplasia. Areas of clear cells and occurrence of papillary malignant foci lead to various differential diagnostic considerations.
