Chemical Peels.

Facial Plast Surg Clin North Am

November 2023

Plastic Surgery, Bensimon Center, 1200 NW Naito Parkway, Suite 390, Portland, OR 97209, USA. Electronic address:

Facial resurfacing is a fundamental part of rejuvenation but it is often ignored because of the perceived difficulty. Lasers are an option, but they have proved inadequate for difficult rhytids (ie, perioral) both in quality and longevity. Croton oil peels can give excellent results with remarkable permanence.

The occurrence of non-melanoma skin cancer (NMSC) is closely linked with advanced age and ultraviolet-B (UVB) exposure. More specifically, the development of NMSC is linked to diminished insulin-like growth factor-1 (IGF-1) signaling from senescent dermal fibroblasts in geriatric skin. Consequently, keratinocyte IGF-1 receptor (IGF-1R) remains inactive, resulting in failure to induce appropriate protective responses including DNA repair and cell cycle checkpoint signaling.

Recognizing, Managing, and Guiding the Patient Through Complications in Facial Plastic Surgery.

Facial Plast Surg Clin North Am

November 2020

Midwest Facial Plastic Surgery, Woodbury, MN, USA.

Complications in facial plastic surgery can occur in both surgical and nonsurgical procedures. Background: Chemical peels are applied to the face and neck to improve rhytides and the photoaged appearance of the skin. Peels can be applied to different skin depths depending on the types of chemicals, the volume of solution, and the amount of pressure or friction applied. If a peel is applied too superficially, rhytides will not be removed.

Background And Objectives: Actinic keratoses (AK) are common pre-cancerous lesions, which are associated with ultraviolet light exposure and aging. Wounding therapies such as fractionated laser resurfacing (FLR) have been previously demonstrated to effectively treat facial AK. However, the effectiveness of FLR on other sites commonly afflicted with AK has not been studied in detail.

Wounding with a microneedling device corrects the inappropriate ultraviolet B radiation response in geriatric skin.

Arch Dermatol Res

January 2020

Department of Dermatology, Biochemistry and Molecular Biology, and The Herman B Wells Center for Pediatric Research, Indiana University School of Medicine, Indianapolis, IN, USA.

Non-melanoma skin cancer primarily affects geriatric patients as evidenced by the fact that only 20% of these cancers are diagnosed in patients under the age of 60 years. Of importance, geriatric skin responds to procarcinogenic ultraviolet B radiation (UVB) in a manner that permits the establishment of tumor cells. Recent studies have indicated that wounding of geriatric skin with fractionated resurfacing lasers and dermabrasion upregulates fibroblast production of insulin-like growth factor-1 (IGF-1) and normalizes the procarcinogenic acute UVB response consisting of basal keratinocytes proliferating while still harboring unrepaired DNA damage.

Dermatol Clin

July 2019

Department of Dermatology, University of California Davis Medical System, 3301 C Street, #1400, Sacramento, CA 95816, USA. Electronic address:

Scar revision is of premier importance to the dermatologic surgeon. Some of the least invasive modalities include use of silicone gel sheets, resurfacing with electrosurgical instruments, dermabrasion, chemical peels, and subcision. Laser technology also has been implemented to selectively target and ablate fibrous scar tissue via selective thermolysis.

Background: Currently, the standard protocol regarding the performance of procedures on patients receiving or having recently received isotretinoin (13--retinoic acid) states that the procedures should not be performed. The recommendations in standard books and drug insert require discontinuation of isotretinoin for 6 months before performing cosmetic procedures, including waxing, dermabrasion, chemical peels, laser procedures, or incisional and excisional cold-steel surgery. These recommendations have been followed for over two decades despite little evidence for the stated increased risk of scarring.
