Interactions in the tumour microenvironment (TME) are complex and pose a major oncological challenge. Immunotherapy has led to significant progress in recent years, however, not all patients benefit from this. An increasing number of trials try to modulate the TME.

In clinical studies, beneficial and harmful effects of interventions are investigated by measuring predefined outcomes. The uncontrolled choice of these outcomes carries a risk of outcome reporting bias, large heterogeneity of outcomes with reduced options for evidence synthesis. Furthermore, such outcomes may not be important for stakeholders including patients.

Objective: In a setting with an extensive SARS-CoV-2 test strategy and availability of effective vaccines, we aimed to investigate if patients with inflammatory rheumatic diseases (IRD) face greater risk of contracting SARS-CoV-2 and have a worse prognosis of increased risk of hospitalisation, assisted ventilation and death compared with the general population.
Methods: This was a nationwide, population-based register study that compared outcomes of SARS-CoV-2 infection in Danish patients with IRD (n=66 840) with matched population controls (n=668 400). The study period was from March 2020 to January 2023.

This review focuses on ageing and sleep. A key focus in aging is to improve senescence by extending good health, optimal cognitive function, and medical and social assistance into later life. Given that one third of the human lifespan is spent sleeping, the importance of maintaining deep, stable, and consistent sleep is self-evident for the good life quality expected and optimal daytime functioning, which the aging process always curtails.

Evaluation of current medication status is an important part of the assessment in case of complaints of disturbed sleep. Medication may affect sleep continuity and sleep architecture directly via effects on wake or sleep promoting neurotransmitter systems and indirectly via beneficial therapeutic effects or unwanted side effects.

Cancer is a cause of high morbidity and mortality. It is not uncommon for a patient to have more than one primary tumour. This review summarises the knowledge of collision tumours which are defined as two adjacent neoplasms in the same organ, while a collision metastasis is the rare occurrence of two different primary cancers metastasising to the same anatomical site.

This review summarises the current knowledge of whole genome sequencing (WGS) which has become the standard method for genetic characterisation of bacteria in surveillance and outbreak investigation. Although the method offers many advantages, its use in outbreak investigations is hampered by the relatively slow turn-around time. Using new approaches to perform WGS, typing and gene detection can now be completed within one day.

Person-specific evidence was developed as a grounded theory by analyzing 20 selected case descriptions from interventions using the guided self-determination method with people with various long-term health conditions. It explains the mechanisms of mobilizing relational capacity by including person-specific evidence in shared decision-making. Person-specific self-insight was the first step, achieved as individuals completed reflection sheets enabling them to clarify their personal values and identify actions or omissions related to self-management challenges.

Colonoscopy is the golden standard when screening for colorectal cancer, but the colonoscopy quality and the adenoma detection rate (ADR) vary widely among different endoscopists. Artificial intelligence (AI) can reduce performance variability by compensating for perceptual errors. As referred to in this review, several studies have shown that AI-assisted colonoscopy increases ADR significantly.

Surgical stress response includes an inflammatory response with production of enzymes which promote tumour progression and potentially alter tumour microenvironment in colorectal cancer patients. SEMS placement as a "bridge-to-surgery" approach was introduced alternatively to acute resection to improve short-term outcomes and mortality. However, SEMS may lead to inflammation with potential long-term oncological complications.
