E3 ubiquitin ligases have been linked to developmental diseases including autism, Angelman syndrome (UBE3A), and Johanson-Blizzard syndrome (JBS) (UBR1). Here, we report variants in the E3 ligase UBR5 in 29 individuals presenting with a neurodevelopmental syndrome that includes developmental delay, autism, intellectual disability, epilepsy, movement disorders, and/or genital anomalies. Their phenotype is distinct from JBS due to the absence of exocrine pancreatic insufficiency and the presence of autism, epilepsy, and, in some probands, a movement disorder.

Pharmacogenetics promises to optimize treatment-related outcomes by informing optimal drug selection and dosing based on an individual's genotype in conjunction with other important clinical factors. Despite significant evidence of genetic associations with drug response, pharmacogenetic testing has not been widely implemented into clinical practice. Among the barriers to broad implementation are limited guidance for how to successfully integrate testing into clinical workflows and limited data on outcomes with pharmacogenetic implementation in clinical practice.

Article Synopsis
  • - The study evaluated the safety and effectiveness of iltamiocel, an autologous muscle cell therapy, for treating stress urinary incontinence (SUI) in women, comparing it to a placebo group.
  • - Results showed no significant difference in the overall reduction of stress incontinence episodes between groups, but iltamiocel was more effective in women with previous SUI surgeries, indicating potential benefits for this specific population.
  • - While the primary endpoint was not achieved, iltamiocel therapy was deemed safe, suggesting further research is needed for women with prior SUI surgery who lack effective treatment options.

Introduction: Brain glucose hypometabolism is a preclinical feature of Alzheimer's disease (AD). Dietary omega-3 fatty acids promote brain glucose metabolism, but clinical research is incipient. Circulating omega-3s objectively reflect their dietary intake.

Given the proven benefits of screening to reduce diabetic ketoacidosis (DKA) likelihood at the time of stage 3 type 1 diabetes diagnosis, and emerging availability of therapy to delay disease progression, type 1 diabetes screening programs are being increasingly emphasized. Once broadly implemented, screening initiatives will identify significant numbers of islet autoantibody-positive (IAb+) children and adults who are at risk for (confirmed single IAb+) or living with (multiple IAb+) early-stage (stage 1 and stage 2) type 1 diabetes. These individuals will need monitoring for disease progression; much of this care will happen in nonspecialized settings.

Importance: The prevalence, pathophysiology, and long-term outcomes of COVID-19 (post-acute sequelae of SARS-CoV-2 [PASC] or "Long COVID") in children and young adults remain unknown. Studies must address the urgent need to define PASC, its mechanisms, and potential treatment targets in children and young adults.

Observations: We describe the protocol for the Pediatric Observational Cohort Study of the NIH's REsearching COVID to Enhance Recovery (RECOVER) Initiative.

Characterization of non-cardiac arrest PulsePoint activations in public and private settings.

BMC Emerg Med

July 2023

Seattle & King County Public Health, 401 5th Ave, Suite 1200, Seattle, WA, 98104, USA.

Article Synopsis
  • Geospatial smartphone applications are being used to gather community responses for out-of-hospital cardiac arrest (OHCA), but many activations may involve patients who aren’t actually experiencing cardiac arrest.
  • A 3-year study in four U.S. communities tracked cases where this technology was activated for suspected OHCA, revealing that over half of the 1,023 non-arrest patients had various medical conditions like syncope and seizures.
  • Common EMS interventions included placing IV lines and administering naloxone, suggesting that these non-arrest conditions could benefit from quicker care response times.

Article Synopsis
  • - The RECOVER-Pediatrics study aims to investigate the prevalence and long-term effects of Long COVID (PASC) in children and young adults, addressing the need for more research in this area.
  • - The study recruits caregiver-child pairs and young adults across 100+ sites in the U.S., focusing on a diverse group of participants with and without prior SARS-CoV-2 infections, and will collect data over several years through various tiers of assessments.
  • - The ultimate goal of the study is to understand the clinical trajectory, mechanisms, and sociodemographic factors related to pediatric PASC, thereby contributing to potential treatments and public health responses.

The clinical and molecular spectrum of the KDM6B-related neurodevelopmental disorder.

Am J Hum Genet

June 2023

Radboudumc, Department of Human Genetics, Nijmegen, the Netherlands; Center for Neuropsychiatry, Vincent van Gogh, Venray, the Netherlands; Department of Clinical Genetics, ErasmusMC, Rotterdam, the Netherlands. Electronic address:

Article Synopsis
  • De novo variants contribute significantly to neurodevelopmental disorders (NDDs), but due to their rarity, understanding the full range of symptoms and genetic variations linked to specific genes like KDM6B poses a challenge.
  • The study of 85 individuals with KDM6B variants reveals that cognitive deficits are common, while features like coarse facies and skeletal issues are rare, indicating that existing descriptions may be misleading.
  • Through innovative testing methods and studies on Drosophila, the researchers highlight the critical role of KDM6B in cognitive function and the importance of international collaboration for accurate diagnosis of rare disorders.

Best practice guidelines have advanced severe traumatic brain injury (TBI) care; however, there is little that currently informs goals of care decisions and processes despite their importance and frequency. Panelists from the eattle nternational severe traumatic rain njury onsensus onference (SIBICC) participated in a survey consisting of 24 questions. Questions queried use of prognostic calculators, variability in and responsibility for goals of care decisions, and acceptability of neurological outcomes, as well as putative means of improving decisions that might limit care.

Aims: During the coronavirus disease 2019 (COVID-19) pandemic, important changes in heart failure (HF) event rates have been widely reported, but few data address potential causes for these changes; several possibilities were examined in the GUIDE-HF study.

Methods And Results: From 15 March 2018 to 20 December 2019, patients were randomized to haemodynamic-guided management (treatment) vs. control for 12 months, with a primary endpoint of all-cause mortality plus HF events.

Background: The Relay Thoracic Stent-Graft with Plus Delivery System (RelayPlus; Terumo Aortic, Sunrise, Fla) was designed to handle the curvature and tortuosity of the thoracic aorta. It was approved by the Food and Drug Administration in 2012; the postapproval study was stopped early because of adequate safety and efficacy data, and no difference was identified in experienced vs first-time users of RelayPlus. The purpose of this study was to report real-world outcomes of patients with thoracic aortic aneurysms and penetrating atherosclerotic ulcers (PAUs) undergoing thoracic endovascular aortic repair (TEVAR) with RelayPlus.

Novel genetic loci underlying human intracranial volume identified through genome-wide association.

Nat Neurosci

December 2016

Imaging Genetics Center, USC Mark and Mary Stevens Neuroimaging and Informatics Institute, Keck School of Medicine of University of Southern California, Los Angeles, California, USA.

Intracranial volume reflects the maximally attained brain size during development, and remains stable with loss of tissue in late life. It is highly heritable, but the underlying genes remain largely undetermined. In a genome-wide association study of 32,438 adults, we discovered five previously unknown loci for intracranial volume and confirmed two known signals.
