Article Synopsis
  • Singapore successfully managed low local COVID-19 cases through proactive health measures, but experienced a significant surge in imported cases leading to increased community transmission.
  • A case study of a 21-year-old student returning from Nottingham illustrates gaps in international screening, revealing that a sore throat as a symptom may mask COVID-19.
  • To improve border control, it is recommended to broaden symptom screening to include resolved sore throat and a history of recent symptoms, aiding in better detection of potential COVID-19 cases.

A Dynamic Immune Response Shapes COVID-19 Progression.

Electronic address:

The inflammatory response to SARS-coronavirus-2 (SARS-CoV-2) infection is thought to underpin COVID-19 pathogenesis. We conducted daily transcriptomic profiling of three COVID-19 cases and found that the early immune response in COVID-19 patients is highly dynamic. Patient throat swabs were tested daily for SARS-CoV-2, with the virus persisting for 3 to 4 weeks in all three patients.

Background: Congenital diaphragmatic hernia (CDH) is a common birth defect associated with significant mortality and morbidity. There is limited outcome data on CDH in the Southeast Asian region. Rapid accessibility to our CDH Perinatal Center, as a consequence of the small geographic size of our country and efficient land transportation system, has largely eliminated deaths of live outborn babies prior arrival at our center.

Behavioral Characterization of MeCP2 Dysfunction-Associated Rett Syndrome and Neuropsychiatric Disorders.

Methods Mol Biol

Neuroscience and Mental Health Faculty, Lee Kong Chian School of Medicine, Nanyang Technological University, Singapore, Singapore.

The methyl-CpG-binding protein 2 (MECP2) gene has been implicated in multiple neuropsychiatric disorders such as autism and schizophrenia and, most notably, Rett syndrome (RTT). Mouse models of MeCP2 dysfunction that have been developed are thus important not only for examining the protein's contribution to RTT, but also for elucidating the etiologies of other MECP2-associated neuropsychiatric disorders. In this chapter, we present protocols for three behavioral assays for characterizing major functional domains of MeCP2 dysfunction-the open field test for measuring general locomotor activity and anxiety-like behavior, the three-chambered Crawley box test for assessing social preference and social novelty, and the rotarod assay for testing locomotor coordination.

MeCP2 Dysfunction in Rett Syndrome and Neuropsychiatric Disorders.

Methods Mol Biol

Neuroscience and Mental Health Faculty, Lee Kong Chian School of Medicine, Nanyang Technological University, Singapore, Singapore.

Elucidating the functions of a particular gene is paramount to the understanding of how its dysfunction contributes to disease. This is especially important when the gene is implicated in multiple different disorders. One such gene is methyl-CpG-binding protein 2 (MECP2), which has been most prominently associated with the neurodevelopmental disorder Rett syndrome, as well as major neuropsychiatric disorders such as autism and schizophrenia.

Purpose: In order to better time salvage radiotherapy (SRT) for post-radical prostatectomy biochemical failure, we examined the association between pre-SRT prostate-specific antigen (PSA) and PSA control as a function of the new prognostic grade group (PGG) system.

Patients And Methods: Using the Shared Equal Access Regional Cancer Hospital database, we identified men after radical prostatectomy with PSA > 0.2 ng/mL and without cancer involvement of lymph nodes who underwent SRT alone.

Relevance of a TCGA-derived Glioblastoma Subtype Gene-Classifier among Patient Populations.

Sci Rep

Humphrey Oei Institute of Cancer Research, Department of Cellular & Molecular Research, National Cancer Center Singapore, Singapore, Singapore.

Glioblastoma multiforme (GBM), a deadly cancer, is the most lethal and common malignant brain tumor, and the leading cause of death in adult brain tumors. While genomic data continues to rocket, clinical application and translation to patient care are lagging behind. Big data now deposited in the TCGA network offers a window to generate novel clinical hypotheses.

Objective: Objective evaluation of disease activity is challenging in patients with juvenile dermatomyositis (DM) due to a lack of reliable biomarkers, but it is crucial to avoid both under- and overtreatment of patients. Recently, we identified 2 proteins, galectin-9 and CXCL10, whose levels are highly correlated with the extent of juvenile DM disease activity. This study was undertaken to validate galectin-9 and CXCL10 as biomarkers for disease activity in juvenile DM, and to assess their disease specificity and potency in predicting the occurrence of flares.

Prostate focal therapy: the rule or exception?

Curr Opin Urol

November 2018

Department of Urology, Singapore General Hospital, SingHealth Duke-NUS Academic Medical Center, Singapore.

Purpose Of Review: Prostate focal therapy has the potential to preserve urinary and sexual function while eliminating clinically significant cancer in a subset of men with low-volume, organ-confined prostate cancer. This systematic review aims to examine current evidence to determine the efficacy and safety of focal therapy for standard clinical application.

Recent Findings: Focal therapy reduces the rate of cancer progression and conversion to radical therapy in men on active surveillance for prostate cancer.

Introduction: Long-term outcomes from large cohorts are not yet available upon which to base recommended follow-up protocols after prostate focal therapy. This is an updated summary of a 2015 SIU-ICUD review of the best available current evidence and expert consensus on guidelines for surveillance after prostate focal therapy.

Methods: We performed a systematic search of the PubMed, Cochrane and Embase databases to identify studies where primary prostate focal therapy was performed to treat prostate cancer.

Characterization of Pyramidal Tract Shift in High-Grade Glioma Resection.

World Neurosurg

November 2017

Singhealth Duke NUS Academic Medical Center, Neuroscience Duke-NUS Medical School, Singapore City, Singapore; National Neuroscience Institute, Department of Neurosurgery, Singapore City, Singapore.

Objective: This study aimed to characterize pyramidal tract shift in different regions of the brain during glioma resection and its association with head position and tumor location.

Methods: From 2008-2013, 14 patients presenting at the National Neuroscience Institute with high-grade glioma (World Health Organization III or IV) underwent preoperative and intraoperative diffusion tensor imaging. A novel method of placing landmarks along the preoperative and intraoperative tracts, with anterior commissure as the origin, was used to determine pyramidal tract shift.

New advances in focal therapy for early stage prostate cancer.

Expert Rev Anticancer Ther

August 2017

b Division of Urology, Department of Surgery , Duke University Medical Center, Durham , NC , USA.

Prostate focal therapy offers men the opportunity to achieve oncological control while preserving sexual and urinary function. The prerequisites for successful focal therapy are to accurately identify, localize and completely ablate the clinically significant cancer(s) within the prostate. We aim to evaluate the evidence for current and upcoming technologies that could shape the future of prostate cancer focal therapy in the next five years.
