The optimal therapeutic intervention for pediatrics with optic pathway glioma (OPG) remained controversial in the literature. Recently, due to substantial adverse events (AEs) of chemotherapy and its impact on children's lives, the efficacy of other options has been investigated. Bevacizumab (BVZ) is an anti-vascular endothelial growth factor (VEGF) agent that alters the lesion microenvironment.

The growing global prevalence of diabetes mellitus (DM), along with its associated complications, continues to rise. When clinically detected most DM complications are irreversible. It is therefore crucial to detect and address these complications early and systematically in order to improve patient care and outcomes.

Equilibrium radionuclide angiography (ERNA) has long been assumed as the preferred method to assess cardiac volumes as well as left ventricular systolic and diastolic indices. ERNA was used to diagnose subtle changes in cardiac function during chemotherapy or early stages of heart failure. Gated myocardial perfusion SPECT (GMPS) was introduced as a more feasible and versatile alternative to ERNA, but the precision of GMPS to assess systolic and diastolic indices has not yet been fully reviewed.

Background: Regarding the newly diagnosed vestibular schwannomas (VSs), active surveillance, microsurgical resection (MS), and stereotactic radiosurgery (SRS) are the leading treatment options. Although SRS is an effective intervention with a low incidence of complications, failure may occur occasionally. Several options, including repeat SRS, are considered salvage treatment after failure of the SRS.

Clinical Outcomes at 2-Year Follow-Up Comparing Open Surgery and Percutaneous Laser Disc Decompression for Radicular Sciatic Pain Patients.

World Neurosurg

November 2024

Shohada Tajrish Neurosurgical Center of Excellence, Functional Neurosurgery Research Center, Shohada Tajrish Hospital, Shahid Beheshti University of Medical Sciences, Tehran, Iran.

Background: This study aims to compare the clinical outcomes of percutaneous laser disc decompression (PLDD) and open surgery for patients with radicular sciatic pain caused by lumbar disc herniation over a 2-year follow-up period.

Methods: In a prospective randomized controlled trial, 84 patients with chronic radicular pain were assigned to either the open surgery group (n = 42) or the PLDD group (n = 42). Patients were evaluated at baseline, and at 4, 8, 24, 48, and 96 weeks postintervention.

Article Synopsis
  • Primary Hemifacial Spasm (pHFS) is linked to facial nerve compression from blood vessels, with studies showing women are affected about twice as often as men, though there's conflicting evidence regarding which side of the face is more commonly impacted.* -
  • The review examined existing literature to identify anatomical factors that could influence the development of HFS and its gender/side preference, finding that both sides are generally affected equally, but specific vascular anatomical variations may lead to side preferences.* -
  • Key anatomical variations, particularly in posterior circulation, may heighten the risk for HFS, with some indications of a tendency toward the left side, yet there's limited research on the differences between men and women, indicating a need for more

  • A 53-year-old man had a problem where his arm bone (humeral shaft) didn't heal properly after a fracture.
  • He had some other health issues, including an unusual blood vessel connection (arteriovenous fistula).
  • The doctors needed to find a way to fix his arm without doing big surgery or putting him fully under anesthesia because of his health concerns.

  • Stereotactic Radiosurgery (SRS) is a technique that focuses high doses of radiation on specific brain areas while protecting surrounding healthy tissue; this study compares the traditional frame-based method to a newer frameless approach for treating brain metastases.
  • The research followed systematic review protocols and analyzed 499 studies, narrowing it down to 35 for detailed data assessment, yielding promising overall survival and local control rates for patients undergoing frameless SRS.
  • The meta-analysis demonstrated that frameless SRS effectively improves survival and local control for brain metastasis patients, with significant rates such as a 75% 6-month overall survival rate and a 93% local control rate after 6 months.*

Unlabelled: Homozygous variants of Calcium Voltage-Gated Channel Subunit Alpha1 S (CACNA1S) gene mutation were previously identified as causes of periodic paralysis and congenital early-onset myopathy, while it could be manifested as a late-onset congenital core myopathy.

Abstract: Calcium Voltage-Gated Channel Subunit Alpha1 S (CACNA1S) gene mutation has been linked to various neuromuscular conditions in recent years. Congenital myopathy with core-like features is one of the cardinal associations reported previously, causing severe respiratory insufficiency and death in neonates.

  • Cushing's syndrome, particularly Cushing's disease, presents diagnostic challenges that this study aims to address using the desmopressin stimulation test (DST) to differentiate between various causes of ACTH-dependent forms of the syndrome.
  • Twenty-three patients underwent a series of tests, including DST and surgeries, while the effectiveness of DST was measured by its sensitivity, specificity, and other predictive values to assess diagnostic performance.
  • Results indicated that DST demonstrates strong sensitivity and positive predictive value, suggesting it could be a valuable non-invasive method for diagnosing ACTH-dependent Cushing's syndrome with further validation needed for its routine use in clinical settings.

  • The study investigates the effectiveness of Endovascular Thrombectomy (EVT) versus Best Medical Treatment (BMT) for patients with large core ischemic infarcts, analyzing data from six randomized control trials (RCTs) involving 1918 patients.
  • Results show that EVT combined with BMT significantly improves 90-day outcomes, yielding higher rates of favorable results (mRS scores), lower mortality rates, and better neurological recovery compared to BMT alone, although it also increases the risk of symptomatic intracerebral hemorrhage (sICH).
  • Specifically, EVT led to a notable absolute improvement in mRS scores (0-1, 0-2, 0-3) by

Side effects of COVID-19 vaccines among Iranian healthcare workers: a retrospective cohort study.

J Infect Dev Ctries

April 2024

Prevention of Cardiovascular Disease Research Center, Department of Epidemiology, School of Public Health and Safety, Shahid Beheshti University of Medical Sciences, Tehran, Iran.

Article Synopsis
  • This study examined the incidence and severity of side effects from COVID-19 vaccinations among healthcare workers in Iran from March to June 2021.
  • Researchers found that over half (55.85%) of the 42,018 participants reported side effects, with factors like gender, education level, comorbidities, and vaccine type influencing the risk and severity.
  • Overall, while side effects were common, especially with certain vaccines, the study concluded that the benefits of vaccination significantly outweigh the risks, particularly for high-risk groups.

  • The study evaluates the effectiveness of eccentric compression therapy versus traditional compression methods after endovenous laser treatment (EVLT) for varicose veins, focusing on the great saphenous vein (GSV).
  • A total of 88 patients were included, with results indicating that eccentric compression led to lower pain levels and fewer complications, including less ecchymosis, compared to standard compression.
  • Ultimately, the findings suggest that eccentric compression may enhance postoperative recovery and improve patient outcomes following EVLT, prompting further investigation into optimal care methods.
