Investigating and Annotating the Human Peptidome Profile from Urine under Normal Physiological Conditions.

Proteomes

June 2024

Biofluid and Biomarker Center, Kidney Research Center, Graduate School of Medical and Dental Sciences, Niigata University, Niigata 951-2181, Japan.

Examining the composition of the typical urinary peptidome and identifying the enzymes responsible for its formation holds significant importance, as it mirrors the normal physiological state of the human body. Any deviation from this normal profile could serve as an indicator of pathological processes occurring in vivo. Consequently, this study focuses on characterizing the normal urinary peptidome and investigating the various catalytic enzymes that are involved in generating these native peptides in urine.

  • In April 2022, insurance claims for exercise instruction during haemodialysis treatment were approved in Japan, prompting a study on its implementation.
  • A survey sent to 4,257 haemodialysis facilities revealed that 33% offered exercise instruction, with 65% claiming the new fee, focusing on resistance and aerobic exercises.
  • Although 39% of facilities reported adverse events related to exercise therapy, none were life-threatening, indicating a positive trend towards incorporating exercise into haemodialysis treatment.

We describe a case of full-house nephropathy without any underlying disease, including systemic lupus erythematosus. A 40-year-old woman was referred to our hospital with mild proteinuria and microscopic hematuria. The patient was diagnosed with immune complex-mediated glomerulonephritis with a predominant mesangioproliferative pattern based on renal histopathological results using full-house immunofluorescence staining.

Background: Dialysis-related amyloidosis (DRA) is a severe complication in end-stage kidney disease (ESKD) patients undergoing long-term dialysis treatment, characterized by the deposition of β-microglobulin-related amyloids (Aβ2M amyloid). To inhibit DRA progression, hexadecyl-immobilized cellulose bead (HICB) columns are employed to adsorb circulating β-microglobulin (β2M). However, it is possible that the HICB also adsorbs other molecules involved in amyloidogenesis.

Introduction: Cardiac rehabilitation (CR) is strongly recommended as a medical treatment to improve the prognosis and quality of life of patients with heart failure (HF); however, participation rates in CR are low compared with other evidence-based treatments. One reason for this is the geographical distance between patients' homes and hospitals. To address this issue, we developed an integrated telerehabilitation platform, RH-01, for home-based CR.

Article Synopsis
  • A nationwide surveillance study in Japan assessed the antimicrobial susceptibility of bacterial pathogens related to respiratory tract infections, collecting data from June 2019 to December 2020.
  • The study analyzed 932 bacterial strains from 32 medical facilities, finding notable resistance rates, such as 35.3% for methicillin-resistant Staphylococcus aureus and 16.2% for β-lactamase-producing resistant Haemophilus influenzae.
  • Results from this surveillance aim to inform treatment strategies for respiratory infections in Japan and promote the responsible use of antimicrobial drugs.

Background: This multicenter, open-label, single-arm phase II study [Niigata Lung Cancer Treatment Group (NLCTG) 1302] was conducted to evaluate the efficacy and safety of nanoparticle albumin-bound paclitaxel (nab-paclitaxel) monotherapy for previously treated patients with advanced non-small cell lung cancer (NSCLC). We also investigated chemotherapy-induced peripheral neuropathy (CIPN) to evaluate the quality of life (QOL).

Methods: Sixty-five patients with advanced NSCLC from 14 participating institutions who had previously undergone one or two cytotoxic chemotherapy regimens were enrolled in this study.

Article Synopsis
  • * Conducted across multiple centers in Japan, the study involved 194 patients, finding a 90.8% rebiopsy implementation rate and varying success rates based on the type of biopsy: histology (81.3%), cytology (66.7%), and liquid biopsy (43.8%).
  • * Both histological and cytological samples showed a strong agreement (90.9%) in detecting the T790M mutation, suggesting that rebiopsy can enhance mutation detection and inform better treatment decisions.

Various forms of glomerular lesions have been described in primary Sjögren's syndrome (pSjS); however, myeloperoxidase (MPO)-antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis is rarely reported, and the disease onset and clinical course of ANCA-associated vasculitis (AAV) complicated by pSjS are not well understood. A 51-year-old woman was referred to our hospital because of mild proteinuria and microscopic hematuria. She fulfilled the classification criteria for pSjS.

Article Synopsis
  • A study was conducted on elderly heart failure patients (ages 65+) in a Japanese community hospital to evaluate cardiac rehabilitation outcomes.
  • The cohort involved 617 patients, mostly aged 80 and above, indicating a high need for long-term care and support, with significant mortality (41.1%) and rehospitalization (38.6%) rates noted after two years.
  • Results showed that while cardiac rehabilitation improved activities of daily living (ADLs), better ADL scores at discharge were key predictors of better survival rates and lower rehospitalization among these patients.

Brain calcification can be either physiological or pathological. Pathological calcification occurs due to a wide spectrum of causes, including congenital disorders, infections, endocrine/metabolic diseases, cerebrovascular diseases, and neoplasms. The patient's age, localization of the calcification, and association with other imaging findings are useful for the correct diagnosis.

  • The study aimed to investigate the effectiveness of nemolizumab, an anti-IL-31 receptor antibody, in treating uremic pruritus in Japanese hemodialysis patients, comparing it to a placebo and an oral medication, nalfurafine.
  • Patients were divided into multiple groups receiving different doses of nemolizumab or placebo, with the primary goal being to measure changes in itching severity after 4 weeks.
  • The results showed that while some patients noticed improvements in itching, the primary endpoint of significant efficacy was not achieved, although nemolizumab was generally well-tolerated without major safety issues.

Introduction: Dipeptidyl peptidase 4 (DPP4) inhibitors are widely used in patients with type 2 diabetes mellitus (T2DM) on maintenance hemodialysis (HD), but the efficacy of the once-weekly DPP4 inhibitor omarigliptin is not known.

Methods: This prospective, randomized, open-label, parallel-group, non-inferiority/superiority, once-daily DPP4 inhibitor linagliptin-controlled, multicenter study examined glycemic control and safety of omarigliptin (UMIN000024284). Sample size was calculated to confirm non-inferiority in terms of changes in glycated hemoglobin (HbA1c).

The prevalence of quinolone- and macrolide-resistant Group B Streptococcus (GBS) is increasing worldwide, but the relationship between the resistance of GBS to these antibiotics and patient outcome remains unclear. Therefore, we evaluated whether blood stream infection caused by quinolone- or macrolide-resistant GBS is associated with high mortality. Our findings in 77 patients with GBS bacteremia demonstrate that quinolone and macrolide resistance may not be risk factors for 30-day mortality.

The nationwide surveillance on antimicrobial susceptibility of bacterial respiratory pathogens from the patients in Japan was conducted by the Japanese Society of Chemotherapy, the Japanese Association for Infectious Diseases, and the Japanese Society for Clinical Microbiology in 2016. The isolates were collected from clinical specimens obtained from well-diagnosed adult patients with respiratory tract infections during the period between February 2016 and August 2016 by three societies. Antimicrobial susceptibility testing was conducted at the central reference laboratory according to the method recommended by Clinical Laboratory Standards Institute.
