Background: Progressive pulmonary fibrosis (PPF) is a critical concern in interstitial lung disease (ILD) management. The HAL score, which incorporates honeycombing (H), age >75 years (A), and serum lactate dehydrogenase >222 U/L (L), can predict acute exacerbations in patients with idiopathic interstitial pneumonia (IIP). This study aims to evaluate the predictive utility of the HAL score for PPF development.

Carotid artery stenting (CAS) has traditionally been performed using the transfemoral approach (TFA). Recently, the transradial approach (TRA) has gained attention for its lower invasiveness and reduced complication risk. This study compares outcomes between two access strategy timeframes, TFA-first and TRA-first, to evaluate how this shift influences outcomes in a real-world setting.

  • The study investigated the effect of administering P2Y inhibitors during endovascular treatment (EVT) in patients with anterior circulation thrombosis due to acute ischemic stroke.
  • Out of 242 patients analyzed, those who received P2Y inhibitors had a significantly higher rate of carotid artery stenting and better functional outcomes after 90 days compared to those who did not.
  • The results showed that patients treated with P2Y inhibitors had an adjusted odds ratio of 6.65 for better outcomes, suggesting a strong positive association with the use of these medications.

Background: Restenosis after carotid artery stenting (CAS) is associated with the risk of developing ischemic stroke. We aimed to evaluate the inhibitory effect of cilostazol addition on in-stent restenosis (ISR) in patients treated with CAS.

Methods: In a randomized, open-label, blind-end point trial, patients with symptomatic and asymptomatic carotid artery stenosis and scheduled for CAS were randomly assigned to adding cilostazol (50 or 100 mg, twice per day) on other antiplatelets from 3 days before CAS or not adding cilostazol.

Background: The addition of antiplatelet therapy to anticoagulant therapy in patients with stroke with non-valvular atrial fibrillation (NVAF) and atherothrombotic disease may increase bleeding risk without reducing recurrent stroke risk.

Aims: To evaluate the clinical benefits of anticoagulant monotherapy compared to combination therapy with anticoagulants and antiplatelet agents.

Methods And Design: This is an investigator-initiated prospective multicenter, randomized, open-label, parallel-group clinical trial.

Objective: Rupture of cerebral aneurysms has a poor prognosis, and growing aneurysms are prone to rupture. Although the number of coil embolization procedures is increasing worldwide, they are more prone to recurrence than clipping surgeries. However, there is still no drug that prevents aneurysm growth or recanalization after coil embolization.

Article Synopsis
  • * Methods: A total of 24 healthy participants underwent a randomized controlled crossover study where they received either HICUS or a placebo on different days, with IFP stiffness and gliding measured at various angles of knee flexion before, immediately after, and 15 minutes post-treatment.
  • * Results: HICUS significantly reduced IFP stiffness at 10 degrees knee flexion and improved gliding in participants, with beneficial effects observed shortly after treatment; however, no significant changes were

  • This study evaluated the safety and effectiveness of the Versi Retriever for mechanical thrombectomy in patients suffering from acute ischemic stroke (AIS) at 10 hospitals in Japan.
  • It involved 58 patients, showing that 62% achieved a good recovery (mRS 0-2) after 90 days, and 100% had successful blood flow restoration.
  • The procedure had an 8% mortality rate within 90 days and a significant occurrence of intracranial hemorrhage, indicating that while effective, some risks are involved.

  • Immune-related pneumonitis (irP) is a significant side effect of immune checkpoint inhibitors, often relapsing after corticosteroid treatment, which complicates cancer therapy.* -
  • A study analyzed 56 patients with irP and found that 39.3% experienced relapses post-prednisolone therapy; significant risk factors included a radiographic pattern of organizing pneumonia and an onset of irP more than 100 days after starting treatment.* -
  • Identifying these risk factors is crucial for better managing irP in patients undergoing treatment, as it could lead to improved outcomes.*

  • This study investigates interstitial pneumonia with autoimmune features (IPAF) in patients with idiopathic interstitial pneumonia (IIP), aiming to identify serum biomarkers related to autoimmune characteristics and prognosis.
  • An analysis of 222 newly diagnosed IIP patients over three years revealed that high serum levels of CXCL10 were significantly associated with better lung function metrics and acute onset of symptoms, as well as a higher likelihood of IPAF classification.
  • The findings suggest that CXCL10 could serve as a valuable biomarker for evaluating the clinical course of IIP patients, highlighting its potential role in understanding disease progression and patient outcomes.

  • Endovascular treatment (EVT) shows promise for addressing extracranial vertebral artery (VA) stenosis, but optimal procedures and efficacy are still unclear.
  • A study analyzed data from 308 patients in Japan who underwent EVT, focusing on procedural success and outcomes at 30 days post-treatment.
  • Results indicated a high procedural success rate (98%) and functional independence (80%) without significant differences based on type of treatment used, highlighting EVT's potential but also the need for additional research.

Background And Objectives: Endovascular treatment (EVT) for medium vessel occlusion (MeVO) raises concern about hemorrhagic complications; however, its clinical impact has not been elucidated. Therefore, we investigated the association between intracranial hemorrhage (ICH) after EVT for MeVO and functional outcomes.

Methods: We conducted a post hoc analysis of the Japan Registry of NeuroEndovascular Therapy 4, a nationwide registry in Japan from 2015 to 2019 including 13 479 patients who underwent EVT for acute ischemic stroke.

  • * Results showed that AF patients had more severe initial strokes (higher NIHSS scores) and less favorable outcomes at discharge, although adjustments indicated some improvement for AF patients over time.
  • * Over the study period, AF patients experienced a noticeable decrease in stroke severity and an increase in favorable outcomes, suggesting that advances in stroke care benefits AF patients more than non-AF patients.

Background: The annual administration of the influenza vaccine is the most effective method for preventing influenza. We have evaluated the effectiveness of the inactivated influenza vaccine in children aged 6 months to 15 years across the seasons from 2013/2014 to 2022/2023. This study aims to investigate the effectiveness of the inactivated influenza vaccine in the 2023/2024 season, the first year following the easing of strict COVID-19 measures, and possibly the last season when only the inactivated vaccine is available on the market.

  • Enteral nutrition is crucial for stroke patients who struggle with eating, but traditional feeding methods are time-consuming and can disrupt care.
  • The Rapid EN trial aims to compare the safety of quickly administering enteral nutrition (100 mL in 5 minutes) versus the conventional method (100 mL in 30 minutes) in acute stroke patients.
  • The study will track complications like vomiting and pneumonia within 7 days, assessing if the faster method is equally safe while also looking at overall treatment times and clinical outcomes.

Purpose: We evaluated the efficacy and safety of antiemetic therapy with olanzapine, a neurokinin-1 receptor antagonist (RA), a 5-hydroxytryptamine-3 (5-HT) RA, and dexamethasone for preventing chemotherapy-induced nausea and vomiting in patients receiving carboplatin-containing chemotherapy.

Patients And Methods: Chemotherapy-naïve patients scheduled to receive carboplatin (AUC ≥5) were randomly assigned to receive either olanzapine 5 mg once daily (olanzapine group) or placebo (placebo group) in combination with aprepitant, a 5-HT RA, and dexamethasone. The primary end point was the complete response (CR; no vomiting and no rescue therapy) rate in the overall phase (0-120 hours).
