Functional and nutritional outcomes after gastric cancer surgery.

Transl Gastroenterol Hepatol

April 2020

Department of Surgery, Saiseikai Shiga Hospital, Ritto City, Japan.

Recent improvements in diagnostic techniques and national screening programs have resulted in increasing number of patients diagnosed with early gastric cancer (EGC). The low incidence rate of lymph node metastasis and excellent survival rates after surgical treatment for EGC enabled the reduction in the extent of lymphadenectomy and the range of gastric resection for function-preserving gastrectomy. Thus, the quality of life (QOL) of patients with gastric cancer (GC) in the curative stage can be maintained.

Article Synopsis
  • The study aims to evaluate the type of mucosa at the incisura angularis in the normal stomach of individuals who have never been infected by Helicobacter pylori, which is important for understanding mucosal atrophy and cancer risk.
  • Researchers recruited 270 subjects aged 40 and older and classified the mucosal types based on biopsy samples, using strict criteria to determine a "H. pylori-naïve normal stomach."
  • The findings reveal that at the incisura angularis, the mucosa was primarily fundic (50%) or transitional (50%), while pyloric mucosa was not present, suggesting a different mucosal classification in these cases.

An early diastolic flow from the left ventricular apex to the base can be shown in patients with hypertrophic cardiomyopathy (HCM). This tiny flow or a diastolic paradoxic jet flow is important to detect on echocardiography because of its association with cardiovascular adverse events. We report an asymptomatic 44-year-old man with mid-ventricular obstructive HCM, in which a diastolic paradoxic jet flow was observed not only in the left ventricle but also in the right ventricle.

Background: The Osborn wave (OW) is often observed in hypothermic patients; however, whether OW in hypothermic patients is related to the development of fatal ventricular arrhythmia, including ventricular fibrillation (VF) and pulseless ventricular tachycardia (VT), remains undetermined. This study aimed to estimate the association between OW and the incidence of fatal ventricular arrhythmias.

Methods and results: This retrospective study used the Japanese Accidental Hypothermia Network registry database and included 572 hypothermic patients.

Background: Accidental hypothermia (AH) is defined as an involuntary decrease in core body temperature to < 35 °C. The management of AH has been progressing over the last few decades, and numerous techniques for rewarming have been validated. However, little is known about the association between rewarming rate (RR) and mortality in patients with AH.

May 2019

1Department of Primary Care and Emergency Medicine, Graduate School of Medicine, Kyoto University, 606-8501, Yoshidakonoe-cho, Sakyo, Kyoto, Japan.

Background: Accidental hypothermia is a serious condition that requires immediate and accurate assessment to determine severity and treatment. Currently, accidental hypothermia is evaluated using the Swiss grading system which uses core body temperature and clinical findings; however, research has shown that core body temperature is not associated with in-hospital mortality in urban settings. Therefore, we developed and validated a severity scale for predicting in-hospital mortality among urban Japanese patients with accidental hypothermia.
