Graduate School of Media and Governance, Keio University, 5322 Endo, Fujisawa City, Kanagawa Prefecture, 252-0882, Japan. Electronic address:

The adoption of residential renewable energy is pivotal for achieving the 'Net Zero' goal, yet financial assessments of household investments in this area remain complex due to dynamic market conditions. This study introduces a novel closed-form financial valuation framework for residential solar photovoltaic (PV) systems, explicitly addressing the uncertainties of electricity market price fluctuations (market risk) and energy policy changes (policy risk) using Geometric Brownian Motion (GBM). A case study in France demonstrates the framework's application, revealing that the discount rate is the most influential factor in solar PV valuation, followed by system lifespan and policy-driven Feed-in Tariff (FiT) rates.

Post-endoscopic mucosal resection (EMR) bleeding, or clinically significant post-EMR bleeding, is influenced by factors such as polyp size, right-sided colonic lesions, laterally spreading tumors, anticoagulant use, and comorbidities like cardiovascular or chronic renal disease. The optimal prophylactic therapy for post-EMR bleeding remains unknown, with no consensus on specific criteria for its application. Moreover, prophylactic measures, including clipping, suturing, and coagulation, have produced mixed results.

Objectives: This study aimed to evaluate the cost-effectiveness of an extended-role general practitioner symptoms clinic (SC), added to usual care (UC) for patients with multiple persistent physical symptoms (sometimes known as medically unexplained symptoms).

Methods: This was a 52-week within-trial cost-utility analysis of a pragmatic multicenter randomized controlled trial comparing SC + UC (n = 178) with UC alone (n = 176), conducted from the primary perspective of the UK National Health Service and personal and social services (PSS). Base-case quality-adjusted life-years (QALYs) were measured using EQ-5D-5L.

Flexor sheath infections require prompt diagnosis, and management with intravenous antibiotics and/or surgical washout followed by hand therapy. Complication rates as high as 38% have been reported. Our unit takes a relatively conservative approach to the management of flexor sheath infections and select patients are managed non-surgically via our outpatient antibiotic service where they are clinically reviewed and receive a once daily dose of intravenous antibiotics.

CURE Group, Sheffield School for Health and Related Research (ScHARR), Faculty of Health, University of Sheffield, Sheffield, UK.

Article Synopsis
  • The study aimed to assess the long-term prevalence and risk factors of depression symptoms in individuals 10 years after a traumatic brain injury (TBI), comparing it with rates found at 10 weeks and 1 year post-injury.
  • Out of 1130 TBI patients initially recruited, follow-up at 10 years showed a decrease in depression prevalence from 56.3% at 10 weeks to 38.4%, though individual scores varied widely.
  • Key risk factors for long-term depression included lower Glasgow Coma Scale (GCS) scores, social deprivation, being female, having a history of psychiatric issues, alcohol use, and unemployment, while factors like age and ethnicity had no

Article Synopsis
  • Over the last decade, microbiome research has uncovered that certain microorganisms are unevenly distributed across different geographical areas, prompting inquiries into the movements and sources of these microorganisms and associated antimicrobial resistance genes.
  • Current biogeographical tools have limitations in resolution and accuracy for predicting patterns relevant to ecological and health-related fields, which makes understanding biodiversity challenging.
  • A new machine learning tool called microbiome geographic population structure was developed to precisely identify the sources of microorganisms, achieving high accuracy in tracing samples for urban, soil, and marine ecosystems, and offering valuable applications in fields like forensics, medicine, and epidemiology.

Background: Lung cancer is the most common cause of cancer death in the UK resulting in 21% of all cancer deaths. In 2016, local lung cancer surgery services required improvement due to under-representation in cancer resections and resource scarcity during the pandemic, which affected critical care bed availability and extended postoperative stays. The aim of this service improvement was to increase the number of lung cancer resection; develop minimally invasive techniques and reduce the use of Critical Care Unit beds by 35% (a subsequent goal).

Extending mechanical thrombectomy service provision to 24/7: a break-even analysis.

BMC Health Serv Res

August 2024

Department of Economics and Management, University of Trento, Via Vigilio Inama n. 5, Trento, 38122, Italy.

Background: Comprehensive stroke centres across England have developed investment proposals, showing the estimated increases in mechanical thrombectomy (MT) treatment volume that would justify extending the standard hours to a 24/7 service provision. These investment proposals have been developed taking a financial accounting perspective, that is by considering the financial revenues from tariff income. However, given the pressure put on local health authorities to provide value for money services, an affordability question emerges.
