Article Synopsis
  • Sexually transmitted infections (STIs) are a major global health issue, and early diagnosis can help reduce transmission; thus, a deep learning model was created to differentiate STIs from non-STIs using clinical images and metadata.
  • The study utilized 4913 clinical images collected over 13 years, developing two models: one relying solely on images and another that combined images with demographic and clinical metadata.
  • The integrated model demonstrated better performance with an AUC of 0.893 compared to the image-only model's 0.859, indicating that including metadata significantly improves the accuracy of STI diagnoses.

First-line treatment with ceftriaxone for infection less likely to be prescribed to patients with a penicillin allergy label: a retrospective audit of medical records.

Sex Health

July 2024

Canberra Sexual Health Centre, Canberra Health Services, Garran, ACT, Australia; and School of Medicine and Psychology, Australian National University, Canberra, ACT, Australia.

Background Gonorrhoea notifications have increased substantially in Australia over the past decade. Neisseria gonorrhoeae is already highly resistant to several antibiotics and so, alternatives to first-line treatment are generally strongly discouraged. The penicillin allergy label (AL) on patient medical records has previously been shown to influence prescribing practices, to the detriment of best-practice management and antimicrobial stewardship.

Randomized controlled trial reveals no benefit to a 3-month delay in COVID-19 mRNA booster vaccine.

J Clin Invest

July 2024

Department of Microbiology and Immunology, Peter Doherty Institute for Infection and Immunity, University of Melbourne, Melbourne, Victoria, Australia.

Article Synopsis
  • A study investigated the timing of bivalent mRNA COVID-19 booster vaccinations in highly vaccinated adults, comparing immediate vs. a 3-month delayed administration.
  • The findings showed no significant difference in immune response (antibody levels and effectiveness against variants) between the two groups.
  • The results suggest that delaying booster shots does not provide any additional benefits in enhancing immunity during the current endemic phase of the virus.

The Victorian Government introduced a time-limited human papillomavirus (HPV) catch-up program for gay, bisexual, and other men who have sex with men (GBMSM) aged ≤ 26 years in 2017-2019. We conducted a retrospective observational study to examine the accuracy of the self-report of HPV vaccination status using computer-assisted self-interviewing versus their immunization history via electronic health records. We included GBMSM aged 23-30 years visiting the Melbourne Sexual Health Centre (MSHC) in 2020-2021 because they were age-eligible for the HPV catch-up program in Victoria, Australia.

Background: In Australia the incidence of HIV has declined steadily, yet sustained reduction of HIV transmission in this setting requires improved public health responses. As enhanced public health responses and prioritisation of resources may be guided by molecular epidemiological data, here we aimed to assess the applicability of these approaches in Victoria, Australia.

Methods: A comprehensive collection of HIV-1 sequences from individuals diagnosed with HIV in Victoria, Australia, between January 1st 2000 and December 31st 2020 were deidentified and used as the basis of our assessment.

Article Synopsis
  • A national program integrated molecular point-of-care (POC) testing for chlamydia, gonorrhoea, and trichomonas in 49 primary care clinics in Australia to improve diagnostic access and treatment speed for First Nations peoples.
  • Between 2016 and 2022, the program showed a significant increase in monthly testing, and treated a higher percentage of patients within two to seven days compared to previous practices, resulting in thousands of infectious days averted.
  • The evaluation revealed high test concordance rates and low failure rates, indicating that the POC testing was effective and of high quality, contributing to both individual health benefits and potentially reducing disease transmission in First Nations communities.

A typology of HIV self-testing support systems: a scoping review.

Sex Health

July 2024

Melbourne Sexual Health Centre, Alfred Health, Melbourne, Vic, Australia; and Central Clinical School, Faculty of Medicine, Nursing and Health Sciences, Monash University, Melbourne, Vic, Australia; and Department of Communicable Diseases, World Health Organization Regional Office for the Eastern Mediterranean, Cairo, Egypt.

To maximise the benefits of HIV self-testing (HIVST), it is critical to support self-testers in the testing process and ensure that they access appropriate prevention and care. To summarise systems and tools supporting HIVST (hereafter, 'support systems') and categorise them for future analysis, we synthesised the global data on HIVST support systems and proposed a typology. We searched five databases for articles reporting on one or more HIVST support systems and included 314 publications from 224 studies.

Influenza exposures early in life are believed to shape future susceptibility to influenza infections by imprinting immunological biases that affect cross-reactivity to future influenza viruses. However, direct serological evidence linked to susceptibility is limited. Here we analysed haemagglutination-inhibition titres in 1,451 cross-sectional samples collected between 1992 and 2020, from individuals born between 1917 and 2008, against influenza B virus (IBV) isolates from 1940 to 2021.

Introduction: Increased awareness of testicular diseases can lead to early diagnosis. Evidence suggests that men's awareness of testicular diseases is low, with many expressing their willingness to delay help-seeking for symptoms of concern. Economic evaluation alongside a clinical trial of near-to-patient testing for sexually transmitted infections.

J Infect Public Health

July 2024

School of Translational Medicine, Monash University, Melbourne, Victoria, Australia; Melbourne Sexual Health Centre, Alfred Health, Melbourne, Victoria, Australia. Electronic address:

Background: Current clinical care for common bacterial STIs (Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG) and Mycoplasma genitalium (MG)) involves empiric antimicrobial therapy when clients are symptomatic, or if asymptomatic, waiting for laboratory testing and recall if indicated. Near-to-patient testing (NPT) can improve pathogen-specific prescribing and reduce unnecessary or inappropriate antibiotic use in treating sexually transmitted infections (STI) by providing same-day delivery of results and treatment.

Methods: We compared the economic cost of NPT to current clinic practice for managing clients with suspected proctitis, non-gonococcal urethritis (NGU), or as an STI contact, from a health provider's perspective.

  • The 'Undetectable=Untransmittable' (U=U) campaign, launched in 2016, aims to empower individuals with HIV by reducing stigma and promoting access to treatment, highlighted during a global roundtable at the 2023 International AIDS Society Conference.
  • Experts noted a disparity in U=U knowledge and trust, predominantly among those living with HIV in certain networks, emphasizing the necessity of inclusive U=U policies for all priority populations.
  • To maximize the benefits of U=U for mental and physical health, there's an urgent call to tackle barriers in HIV care access and foster collaboration between policymakers and community organizations.
