Instituto de Ciencias Aplicadas y Tecnología (ICAT), Universidad Nacional Autónoma de México (UNAM), Ciudad de México, México.

In laparoscopy, the absence of ergonomics in the instruments affects the performance and efficiency of the surgeon, increasing the likelihood of developing musculoskeletal injuries. This article presents the development of a laparoscopic needle holder with an ergonomic handle and the experience in its use with surgeons in the intracorporeal suturing task. The handle of the laparoscopic needle holder consists of a semi-spherical piece that easily adapts to the palm of the surgeon's hand and improves the posture and ergonomics of the wrist, allowing the direct transmission of rotational movements around the longitudinal axis of the instrument towards the tip.

Background: The mitroaortic intervalvular fibrosa is an avascular structure near the left ventricular outflow tract, between the mitral and aortic valves. Mitroaortic intervalvular fibrosa complications, such as tamponade, hemopericardium, and abscesses, are rare and often diagnosed postmortem. On the other hand, the COVID-19 pandemic notably impacted pediatric patients with congenital heart diseases, who frequently presented cardiac complications including arrhythmias, elevated troponins, myocarditis, and heart failure.

Unlabelled: People with type 2 diabetes mellitus (DM2) experience difficulties in self-management. One of the variables involved is the low level of health literacy (HL) which corresponds to the degree to which people access, understand, evaluate and use information to engage with health demands in different contexts, in order to promote and maintain good health.

Hypothesis: People diagnosed with diabetes mellitus who participate in an intervention to increase their level of health literacy improve their metabolic compensation measured in HBA1c.

Targeting Soluble Amyloid Oligomers in Alzheimer's Disease: A Hypothetical Model Study Comparing Intrathecal Pseudodelivery of mAbs Against Intravenous Administration.

Diseases

January 2025

Departamento de Medicina, Facultad de Ciencias de la Salud, Universidad de Oviedo, ES-33006 Oviedo, Spain.

Background/objective: Neurotoxic soluble amyloid-β (Aβ) oligomers are key drivers of Alzheimer's pathology, with evidence suggesting that early targeting of these soluble forms may slow disease progression. Traditional intravenous (IV) monoclonal antibodies (mAbs) face challenges, including limited brain penetration and risks such as amyloid-related imaging abnormalities (ARIA). This hypothetical study aimed to model amyloid dynamics in early-to-moderate Alzheimer's disease (AD) and compare the efficacy of IV mAn with intrathecal pseudodelivery, a novel method that confines mAbs in a subcutaneous reservoir for selective amyloid clearance in cerebrospinal fluid (CSF) without systemic exposure.

Objective: The objective of this study was to examine the effect of observing actions at different speeds on the speed of motor task performance in subsequent actions.

Methods: Sixty individuals, divided equally between those with non-specific chronic low back pain (NSCLBP) and asymptomatic subjects, were enrolled. Participants were further split into subgroups to observe lumbar flexion and Timed Up and Go (TUG) test actions at either a slow or fast pace, following a randomized assignment.

Introduction: Polycystic Ovary Syndrome (PCOS) affects 5-20% of reproductive-aged women. Insulin resistance (IR) is common in PCOS with consequent elevated risks of metabolic disorders and cardiovascular mortality. PCOS and obesity are complex conditions associated with Metabolic Syndrome (MS), contributing to cardiovascular disease and type 2 diabetes mellitus (T2D).

Resting state electroencephalography (EEG) has proved useful in studying electrophysiological changes in neurodegenerative diseases. In many neuropathologies, microstate analysis of the eyes-closed (EC) scalp EEG is a robust and highly reproducible technique for assessing topological changes with high temporal resolution. However, scalp EEG microstate maps tend to underestimate the non-occipital or non-alpha-band networks, which can also be used to detect neuropathological changes.

Introduction: The Community Pharmacy Survey on Patient Safety Culture (CPSOPSC) is a tool created by the Agency for Healthcare Research and Quality and used in the United States to assess the patient safety culture among community pharmacy workers. This survey has been adapted for use in hospital pharmacies in other countries. However, it has not yet been implemented in Spanish hospital pharmacies due to the lack of an applicable version in Spain.

Local resection in rectal cancer: when, who and how?

Cir Esp (Engl Ed)

January 2025

Unidad de Coloproctología, Hospital Universitario Parc Tauli, Sabadell. Institut d'investigació i innovació Parc Tauli I3PT-CERCA, Department of Surgery, Universitat Autònoma de Barcelona. Electronic address:

Local resection (LR) in rectal cancer is indicated in stage T1N0M0 without unfavorable pathological factors, achieving oncologically satisfactory outcomes through transanal endoscopic surgery techniques. However, the initial step involves accurate staging and selection of these tumors through specific tests conducted in specialized colorectal units. For T2N0M0 tumors and T1 tumors with poor prognostic factors, the standard treatment is total mesorectal excision (TME), a procedure associated with high postoperative morbidity and mortality, functional impairments, and reduced quality of life.

Background And Objective: Complicated urinary tract infections (cUTIs) are serious, potentially life-threatening infections that occur in patients with an increased disease progression risk. Antimicrobial resistance represents an important health issue worldwide, contributing to relapses, which can generate further resistances. It is necessary to clarify the role of microbiological eradication as an additional objective in the management of cUTIs.

New COVID-19 vaccination recommendations in Spain: Optimizing for next seasons.

Enferm Infecc Microbiol Clin

January 2025

Centro Nacional de Gripe de Valladolid (GISRS/WHO), Spain.

Despite high initial vaccination rates, Spain's current COVID-19 vaccination coverage in recommended groups does not meet WHO targets. For the upcoming season, challenges include revising vaccination age, updating risk groups, and unifying criteria with flu vaccine co-administration. European Commission's advance purchase agreements limit access to certain vaccines, and the need for vaccines effective against current variants adds administrative complexities.

The AEP 2025 Vaccination and Immunization Schedule recommended for children, adolescents and pregnant women residing in Spain features the following novelties: Due to the increase in measles cases and outbreaks in recent years, we recommend advancing the second dose of measles, mumps and rubella (MMR) vaccine to 2 years of age. As a consequence of the above, since many autonomous communities (ACs) use the quadrivalent vaccine for the second dose of MMR and varicella vaccines, we recommend, for all ACs, advancing the second dose of varicella vaccine to 2 years of age. Due to the very significant increase in cases of pertussis since late 2023 and especially in 2024, we recommend advancing the dose of Tdap given in adolescence to 10-12 years of age.

Objectives: To evaluate the impact of hospitalisation for infectious diseases on the Health-Related Quality of life (HRQOL), multidimensional frailty, and functioning of older patients, we conducted a longitudinal matched cohort study in four European countries.

Methods: HRQOL, frailty, and functioning were assessed using validated questionnaires at inclusion, at discharge, and up to six months later (M6) in patients aged over 65 years hospitalised for severe acute respiratory or bloodstream infections, and matched controls hospitalised for non-infectious conditions. Comparative analyses employed multilevel mixed-effect linear or logistic models to assess changes from inclusion.
