Psychogeriatric care in times of COVID. Lessons learned and proposals for similar situations.

The objective is to describe the problems related to outpatient psychogeriatric care in the context of the SARS-CoV-2 pandemic, as well as the proposed and implemented solutions for optimizing care for elderly people with mental disorders during the pandemic, that can also be applied in emerging similar situations in the future.

Cerebellar gangliocytoma as a cause of fluctuating hearing loss.

Cerebellar lesions could be a cause of fluctuating hearing loss, due to the increase of intracranial pressure by partially or complete blocking of the Foramen of Luschka. Patients with intracranial hypertension may present audio-vestibular symptoms. Fluctuating sensorineural hearing loss may be the manifestation of different inner ear disorders such as Meniere's disease (MD), immune-mediated inner ear disease (IMIED), otosyphilis, and labyrinthine fistula including semicircular canal dehiscence.

[Infections in immunocompromised patients (II). The transplanted patient].

Recipients of the different types of transplantation are at high risk of infection. Nosocomial infections predominate in patients who have undergone haematopoietic stem cell transplantation during the early post-engraftment period (30-100 days after the infusion); the incidence of infection is higher in graft-versus-host disease. Nodular lesions can appear in the lungs due to invasive fungal infection.

[Cost Effectiveness of Treatments of Psoriasis with a PASI 75 and one Period of 12 Weeks].

Objective: The objective was to evaluate the efficiency (relation between the cost and the results in health) of the treatments in psoriasis, seeking a higher quality of economic evaluations, consistency and transparency in these studies.

Methods: We developed a model of economic evaluation in psoriasis collecting all the many direct and indirect costs of each treatment. The effectiveness indicator used was Psoriasis Area Severity Index [PASI 75] which is generally acceptable in studies of psoriasis.

Cost analysis of the adverse reactions of bipolar disorder treatment with aripiprazole and olanzapine in Spain.

Objective: This study investigates the healthcare costs of adverse events (AE) associated with treatment of bipolar disorder with two atypical oral antipsychotics (AOA): aripiprazole (ARI) and olanzapine (OLA).

Methods: A cost analysis using a Markov model considering the following health states was performed: no existence of adverse events (NAE); extrapyramidal symptoms (EPS); weight gain (WG); and sexual dysfunction (SD). Transition probabilities amongst health states were estimated from meta-analyses of clinical trials and from a retrospective Spanish study.

Development of a program for kidney transplants using organs donated from donors awaiting cardiac arrest (type III Maastricht).

Introduction: The availability of organ donors is a limiting factor for kidney transplants. Donations from non-heart-beating donors (NHBD) can provide as many as one-third of all organs. Controlled patients awaiting cardiac arrest following limitation of life support techniques, or type III Maastricht donors, constitute an alternative that still has yet to be systematically developed.

[Endovascular treatment of long-term complications following surgical repair of aortic coarctation].

Surgical treatment of aortic coarctation has a high success rate. However, irrespective of the surgical technique used, a significant percentage of patients develop late complications affecting the aortic wall. Reoperation to repair these complications is a complex procedure and is associated with high mortality.

[Cardiac myxoma: an analysis of 30 patients].

Introduction: Myxomas are the most common type of benign heart tumors. The aim of this study was to correlate the clinical forms of presentation of cardiac myxoma and complementary laboratory results with the morphological features of the tumor.

Materials And Methods: We reviewed retrospectively a total of 30 cardiac myxomas seen in 2 institutions after a period of 22 years.

[Infliximab in ankylosing spondylitis associated with chronic hepatitis B infection. Role of Lamivudine therapy].

Anti-tumor necrosis factor-a (TNF) therapy has been associated with reactivation of hepatitis B virus infection. Case reports have suggested the concomitant need of lamivudine treatment in patients with HBV infection treated with antiTNFa agents. We describe a case of ankylosing spondylitis with positive HBV surface antigen (HBsAg) treated with infliximab and lamivudine.

[Use of glucocorticosteroids in rheumatoid arthritis. How and when should steroids be used in rheumatoid arthritis?].

Glucocorticoids (GC) are a mainstay of the therapy in rheumatoid arthritis (RA). Currently, and despite their extensive use, the discussion about the benefits and adverse effects of low dose GC in the management of RA persists. In recent years, a number of clinical trials have attempted to establish the benefits of long-term GC use as a disease-modifying antirheumatic drug in RA, and to define their side effects.

[Antimalarials: an update in rheumatic diseases].

Antimalarials are long-standing drugs that have been used since the nineteenth century for the treatment of skin rashes and lesions in lupus and rheumatoid arthritis. In recent decades, their use in these disorders has been consolidated, and new mechanisms of action have been incorporated, broadening the therapeutic perspectives of these drugs. Antimalarials are the treatment of choice in mild and moderate manifestations of systemic lupus erythematosus and are established as part of combined therapy in rheumatoid arthritis.

[Pulmonary atresia with intact ventricular septum associated with severe aortic stenosis].

Pulmonary atresia with intact ventricular septum is the complete obstruction of the right ventricular outflow tract due to pulmonary valve atresia in the absence of ventricular septal defect. Pulmonary flow is dependent on the ductus arteriosus. Other morphological anomalies are also present.

[Mediastinal dumbbell angiolipoma].

Dumbbell tumors are extremely rare. Most are neurogenic in origin. Growth is both inside and outside the spinal canal, and symptoms debut with medullary or spinal compression.
