Thymic stromal lymphopoietin (TSLP) is a pleiotropic cytokine that has emerged as a critical player in the development and progression of allergy and asthma. It is primarily produced by epithelial cells and functions as a potent immune system activator. TSLP acts through interaction with its receptor complex, composed of the TSLP receptor (TSLPR) and interleukin-7 receptor alpha chain (IL-7Rα), activating downstream complex signalling pathways.

Bradykinin Metabolism and Drug-Induced Angioedema.

Int J Mol Sci

July 2023

Department of Allergology "Nicolae Malaxa" Clinical Hospital, "Carol Davila" University of Medicine and Pharmacy, 022441 Bucharest, Romania.

Bradykinin (BK) metabolism and its receptors play a central role in drug-induced angioedema (AE) without urticaria through increased vascular permeability. Many cardiovascular and diabetic drugs may cause BK-mediated AE. Angiotensin-converting enzyme inhibitors (ACEIs) and neprilysin inhibitors impair BK catabolism.

Article Synopsis
  • The primary treatment for severe asthma exacerbations with respiratory failure includes both invasive and non-invasive ventilatory support alongside asthma medications, with mortality rates of 6.5% to 10.3% for those needing invasive ventilation.
  • Significant improvements in asthma management have led to a decrease in mortality rates, but in critical cases where conventional treatments fail, advanced options like ECMO and ECCO2R are necessary.
  • ECMO can reduce ventilator-associated lung injury and allow for important diagnostic procedures, while ECCO2R has gained popularity as an alternative treatment for both children and adults with severe respiratory failure due to asthma.

Objectives: Anxiety and depression are relevant comorbidities in asthma, but, in Portugal and Spain, data on this topic are scarce. We assessed, in patients with asthma, the frequency of anxiety and depression using the Hospital Anxiety and Depression Scale (HADS) and the European Quality of Life Five Dimension Questionnaire (EQ-5D); the level of agreement between these questionnaires, and the factors associated with these symptoms.

Methods: This is a secondary analysis of the INSPIRERS studies.

Newborn screening for cystic fibrosis (CF) can identify affected but asymptomatic infants. The selection of omic technique for gut microbiota study is crucial due to both the small amount of feces available and the low microorganism load. Our aims were to compare the agreement between 16S rRNA amplicon sequencing and metaproteomics by a robust statistical analysis, including both presence and abundance of taxa, to describe the sequential establishment of the gut microbiota during the first year of life in a small size sample (8 infants and 28 fecal samples).

Peptide microarrays are a powerful tool to identify linear epitopes of food allergens in a high-throughput manner. The main advantages of the microarray-based immunoassay are as follows: the possibility to assay thousands of targets simultaneously, the requirement of a low volume of serum, the more robust statistical analysis, and the possibility to test simultaneously several immunoglobulin subclasses. Among them, the last one has a special interest in the field of food allergy, because the development of tolerance to food allergens has been associated with a decrease in IgE and an increase in IgG4 levels against linear epitopes.

Drug hypersensitivity reactions (DHRs) to intravenous drugs can be severe and might leave patients and doctors in a difficult position where an essential treatment or intervention has to be suspended. Even if virtually any intravenous medication can potentially trigger a life-threatening DHR, chemotherapeutics, biologics, and antibiotics are amongst the intravenous drugs most frequently involved in these reactions. Admittedly, suspending such treatments may negatively impact the survival outcomes or the quality of life of affected patients.

Article Synopsis
  • Allergic asthma (AA) is a type of asthma that requires proof of IgE-sensitization to allergens and its role in causing symptoms; bronchial allergen challenge (BAC) may help identify AA cases among atopic asthmatics.
  • Current BAC methods are mainly used in research settings focused on mild asthma, which limits their application for moderate-to-severe cases in clinical practice.
  • The EAACI Task Force suggests a new BAC methodology aimed at improving patient identification for effective treatments like immunotherapy, ultimately enhancing asthma management in real-world scenarios.
