Background: Patients with human epidermal growth factor receptor 2 (HER2)-positive early breast cancer with residual invasive disease after neoadjuvant systemic therapy have a high risk of recurrence and death. The primary analysis of KATHERINE, a phase 3, open-label trial, showed that the risk of invasive breast cancer or death was 50% lower with adjuvant trastuzumab emtansine (T-DM1) than with trastuzumab alone.

Methods: We randomly assigned patients with HER2-positive early breast cancer with residual invasive disease in the breast or axilla after neoadjuvant systemic treatment with taxane-based chemotherapy and trastuzumab to receive T-DM1 or trastuzumab for 14 cycles.

This study addresses the gap in methodological guidelines for neuroergonomic attention assessment in safety-critical tasks, focusing on validating EEG indices, including the engagement index (EI) and beta/alpha ratio, alongside subjective ratings. A novel task-embedded reaction time paradigm was developed to evaluate the sensitivity of these metrics to dynamic attentional demands in a more naturalistic multitasking context. By manipulating attention levels through varying secondary tasks in the NASA MATB-II task while maintaining a consistent primary reaction-time task, this study successfully demonstrated the effectiveness of the paradigm.

Artificial communication with the brain through peripheral nerve stimulation shows promising results in individuals with sensorimotor deficits. However, these efforts lack an intuitive and natural sensory experience. In this study, we design and test a biomimetic neurostimulation framework inspired by nature, capable of "writing" physiologically plausible information back into the peripheral nervous system.

Atrial fibrillation and stroke prevention: 25 years of research at EP Europace journal.

Europace

August 2023

Cardiology Division, Department of Biomedical, Metabolic and Neural Sciences, University of Modena and Reggio Emilia, Policlinico di Modena, via del Pozzo 71, 41125 Modena, Italy.

Stroke prevention in patients with atrial fibrillation (AF) is one pillar of the management of this common arrhythmia. Substantial advances in the epidemiology and associated pathophysiology underlying AF-related stroke and thrombo-embolism are evident. Furthermore, the introduction of the non-vitamin K antagonist oral anticoagulants (also called direct oral anticoagulants) has clearly changed our approach to stroke prevention in AF, such that the default should be to offer oral anticoagulation for stroke prevention, unless the patient is at low risk.

Incidence of Lyme Borreliosis in Europe from National Surveillance Systems (2005-2020).

Vector Borne Zoonotic Dis

April 2023

Pfizer Vaccines Medical Development, Scientific and Clinical Affairs, Collegeville, Pennsylvania, USA.

Article Synopsis
  • Lyme borreliosis (LB) is the most prevalent tick-borne disease in Europe, with a systematic review showing significant variation in incidence reporting across 25 countries from 2005 to 2020.
  • Most countries (84%) employed passive surveillance methods, while only a few (Bulgaria, France, Poland, Romania) used standardized case definitions as recommended by public health authorities, which hampered cross-country comparisons.
  • The highest LB incidences were recorded in Estonia, Lithuania, Slovenia, and Switzerland (over 100 cases/100,000 PPY), with an estimated 128,888 cases reported annually and 24% of Europeans living in high-incidence areas.

Atrial fibrillation (AF) is the most prevalent arrhythmia worldwide. The presence of AF is associated with increased risk of systemic thromboembolism, but with the uptake of oral anticoagulant (OAC) and implementation of a holistic and integrated care management, this risk is substantially reduced. The diagnosis of AF requires a 30-s-long electrocardiographic (ECG) trace, irrespective of the presence of symptoms, which may represent the main indication for an ECG tracing.

Wastewater (WW) is considered a source of antibiotic-resistant bacteria with clinical relevance and may, thus, be important for their dissemination into the environment, especially in countries with poor WW treatment. To obtain an overview of the occurrence and characteristics of carbapenem-resistant Gram-negative bacteria (CR-GNB) in WW of Belgrade, we investigated samples from the four main sewer outlets prior to effluent into international rivers, the Sava and the Danube. Thirty-four CR-GNB isolates were selected for antimicrobial susceptibility testing (AST) and whole-genome sequencing (WGS).
