Medial Flap Turbinoplasty is Unlikely to Cause Empty Nose Syndrome.

Laryngoscope

December 2024

Department of Otolaryngology-Head and Neck Surgery, Henry Ford Health, Detroit, Michigan, U.S.A.

Background: Empty nose syndrome (ENS) is a poorly understood condition that affects a minority of patients who undergo inferior turbinate (IT) surgery. The Empty Nose Syndrome 6-item Questionnaire (ENS6Q) was validated to diagnose ENS following IT reduction, with an ENS6Q ≥ 11 being suggestive of ENS. Medial flap turbinoplasty (MFT) involves IT bone removal ± submucosal reduction (SMR) and is highly effective at surgically treating IT hypertrophy.

Deviated nasal septum (DNS) is a common condition affecting nasal breathing, which is generally treated using septoplasty. However, this invasive surgical method carries potential risks of post-surgical complications. Alternatively, electromechanical reshaping (EMR) is a novel method that has evolved as a non-thermal, minimally invasive option to reshape the cartilage using mechanical pressure and direct current (DC) without significant tissue damage.

Objective: Accurate management of acute nasoseptal fractures can mitigate cosmetic and functional complications. There is a lack of consensus on the ideal approach to manage acute nasoseptal fractures, which are often addressed with closed nasal and/or septal reduction (CN/SR) but sometimes addressed using an open approach with septoplasty. We aimed to systematically assess and compare outcomes of CN/SR versus open approaches to treat acute nasoseptal fractures.

Sci Rep

November 2024

Department of Otolaryngology, Head and Neck Surgery, The First Hospital of Jilin University, No. 1 Xinmin Street, Changchun, 130000, Jilin Province, China.

Article Synopsis
  • A study assessed the effectiveness of endoscopic septoplasty for patients with a deviated nasal septum and allergic rhinitis, using a meta-analysis of various databases up to 2024.
  • Results showed that patients who underwent the surgery experienced significant improvement in total nasal symptoms and specific allergic rhinitis symptoms compared to those treated with spray therapy alone.
  • The study concluded that while septoplasty provides substantial benefits, it should be conducted by skilled clinicians who can evaluate the individual anatomy of the patient's nasal septum.

Obstructive sleep apnoea syndrome (OSA) is a multi-factorial disorder, with quite complex endotypes, consisting of anatomical and non-anatomical pathophysiological factors. Continuous positive airway pressure (CPAP) is recognized as the first-line standard treatment for OSA, whereas upper airway (UA) surgery is often recommended for treating mild OSA patients who have refused or cannot tolerate CPAP, mild and primary snorers. The main results achievable by the surgery are UA expansion, and/or stabilization, and/or removal of the obstructive tissue to different UA levels.

Article Synopsis
  • Researchers introduce a new method for closing nasal septal perforations that involves prolonged isolation of the surgical site using silicone sheets.
  • Traditional techniques focus on direct mucosal closure with flaps or grafts, but the new approach uses a temporalis fascia graft with the septum protected for 6 weeks, allowing for natural healing.
  • In a study of 10 patients, there was a 90% success rate in closing defects with no serious complications, highlighting the method's simplicity and effectiveness.

Unlabelled: In otorhinolaryngology, the question of improving the tactics of early postoperative management in patients who have undergone surgery on the structures of the nasal cavity remains open. The timing of rehabilitation, the possible outcome of postoperative inflammation of the mucous membrane and the patient's ability to work depend on the choice of tactics. The nasal cavity differs from other anatomical areas in its rich blood supply, and therefore the problem of adequate hemostasis in the early stages after undergoing surgical treatment is still extremely relevant.

Objectives: Nasal septal deviation can cause nasal breathing issues, contribute to obstructive sleep apnea (OSA) development, and often hinders successful CPAP therapy. We hypothesized that although prevalent in the general population, nasal septal deviations differ structurally between OSA and non-OSA patients. This study evaluated nasal septal deviation morphology in OSA versus non-OSA patients using computed tomography (CT).

Background Eustachian tube dysfunction is characterized by insufficient dilation, leading to secondary pathologies in the middle ear. By comparing pre- and post-operative grades of Eustachian tube function and nasal resistance measurements, this study seeks to determine if septoplasty can improve Eustachian tube function in cases where nasal septal deviation is likely to cause mechanical dysfunction. We also aim to validate the Jain Bhalerao endoscopic classification of nasal septal deviation by assessing its utility in identifying septal deviations at a higher risk of causing Eustachian tube dysfunction.
