Tissue Eng Part B Rev

June 2020

Department of Oral and Maxillofacial Surgery, School of Dentistry and Institute of Oral Bioscience, Research Institute of Clinical Medicine of Chonbuk National University, Jeonju, Republic of Korea.

Article Synopsis
  • In 2007, rhBMP-2 was approved to augment bone tissue in dental procedures, but high doses and poor retention in carriers like collagen sponges can lead to side effects.
  • Demineralized dentin matrix (DDM), which has been used alongside rhBMP-2 since 1998, shows promise as a better carrier, offering sustained release and supporting bone growth over 30 days.
  • This review examines the histological results of using DDM with rhBMP-2, aiming to emphasize its effectiveness and biological benefits in dental implant applications.

Three-dimensional computer modeling of standard orbital mean shape in Asians.

J Plast Reconstr Aesthet Surg

March 2020

Department of Plastic and Reconstructive Surgery, University of Ulsan College of Medicine, Seoul Asan Medical Center, 88, Olympic-ro 43-gil, Songpa-gu, Seoul 05505, Republic of Korea. Electronic address:

Although some data for western norms in orbit shape were reported, the standard norms for Asian orbits were not established yet. The data would be very valuable for the various surgical procedures as well as the production of the appropriate instruments and implants. Therefore, we suggest a Korean orbit mean shape model based on the three-dimensional computer modeling, which includes the analysis of the various parameters with the calculated average value, thereby providing a standard mean shape orbital model that could be used for the Asian patients' orbital surgeries.

Demineralized dentin matrix (DDM) has been used as a recombinant human bone morphogenetic protein-2 (rhBMP-2) carrier in many clinical trials. To optimize the clinical safety and efficacy of rhBMP-2 with DDM, efforts have been made to improve the delivery of rhBMP-2 by 1) lowering the administered dose, 2) localizing the protein, and 3) prolonging its retention time at the action site as well as the bone forming capacity of the carrier itself. The release profile of rhBMP-2 that is associated with endogenous BMP in dentin has been postulated according to the type of incorporation, which is attributed to the loosened interfibrillar space and nanoporous dentinal tubule pores.

Several methods for the quantification of human anti-HBs, an antibody to hepatitis B surface antigen (HBsAg), have been developed based on enzyme reaction, chemiluminescence, fluorescence, and radioactivity for application to human serum or plasma. Commercial anti-HBs immunoassay kits use a sandwich method in which a bridge is formed by the anti-HBs between a HBsAg immobilized solid matrix and the labeled HBsAg. However, this direct sandwich enzyme-linked immunosorbent assay (ELISA) is insufficient to accurately evaluate the activity of the human monoclonal anti-HBs, GC1102.

The surgery-first approach (SFA) is a new paradigm in orthognathic surgery. In our experience over the last 10 years, SFA, particularly for the correction of the class III dentofacial deformity and facial asymmetry, has demonstrated high success rates without any major complications. However, many craniofacial surgeons remain concerned about the skeletal stability of SFA.

Background: The conventional approach during orthognathic surgery for cleft-related deformities has focused largely on restoration of maxilla retrusion, using the maxillary advancement as a standard treatment objective. However, the authors thought that maxillary vertical shortening and deficient incisor show could be additional key elements of cleft-related deformities. Although slight vertical lengthening can be obtained with only maxillary advancement, it would not be enough to obtain satisfactory aesthetic results in terms of the anterior facial height.

Surgery First Orthognathic Approach Without Presurgical Orthodontic Treatment: Questions and Answers.

J Craniofac Surg

July 2017

*Department of Plastic and Reconstructive Surgery, Ulsan University, College of Medicine, Seoul Asan Medical Center, Seoul, Korea †Temple University, School of Medicine, Philadelphia, PA.

"Surgery-First" for patients with malocclusion and skeletal disharmony is a new process of employing bony corrective surgery (first) without removal of dental compensations followed by orthodontic finishing (second). This process breaks with the time-tested principles of traditional orthognathic surgery. Not unexpectedly, there is slow adoption of this new process, particularly in the West.

Background: Although the perforator free flap is now a standard choice for head and neck reconstruction, problems such as microvascular complications, insufficient volume support for the defect, and fistula formation occur. We revisited a de-epithelialized concept for superficial circumflex iliac artery and anterolateral thigh perforator free flap to overcome these problems.

Methods: We applied the de-epithelized perforator free flaps in 35 cases among 761 microsurgical head and neck reconstructions and investigated flap characteristics (length gain of pedicle, flap size, and volumetric analysis) and outcomes (flap failure, partial flap necrosis, hematoma, infection, and fistula).

Although pre-surgical orthodontic treatment has been accepted as a necessary process for stable orthognathic correction in the traditional orthognathic approach, recent advances in the application of miniscrews and in the pre-surgical simulation of orthodontic management using dental models have shown that it is possible to perform a surgery-first orthognathic approach without pre-surgical orthodontic treatment. This prospective study investigated the surgical outcomes of patients with diagnosed skeletal class III dentofacial deformities who underwent orthognathic surgery between December 2007 and December 2014. Cephalometric landmark data for patients undergoing the surgery-first approach were analyzed in terms of postoperative changes in vertical and horizontal skeletal pattern, dental pattern, and soft tissue profile.
