Article Synopsis
  • Biliary drainage is a key treatment for patients with cholangiocarcinoma and aims to relieve cholestasis in both curative and palliative settings, but there are uncertainties about the best strategies and what counts as clinical success (CS).
  • A study analyzed data from patients with advanced hilar obstruction due to cholangiocarcinoma treated at a German center from 2010 to 2020, focusing on how often CS is achieved and complications in curative versus palliative cases.
  • Findings showed that curative patients had higher rates of CS and fewer complications compared to palliative patients, indicating that treatment intent affects outcomes significantly.

Background: Numerous studies have investigated the eyelid margin using confocal laser scanning microscopy (CLSM) and have presented morphological alterations of the examined structures, which were presumed to be Meibomian acini. However, recent data confirm that these structures are the cross-sections of dermal papillae of the dermoepidermal junction. This study aims to present the morphological appearance of Meibomian acini examined by confocal laser scanning microscopy in comparison to dermal papillae, and to reveal the corresponding patterns with specific histological sections.

Introduction: Surgery for perihilar cholangiocarcinoma (pCCA) is associated with high rates of postoperative morbidity and mortality. Extended liver resection (EXT) increases R0 resection rate and survival; however, patients with high perioperative risk are not suitable for extended resection. This study aimed to compare overall survival and surgical morbidity in patients with extended liver resection and parenchyma-preserving hepatectomy (PPH).

Article Synopsis
  • The study evaluates the effectiveness of defocus incorporated multiple segments (DIMS) spectacle lenses in reducing myopia progression in a Central European pediatric population aged 4-17 years, with myopia levels ranging from -0.88 to -8.25 D.
  • Over a 12-month period, results showed that while myopia progression was similar to previous studies, 50% of participants exhibited no progression, with factors like younger age, baseline astigmatism, and maternal myopia influencing treatment outcomes.
  • Participants reported high satisfaction with the DIMS treatment, experiencing few side effects that diminished over time, indicating the need for more research on the role of astigmatism in myopia management.

Background/aims: Hypoxemia is a common side effect of propofol sedation during endoscopy. Applying mild positive airway pressure (PAP) using a nasal mask may offer a simple way to reduce such events and optimize the conditions for diagnostic and therapeutic upper gastrointestinal endoscopies.

Methods: We compared overweight patients (body mass index >25 kg/m2) with a nasal PAP mask or standard nasal cannula undergoing upper gastrointestinal endoscopies by non-anesthesiologists who provided propofol sedation.

The crosstalk between NK cells and their surrounding environment is enabled through activating and inhibitory receptors, which tightly control NK cell activity. The co-inhibitory receptor TIGIT decreases NK cell cytotoxicity and is involved in NK cell exhaustion, but has also been associated with liver regeneration, highlighting that the contribution of human intrahepatic CD56 NK cells in regulating tissue homeostasis remains incompletely understood. A targeted single-cell mRNA analysis revealed distinct transcriptional differences between matched human peripheral blood and intrahepatic CD56 NK cells.
