Blood Loss in Women of Childbearing Potential Taking Oral Anticoagulants for Venous Thromboembolism (The BLEED Study).

Thromb Haemost

November 2024

Division of Cardiovascular Medicine and Thrombosis Research Group, Brigham and Women's Hospital, Harvard Medical School, Boston, Massachusetts, United States.

Background:  Oral anticoagulants (OAC) may exacerbate menstrual bleeding in women of childbearing age; however, the existing literature on this issue has several limitations.

Materials And Methods:  This study investigates abnormal uterine bleeding in women of childbearing age taking OAC-vitamin K antagonists or direct oral anticoagulants-for venous thromboembolism through a retrospective analysis of prospectively collected data. Uterine bleeding was assessed using the Pictorial Blood Assessment Chart (PBAC) and hemoglobin (Hb) values during anticoagulation compared with prior therapy.

Article Synopsis
  • * A study in Russia examined 574 hospitalized children under 18 with COVID-19 from January 2022 to March 2023, finding about one-third had coinfections, predominantly adenovirus and bacterial infections.
  • * Results indicated that while viral coinfections led to higher fevers and bronchitis, bacterial coinfections resulted in longer illnesses and increased pneumonia, suggesting that seasonal viruses may have been more harmful to children during the Omicron wave than SARS-CoV-2 alone.

Prothrombin complex concentrate for emergency surgery in patients on oral Xa-inhibitors.

J Thromb Haemost

October 2024

Department of Medicine and Thrombosis and Atherosclerosis Research Institute, McMaster University, Hamilton, Ontario, Canada.

Background: It is uncertain whether prothrombin complex concentrate (PCC) improves hemostasis in patients on treatment with oral factor Xa-inhibitors (XaI) who require emergency surgery.

Objectives: To evaluate whether, in patients with therapeutic levels of oral XaI, preoperative PCC prevents excessive bleeding during and after emergency surgery and is not associated with thrombotic complications.

Methods: We conducted a prospective cohort study wherein a fixed 2000 IU dose of 4-factor PCC was given to patients taking oral XaI with plasma XaI levels of at least 75 ng/mL before the emergency surgery with an expected blood loss of at least 50 mL.

Lung transplant is an effective method of treating patients with end-stage respiratory diseases, but problems such as the imbalance between the number of donor organs and the number ofrecipients needing organs still play a leading role. From a transplant point of view, a multiorgan donor is considered of greatest efficiency, so that all organs that can potentially be used should be transplanted. The combination of the vast geographical territory of Russia, the shortage of actual donors, and the relatively small number of transplant centers has led to the need to transport donor lungs by air over long distances.

Low absolute risk of thrombotic and cardiovascular events in outpatient pregnant women with COVID-19.

Thromb Res

May 2024

Cardiovascular Medicine Division, Brigham and Women's Hospital, Harvard Medical School, USA; Thrombosis Research Group, Brigham and Women's Hospital, Harvard Medical School, USA. Electronic address:

Article Synopsis
  • Pregnancy can increase the risk of thrombotic and cardiovascular events, but the study investigates this risk specifically for outpatient pregnant women with COVID-19.
  • In a sample of 6,585 outpatients, only 169 were pregnant, and by 90 days post-diagnosis, two women had venous thrombosis, resulting in a low thrombotic event rate of 1.20%.
  • The study concludes that while the risk of thrombotic and cardiovascular events exists, the absolute rates are low enough that widespread preventative measures may not be necessary for outpatient pregnant women with COVID-19.

Background:  With the widespread use of direct oral anticoagulants (DOACs), there is an urgent need for a rapid assay to exclude clinically relevant plasma levels. Accurate and rapid determination of DOAC levels would guide medical decision-making to (1) determine the potential contribution of the DOAC to spontaneous or trauma-induced hemorrhage; (2) identify appropriate candidates for reversal, or (3) optimize the timing of urgent surgery or intervention.

Methods And Results:  The DOAC Dipstick test uses a disposable strip to identify factor Xa- or thrombin inhibitors in a urine sample.

Article Synopsis
  • A study was conducted to analyze the type and duration of antithrombotic therapy in patients treated with catheter-based thrombolysis and venous stents for acute deep vein thrombosis (DVT) from 2005 to 2019.
  • The research included 173 patients, mainly young females with risk factors for thrombotic diseases, revealing that most received anticoagulant-only therapy post-stenting, while a smaller group received combined therapies.
  • The findings suggest that anticoagulant therapy without antiplatelet agents is the most common approach, indicating a need for further research on the impact of these treatments on patient outcomes.

Primary prevention of cancer-associated venous thrombosis: Rationale and challenges in clinical practice.

Curr Res Transl Med

July 2023

Service d'Oncologie médicale, APHP-SU, IUC, Hôpital La Pitié-Salpêtrière, Paris, France.

Cancer-associated venous thrombosis (CAT) is a common, multifactor event known to complicate the course of cancer and jeopardize a patient's prognosis. The current guidelines regarding the prevention of CAT are sometimes considered insufficiently precise about specific situations, or are poorly applied. The expected benefits of thromboprophylaxis are balanced by the risk of major bleeding induced by anticoagulation, which implies a need to accurately identify ambulatory patients at high risk of thrombosis or hemorrhage.

Spirocyclic compounds containing heterocyclic moieties represent promising 3D scaffolds for modern drug design. In the search for novel anti-flaviviral agents, we have obtained a series of 3-[-bis(sulfonyl)amino]isoxazolines containing spiro-annulated cyclooctane rings and assessed their antiviral activity against tick-borne encephalitis (TBEV), yellow fever (YFV), and West Nile (WNV) viruses. The structural analogs of spirocyclic compounds with a single sulfonyl group or 1,2-annulated cyclooctane ring were also investigated.

Contraceptives and Thrombosis: An Intertwined Revolutionary Road.

Semin Thromb Hemost

February 2024

Thrombosis and Haemostasis Unit, I.R.C.C.S. 'Casa Sollievo della Sofferenza', San Giovanni Rotondo, Foggia, Italy.

The development of oral contraceptives (OCs) began in 1921 and continued in the following years until the first regulatory approval from the Food and Drug Administration was granted in 1960. However, it took several years to realize that OCs presented an important but not frequent risk of venous thrombosis. Several reports ignored this dangerous effect and only in 1967 the Medical Research Council clearly stated this as an important risk.
