The intricate relationship between sports participation and cardiac arrhythmias is a key focus of cardiovascular research. Physical activity, integral to preventing atherosclerotic cardiovascular disease, induces structural, functional, and electrical changes in the heart, potentially triggering arrhythmias, particularly atrial fibrillation (AF). Despite the cardiovascular benefits, the optimal exercise amount remains unclear, revealing a J-shaped association between AF and exercise.

From the moment the SARS-CoV-2 virus was identified in December 2019, the COVID-19 disease spread around the world, causing an increase in hospitalisations and deaths. From the beginning of the pandemic, scientists tried to determine the major cause that led to patient deaths. In this paper, the background to creating a research model was diagnostic problems related to early assessment of the degree of damage to the lungs in patients with COVID-19.

Background: Evidence suggests that drug-coated balloons may benefit in-stent restenosis (ISR) treatment. However, the efficacy of new-generation sirolimus-coated balloon (SCB) compared with the latest generation drug-eluting stents (DESs) has not been studied in this setting.

Methods: All patients in the EASTBORNE (The All-Comers Sirolimus-Coated Balloon European Registry) and DEB-DRAGON (DEB vs Thin-DES in DES-ISR: Long Term Outcomes) registries undergoing percutaneous coronary intervention for DES-ISR were included in the study.

Article Synopsis
  • Left atrial appendage closure (LAAC) during heart surgery for atrial fibrillation (AF) patients may lower long-term stroke risk and mortality, but mixed evidence exists regarding its effectiveness in non-AF patients.
  • A review of 25 studies indicated that while LAAC reduced early stroke risk by 19% and late stroke risk by 13%, it showed no advantages for patients without AF.
  • Overall, LAAC appears beneficial in patients with preoperative AF, suggesting it could reduce long-term mortality and stroke rates, but there’s insufficient evidence to recommend it for those without AF.

Article Synopsis
  • The study investigates long-term outcomes of patients with in-stent restenosis (ISR) and chronic coronary syndrome (CCS) who underwent PCI using drug-eluting stents (DES) versus drug-coated balloons (DCB).
  • Patients treated with DES had significantly lower rates of target lesion revascularization (TLR), target vessel revascularization (TVR), and device-oriented composite events (DOCE) compared to those treated with DCB.
  • Overall, the findings suggest that using DES is more effective than DCB for this patient group in reducing revascularization needs over a long-term follow-up period.

Healthcare organizations worldwide face challenges in retaining their healthcare workforce, with individual and organizational factors influencing their intentions to leave. This study conducted eight online co-creation workshops and four Delphi sessions to gain qualitative and in-depth insights into job retention interventions, involving healthcare workers, hospital managers, and policymakers. A thematic analysis was conducted, resulting in multiple interventions that were clustered in four pre-defined themes: professional and personal support, education, financial incentives, and regulatory measures.

Metastatic brain cancer is a condition characterized by the migration of cancer cells to the brain from extracranial sites. Notably, metastatic brain tumors surpass primary brain tumors in prevalence by a significant factor, they exhibit an aggressive growth potential and have the capacity to spread across diverse cerebral locations simultaneously. Magnetic resonance imaging (MRI) scans of individuals afflicted with metastatic brain tumors unveil a wide spectrum of characteristics.

Metabolic dysfunction-associated steatotic liver disease: Navigating terminological evolution, diagnostic frontiers and therapeutic horizon-an editorial exploration.

World J Gastroenterol

May 2024

Department of Internal Medicine and Clinical Pharmacology, School of Medicine in Katowice, Medical University of Silesia, Katowice 40-752, Poland.

Metabolic dysfunction-associated steatotic liver disease (MASLD), once known as non-alcoholic fatty liver disease (NAFLD), represents a spectrum of liver disorders characterized by lipid accumulation within hepatocytes. The redefinition of NAFLD in 2023 marked a significant reposition in terminology, emphasizing a broader understanding of liver steatosis and its associated risks. MASLD is now recognized as a major risk factor for liver cirrhosis, hepatocellular carcinoma, and systemic complications such as cardiovascular diseases or systemic inflammation.

Background: Multisystem inflammatory syndrome in children (MIS-C) is a rare, severe complication of coronavirus disease 2019, commonly involving the gastrointestinal tract. Some children with MIS-C undergo appendectomy before the final diagnosis. There are several hypotheses explaining the pathomechanism of MIS-C, including the central role of the viral antigen persistence in the gut, associated with lymphocyte exhaustion.

Article Synopsis
  • The study investigates elderly patients (over 80 years old) in Polish ICUs, aiming to understand their clinical features and outcomes.
  • Researchers collected data from 371 patients, comparing frail (CFS > 4) to non-frail patients and assessing mortality rates.
  • Findings indicate higher ICU and 30-day mortality in frail patients, with clinical frailty scale (CFS) and SOFA score being significant predictors of ICU mortality.

Comparative Analysis of Long-Term Outcomes in Valve-Sparing Aortic Root Reimplantation: Full Sternotomy versus Mini-Sternotomy Approach.

J Clin Med

May 2024

Department of Cardiac Surgery and Transplantology, National Medical Institute of the Ministry of Interior and Administration, Centre of Postgraduate Medical Education, 02-507 Warsaw, Poland.

Aortic valve-sparing aortic root replacement (VSARR) David procedure has not been routinely performed via minimally invasive access due to its complexity. We compared our results for mini-VSARR to sternotomy-VSARR from another excellence center. Eighty-four patients, 62 in the sternotomy-VSARR group and 22 in the mini-VSARR group, were included.

Despite evidence suggesting the benefit of prophylactic regional antibiotic delivery (RAD) to sternal edges during cardiac surgery, it is seldom performed in clinical practice. The value of topical vancomycin and gentamicin for sternal wound infections (SWI) prophylaxis was further questioned by recent studies including randomized controlled trials (RCTs). The aim of this systematic review and meta-analysis was to comprehensively assess the safety and effectiveness of RAD to reduce the risk of SWI.

Article Synopsis
  • Treatment-free remission (TFR) is achievable in about 50% of CML patients on tyrosine kinase inhibitors, but the mechanisms for maintaining TFR are not well understood.
  • This study analyzed immune markers in 63 CML patients who had stopped imatinib, finding that an increase in CD8PD-1 cells correlates with a higher chance of losing TFR.
  • Results indicate that the level of CD8PD-1 cells could help predict early molecular recurrence, particularly highlighting differences between patients with different e13a2 and e14a2 transcripts.
