Single Subcutaneous Ketamine Dose Followed by Oral Ketamine for Depression Symptoms in Hospice Patients: A Case Series.

J Pain Palliat Care Pharmacother

June 2021

Natalie M. Latuga, PharmD, BCPS, Debra L. Luczkiewicz, MD, Christopher W. Kerr, MD, PhD, are with Hospice & Palliative Care Buffalo, Cheektowaga, NY, USA.

Management of depression symptoms in hospice patients is complicated by the fact that an appropriate trial of antidepressant therapy requires 4-6 weeks and most hospice patients receive hospice services for less than 8 weeks. Intravenously administered ketamine has been shown to produce rapid improvement in depression symptoms but is not an ideal route for hospice patients and oral ketamine appears to have a slower onset of antidepressant activity. We present a case series that illustrates the use of a single subcutaneous dose of ketamine (0.

Surface hydrophobics mediate functional dimerization of CYP121A1 of Mycobacterium tuberculosis.

Sci Rep

January 2021

Department of Biochemistry, Jacobs School of Medicine and Biomedical Science, University at Buffalo, Buffalo, NY, 14203, USA.

Tuberculosis is caused by the pathogenic bacterium Mycobacterium tuberculosis (Mtb) and remains the leading cause of death by infection world-wide. The Mtb genome encodes a disproportionate number of twenty cytochrome P450 enzymes, of which the essential enzyme cytochrome P450 121A1 (CYP121A1) remains a target of drug design efforts. CYP121A1 mediates a phenol coupling reaction of the tyrosine dipeptide cyclo-L-Tyr-L-Tyr (cYY).

To date, there is little consensus on the role of pelvic imaging in assessing local disease extent during initial staging in patients with endometrial carcinoma, with practices differing widely across centers. However, when pretreatment assessment of local tumor extent is indicated, MRI is the preferred imaging modality. Preoperative imaging of endometrial carcinoma can define the extent of disease and indicate the need for subspecialist referral in the presence of deep myometrial invasion, cervical extension, or suspected lymphadenopathy.

Nursing Home-Associated Pneumonia, Part II: Etiology and Treatment.

J Am Med Dir Assoc

March 2020

Professor Emeritus, Department of Medicine, Division of Infectious Diseases, Jacobs School of Medicine and Biomedical Science, State University of New York at Buffalo, Buffalo, NY. Electronic address:

This is the second of 2 parts of a narrative review of nursing home-associated pneumonia (NHAP) that deals with etiology and treatment in the nursing home. In the 1980s and 1990s, the etiology of NHAP was considered to be similar to community-acquired pneumonia (CAP). This belief was reflected in CAP guidelines until 2005 when the designation healthcare-associated pneumonia or HCAP was introduced and nursing home residents were included in the HCAP category.

What is the Role of Nursing Homes in the Surviving Sepsis Campaign?

J Am Med Dir Assoc

January 2020

Department of Medicine, Division of Infectious Diseases, Jacobs School of Medicine and Biomedical Science, State University of New York at Buffalo, Buffalo, NY. Electronic address:

Recently, there have been several publications advocating for an expansive role for nursing homes (NHs) in the Surviving Sepsis Campaign (SSC). The rationale for this effort is the problem of high rates of 30-day readmissions from NHs and a disproportionate percentage of residents with a diagnosis of sepsis in emergency departments. This article provides a brief history of the SSC and the evolution of the definition of sepsis and of the timing of interventions that make up a "sepsis bundle.

Specificity of the Redox Complex between Cytochrome P450 24A1 and Adrenodoxin Relies on Carbon-25 Hydroxylation of Vitamin-D Substrate.

Drug Metab Dispos

September 2019

Department of Biochemistry, Jacobs School of Medicine and Biomedical Science, University at Buffalo, Buffalo New York

Metabolic deactivation of 1,25(OH)D3 is initiated by modification of the vitamin-D side chain, as carried out by the mitochondrial cytochrome P450 24A1 (CYP24A1). In addition to its role in vitamin-D metabolism, CYP24A1 is involved in catabolism of vitamin-D analogs, thereby reducing their efficacy. CYP24A1 function relies on electron transfer from the soluble ferredoxin protein adrenodoxin (Adx).

The lateral fluid percussion injury (FPI) model is well established and has been used to study TBI and post-traumatic epilepsy (PTE). However, considerable variability has been reported for the specific parameters used in different studies that have employed this model, making it difficult to harmonize and interpret the results between laboratories. For example, variability has been reported regarding the size and location of the craniectomy, how the Luer lock hub is placed relative to the craniectomy, the atmospheric pressure applied to the dura and the duration of the pressure pulse.
