Objectives: We aim to evaluate the risk of recurrence after neoadjuvant chemotherapy followed by radical cystectomy, particularly in ypT2 disease in patients with urothelial carcinoma, because it is not clear if all eligible patients with high-risk muscle-invasive urothelial carcinoma should be treated with adjuvant nivolumab.

Materials And Methods: We analysed the radiological and clinicopathological features, including cT and ypT stages, of 197 patients who had undergone two to four cycles of cisplatin-based neoadjuvant chemotherapy and radical cystectomy without adjuvant chemotherapy. We stratified the risk of postoperative recurrence by these factors.

Article Synopsis
  • - The study aimed to develop a systematic method for diagnosing atrioventricular nodal reentrant tachycardia (AVNRT) that has a concealed nodoventricular pathway (cNVP).
  • - Researchers analyzed 13 cases of AVNRT involving cNVP, focusing on how the pathway connected to either the slow or fast pathways, and also looked at cases of orthodromic reciprocating tachycardia related to cNVP.
  • - The diagnostic process involves three key steps: identifying accessory pathways via His-refractory PVCs, excluding orthodromic reciprocating tachycardia, and confirming the AVN as the cNVP insertion site, with specific criteria for tachy

Background: The effects of the patient's disease awareness on the management of postablation of atrial fibrillation (AF) are unknown.

Methods: One hundred thirty-three AF patients undergoing an initial ablation were given a disease awareness questionnaire with a score of 16 points (8 points about AF in general and 8 points about oral anticoagulants) for the Jessa Atrial Fibrillation Knowledge Questionnaire (JAKQ) before and 1-year-after ablation. We divided them into the poor disease awareness group and good disease awareness group according to the median value (75%) of the total JAKQ score about AF in general, and compared the baseline patient characteristics and the 1-year changes in the JAKQ score, medication adherence, blood pressure, laboratory data, echocardiographic parameters, and AF/atrial tachycardia (AT) recurrence rate between the two groups.

Article Synopsis
  • - The study examines how the combined score of C-reactive protein and albumin ratio (CAR) and modified albumin-bilirubin (mALBI) affects prognosis in patients with hepatocellular carcinoma (HCC) after liver surgery.
  • - Conducted on 214 patients who had liver resection for HCC between 2008 and 2018, the research categorizes patients based on their CAR and mALBI scores to assess their disease-free and overall survival rates.
  • - Results show that a higher CAR-mALBI score correlates with lower survival rates, highlighting its potential as a significant prognostic tool for evaluating both the liver's function and the patient's inflammatory state in HCC management.

Patients with a long history of ulcerative colitis (UC) are at risk of developing a significant complication known as UC-associated neoplasia (UCAN). To reduce the risk of UCAN and the associated mortality, the current guidelines recommend initiating surveillance colonoscopy 8-10 years after confirmation of UC diagnosis. In recent years, advancements in endoscopic diagnostic technologies, including magnifying and image-enhancing techniques, have allowed for the production of high-contrast images that emphasize mucosal structures, vascular patterns, and color tones.

Extrapleural air is a rare condition that may concurrently develop with pneumomediastinum and pneumothorax, especially in older patients with fragile connective tissues. Physicians should consider extrapleural air to prevent inadvertent harm. Coronal reconstruction computed tomography images help appreciate extrapleural air and recognize the track of extrapulmonary air.

Background: Risk stratification is important in patients with post-cardiac arrest syndrome. The Post-Cardiac Arrest Syndrome for Therapeutic Hypothermia (CAST) and revised CAST (rCAST) scores have been well validated for predicting neurological outcomes, particularly for conventionally resuscitated patients with post-cardiac arrest syndrome. However, no studies have evaluated patients undergoing extracorporeal cardiopulmonary resuscitation.

Adolescent idiopathic scoliosis (AIS) is the most common form of pediatric musculoskeletal disorder. Observational studies have pointed to several risk factors for AIS, but almost no evidence exists to support their causal association with AIS. Here, we applied Mendelian randomization (MR), known to limit bias from confounding and reverse causation, to investigate causal associations between body composition and puberty-related exposures and AIS risk in Europeans and Asians.

Article Synopsis
  • Sterile inflammation from metabolic disorders affects blood vessel function, and this study investigates how hyperglycemia-linked inflammation, particularly through the STING pathway, contributes to endothelial dysfunction in diabetic mice.
  • Researchers found that high blood sugar levels increased STING and DNA damage markers in the aorta, and deleting STING improved blood vessel response to certain stimuli in diabetic conditions.
  • The study concluded that the STING pathway exacerbates endothelial dysfunction and suggests that targeting STING could be a potential treatment for these issues related to high blood sugar levels.
