Background: We investigated the role of A2B-adenosine receptor in regulating immunosuppressive metabolic stress in the tumor microenvironment. Novel A2B-adenosine receptor antagonist PBF-1129 was tested for antitumor activity in mice and evaluated for safety and immunologic efficacy in a phase I clinical trial of patients with non-small cell lung cancer.

Methods: The antitumor efficacy of A2B-adenosine receptor antagonists and their impact on the metabolic and immune tumor microenvironment were evaluated in lung, melanoma, colon, breast, and epidermal growth factor receptor-inducible transgenic cancer models.

Article Synopsis
  • - The text discusses a rare case of phlegmonous gastritis found during an autopsy of a patient who had cirrhosis related to nonalcoholic steatohepatitis.
  • - The patient experienced severe health decline, including anorexia and liver dysfunction, ultimately leading to death due to hepatic failure within two weeks.
  • - The report emphasizes the need for awareness that phlegmonous gastritis can occur as a complication in patients with advanced liver cirrhosis, despite its rare occurrence.

Article Synopsis
  • * She was admitted to the hospital due to jaundice and liver dysfunction, leading to diagnosis through various tests and liver histology.
  • * This case is notable as it appears to be the first documented instance of autoimmune hepatitis occurring post-COVID-19 vaccination in a patient with a background of Sjögren's syndrome.

Objective: This study aimed to develop a simple and reliable technique to assess excitation-contraction (E-C) coupling for early diagnosis of critical illness myopathy (CIM).

Methods: We prospectively performed clinical and electrophysiological examinations on patients admitted to intensive care unit (ICU). In addition to full neurological examinations and routine nerve conduction study, motor related potential (MRP) was recorded using an accelerometer attached to the base of hallux after tibial nerve stimulation, and E-C coupling time (ECCT) was measured from the latency difference between soleus compound muscle action potential (CMAP) and MRP.

Objective The change in serum lipid levels by direct-acting antiviral (DAA) treatment for chronic hepatitis C varies depending on the type of DAA. How the lipid level changes induced by glecaprevir-pibrentasvir (G/P) treatment contribute to the clinical outcome remains unclear. We conducted a prospective observational study to evaluate the effectiveness of G/P treatment and the lipid level changes.

Article Synopsis
  • * He received hepatic arterial infusion chemotherapy (HAIC) and then a combination of sorafenib and HAIC after the tumors progressed.
  • * Although the sequential therapy was effective in shrinking the liver tumors, the patient ultimately died of respiratory failure attributed to primary lung cancer, with autopsy findings indicating necrotic tissue and few viable HCC cells left.

Article Synopsis
  • A unique case report describes an elderly Japanese man with intraductal papillary mucinous neoplasm complicated by a pancreatogastric fistula, a rare occurrence.
  • The fistula was confirmed through endoscopic retrograde pancreatography, which involved inserting a cannulated guidewire into the stomach.
  • After six months, the patient was diagnosed with intraductal papillary mucinous carcinoma, and notable mucosal changes were observed both endoscopically and histochemically in the fistula.

A rare case of Helicobacter pylori-uninfected intramucosal poorly differentiated adenocarcinoma that occurred in the gastric fornix.

Clin J Gastroenterol

February 2021

Department of Internal Medicine and Gastroenterology, Sapporo Shirakabadai Hospital, 2-18 7-26, Tsukisamu Higashi, Toyohira-ku, Sapporo, Hokkaido, 062-0052, Japan.

We report a rare case of undifferentiated-type intramucosal gastric cancer that occurred in the fornix of the stomach without Helicobacter pylori infection, which consisted mainly of poorly differentiated adenocarcinoma. A 49-year-old man visited our hospital for a follow-up endoscopic examination of a small depressed lesion of the gastric fornix detected by surveillance esophagogastroduodenoscopy. On magnifying endoscopy with blue laser imaging, the depressed lesion (approximately 10 mm in diameter) was regarded as undifferentiated-type early gastric cancer that proved to be a poorly differentiated adenocarcinoma by histological examination of biopsied specimens.

Pancreatic cancer is the fourth leading cause of cancer-related deaths in Japan. To identify risk loci, we perform a meta-analysis of three genome-wide association studies comprising 2,039 pancreatic cancer patients and 32,592 controls in the Japanese population. Here, we identify 3 (13q12.

Background/aims: A multifunctional snare SOUTEN has a sharp tip at the top of the snare loop that enables incision of the mucosa, dissection of the submucosal layer, and snaring of lesion. This study assessed the efficacy and safety of complete endoscopic resection of colorectal neoplasia using SOUTEN.

Methods: We analyzed the rates of gross en bloc resection and complete resections of 108 consecutive tumors from 69 patients resected by precutting endoscopic mucosal resection (precutting), hybrid endoscopic submucosal dissection (hybrid), or conventional endoscopic submucosal dissection (conventional) using SOUTEN.

Article Synopsis
  • - A 79-year-old man was admitted to the hospital due to severe weight loss and signs of inflammation, diagnosed with metastatic lung cancer.
  • - Lab tests revealed high levels of granulocyte colony-stimulating factor (G-CSF) and interleukin-6 (IL-6) in his system, indicating a unique case of lung cancer.
  • - An autopsy confirmed that the lung tumor produced both cytokines and exhibited characteristics of large-cell carcinoma with sarcomatoid features, likely due to changes induced by IL-6 that promoted metastasis.

Objective: The ice-pack test is a convenient diagnostic testing procedure for myasthenia gravis (MG). We investigated the underlying mechanism of the ice-pack test performed on bilateral masseters.

Methods: We performed trigeminal repetitive nerve stimulation (RNS), excitation-contraction (E-C) coupling assessment (Imai's method) and bite force measurement before and after cooling of the masseters in MG patients and normal controls.

We herein report the case of a 78-year-old woman with an intraductal tumor with scant mucin production in a moderately dilated main pancreatic duct that resembled an intraductal tubulopapillary neoplasm (ITPN) on imaging. An endoscopic transpapillary forceps biopsy enabled an accurate preoperative diagnosis of the tumor as an oncocytic type intraductal papillary mucinous neoplasm (IPMN) of the pancreas microscopically showing papillary growth consisting of oncocytic cells with a typical mucin expression profile, although with few intraepithelial lumina containing mucin. This is the first case of an oncocytic type IPMN mimicking an ITPN that was able to be diagnosed preoperatively.

Objective: The aim of this study was to evaluate post-tetanic potentiation of muscle twitch in myasthenia gravis (MG).

Methods: Post-tetanic potentiation was evaluated by recording the compound muscle action potential (CMAP) of abductor pollicis brevis and movement-related potential (MRP) of the thumb using an accelerometer after tetanic stimulation of the median nerve at the wrist. After baseline recording, tetanic stimulation was delivered to the median nerve at a frequency of 10 Hz for 10s.

Background: Interstitial cells of Cajal (ICCs) are detected as a pacemaker of gastrointestinal movement and express c-kit and CD34. Recently, ICCs have implicated pathogenesis in several human diseases presenting gastrointestinal motor dysfunction. This study was performed to clarify the role of ICCs in idiopathic sigmoid megacolon using histological and immunohistochemical examinations.
