Intestinal-type intraductal papillary mucinous neoplasm (IPMN) of the pancreas is clinicopathologically distinctive. Our research aimed to elucidate the molecular mechanism of the development and progression of the intestinal-type IPMN. In 60 intestinal-type IPMN specimens, histological transitions from gastric-type epithelia to intestinal-type epithelia were observed in 48 cases (80%).

Introduction: Polymyxin B hemoperfusion (PMX-HP) is an adjuvant therapy for sepsis or septic shock that removes circulating endotoxin. However, PMX-HP has seldom achieved expectations in randomized trials targeting nonspecific overall sepsis patients. If used in an optimal population, PMX-HP may be beneficial.

Article Synopsis
  • - Patients with diabetes mellitus (DM) experienced a longer delay in receiving antibiotics for severe sepsis, with a median time of 103 minutes compared to 86 minutes for those without DM, showing a significant difference (P = .05).
  • - The study conducted in 53 ICUs in Japan analyzed 619 patients admitted from January 2016 to March 2017 and found that although DM was linked to delayed treatment, there were no significant differences in mortality rates between patients with (23.9%) and without DM (21.6%).
  • - Despite DM being associated with a delay in antibiotic administration, it did not affect overall in-hospital mortality, suggesting that while the timing of care might differ, patient

Intraductal papillary neoplasm of the bile duct (IPNB) is a grossly visible papillary biliary neoplasm with morphological variations and occasional invasion. Recently a new classification of IPNB into type 1 and type 2 was proposed in which the type 1 IPNBs consist of fine papillary neoplastic glands and the type 2 IPNBs consist of complex branching glands, seldom with foci of solid-tubular components. However, clinicopathological and molecular characteristics of these types of IPNBs are yet to be identified.

Article Synopsis
  • The study analyzed severe sepsis patients in 59 ICUs in Japan to compare those with and without bacteremia.
  • Of the 1,184 patients, 636 (54.5%) had bacteremia, which was associated with higher rates of septic shock and worse sepsis severity scores.
  • However, there was no significant difference in in-hospital mortality between those with bacteremia and those without.

Chemotherapy often induces oral ulcerative mucositis (OUM) in patients with cancer, characterized by severe painful inflammation. Mouth-washing with the Japanese herbal medicine hangeshashinto (HST) ameliorates chemotherapy-induced OUM in patients with colorectal cancer. Previously, we demonstrated that HST decreased interleukin 1β-induced prostaglandin E2 production in human oral keratinocytes (HOKs) and OUM-induced mechanical or spontaneous pain in rats.

Article Synopsis
  • The study investigates the relationship between predisposition conditions and risk modifiers in patients at risk for acute respiratory distress syndrome (ARDS) due to non-pulmonary sepsis, as previous data on this is limited.
  • A secondary analysis of the FORECAST cohort study included 594 patients, finding that 14.3% developed ARDS, with a notable association between septic shock and ARDS occurrence.
  • Key independent risk modifiers for ARDS identified in the study include the source of ICU admission, current smoking status, and infection site, among others, highlighted through multivariate logistic regression analysis.

We previously reported the association of the estimated glomerular filtration rate (eGFR) calculated from the serum creatinine level (S-Cr) measured using the Jaffe method with the GFR (eGFR) estimated from the serum cystatin C level (CysC). However, few studies have compared the eGFR using the enzymatic method with the eGFR. It is unclear whether there are differences in the results of renal function assessment.

Needle tract seeding during endoscopic ultrasound fine-needle biopsy (EUS-FNB) remains a concern. We investigated whether such seeding occurred in a patient with pancreatic ductal adenocarcinoma (PDA).  Surgically resected and EUS-FNB-derived specimens were genotyped to determine if a gastric wall tumor that emerged 3 years after curative resection of an early-stage PDA was clonally related to the original tumor.

Article Synopsis
  • - The study investigates the importance of timely antibiotic administration in patients with severe sepsis and septic shock in Japan, highlighting that sepsis care differs from other emergency conditions and is hard to implement effectively.
  • - Conducting a prospective observational analysis of 1184 adult patients in 59 ICUs, researchers assessed in-hospital mortality based on the timing of antibiotic delivery, comparing different time intervals from recognition of sepsis.
  • - The findings reveal that while 30.5% received antibiotics within 1 hour and 73.9% within 3 hours, longer delays correlated with higher mortality rates, emphasizing the critical role of timely treatment in improving outcomes for sepsis patients.

Metachronous development of intraductal papillary mucinous neoplasms in the remnant pancreas following resection is a significant clinical burden. Our aim was to characterize the clinicopathological and molecular features of the patients with metachronous tumor development to identify predictive factors and the possible route(s) of dissemination. Seventy-four patients who underwent resection of intraductal papillary mucinous neoplasms with no invasive compartment or associated carcinoma were retrospectively analyzed.

Background:  Anticoagulant therapy has seldom been achieved in randomized trials targeting nonspecific overall sepsis patients. Although the key components to identify the appropriate target in sepsis may be disseminated intravascular coagulation (DIC) and high disease severity, the interaction and relation of these two components for the effectiveness of therapy remain unknown.

Objective:  This article identifies the optimal target of anticoagulant therapy in sepsis.

Dropped head syndrome (DHS) has been rarely observed in amyotrophic lateral sclerosis (ALS), and the neuropathological findings of this condition have almost never been described. The identification of transactivation response DNA-binding protein 43 kDa (TDP-43), which binds to RNA/DNA has provided a new method for studying ALS and frontotemporal lobar degeneration (FTLD). Post-mortem examination of an adult sudden death case of a 71-year-old patient who complained of DHS exhibited severe loss of anterior motor neurons in the cervical cord (C4-6).

Patients with diabetes mellitus are prone to increased adverse outcomes after percutaneous coronary intervention, even with contemporary drug-eluting stents. Randomized controlled trials have demonstrated comparable clinical outcomes between an ultrathin bioresorbable-polymer sirolimus-eluting stent (BP-SES) and a thin-strut durable-polymer everolimus-eluting stent (DP-EES) that has specific labeling for patients with diabetes. We aimed to evaluate the safety and efficacy of the BP-SES in patients with diabetes mellitus.

Introduction: Sepsis leads to coagulopathy by the activation of inflammatory mediators and vascular endothelial cell injury. A number of biomarkers are used to evaluate coagulopathy on sepsis. Fibrinogen and antithrombin activity have been reported as biomarkers of coagulopathy; however, the utility of these two markers has not been well established.
