Herein we describe a case of Reiter's syndrome coexisting with HIV infection in a female parenteral drug addict who was also predisposed to multiple genitourinary involvement. In our view. the manifestation of additional autoimmune states could herald the worsening of the prognosis of HIV infection and the nephrological and urological course of these patients.

A case of mosaic Turner's syndrome with a 45,X/46,XX/47,XXX karyotype, who was also a fragile X obligate carrier as the mother of an affected boy, was identified by molecular diagnosis. Complete haplotyping and direct DNA analysis showed that the X chromosome in all metaphases was the normal X. At the age of 57, she is mentally normal.

Review of several facts of epidemiological nature, from a study on bladder urothelial carcinoma cases detected over the period 1983-1990 in our centre, a reference hospital serving the Bilbao area. Certain data recently published by the Euskadi Cancer Register allowed us to establish some considerations related to the presence of this entity in our setting. Its incidence nears the highest rates in industrial countries.

We report a case of advanced ureteropelvic junction obstruction associated with contralateral renal carcinoma in a 39-year-old male patient who consulted for frequent episodes of renal colic and gross asymptomatic hematuria. The results of the renal function tests and the radiological and isotope work up indicated a two-stage surgical procedure of pyeloplasty and contralateral partial nephrectomy. At 6 years follow up the patient is tumor free and renal function is preserved.

The stromal component of the urethral caruncle (UC) comprises a mixture of inflammatory, vascular, and fibrotic phenomena. Although mastocytes are cell elements known to appear in relation with chronic inflammation, there is no data in the literature explaining their significance in UCs or their ultrastructure relation with fibroblasts. We postulate that this special form of intercellular attachment could play a role in this condition's pathogenesis.

We describe 2 cases of cavitary lung metastasis from bladder cancer and retrospectively analyze the frequency of such an event in our environment. Although rare, this finding is not exceptional and should be taken into account when clinicians and oncologists face the differential diagnosis of pulmonary cavitation. Moreover, a cavitary pulmonary nodule may also be the presenting sign of bladder cancer.

A case of recurrent emphysematous cystitis in a 44-year-old male diabetic is described. Treatment of the first episode had been effective and demonstrable radiologically. A recurrent episode of cystitis disclosed a neurogenic bladder, which had not been detected previously.

Presentation of one case of bacterial acute focal nephritis (acute lobe nephrosis) in a parenterally drug-abuser, HIV-infected male. From the clinical point of view, the picture was interpreted as acute hepatitis, and the lesion was detected by ultrasound techniques. A right side location, relative lack of urological symptoms and coexistence of a chronic liver disease contributed to the diagnostic error.

Presentation of a new case of synchronic concurrence of renal adenocarcinoma and papillar carcinoma of renal pelvis on the same kidney. The origin of this double malignant renal tumour remains unknown, although several hypothesis have been raised. We believe that coexistence of both tumours is more than casual and we would underscore the morphogenetic interpretation of such coincidence.

The clinicopathologic features of a hibernoma occurring in a 26 year-old female are reported. The tumor was located in the interscapular region and eventually became painful. Surgical excision was performed and to date, no recurrences have been detected.

In the context of a misleading diagnosis of paraurethral abscess, we report a case of unrecognized metastatic squamous cell carcinoma of the male urethra that mimicked a picture of paravertebral abscess with accompanying massive caval vein thrombosis. Fine needle aspiration cytology of both paravertebral abscess and urethral nodule allowed the diagnosis. This report calls the attention of clinicians to think of a malignant process in case of a paravertebral abscess.

We report on a patient with von Hippel-Lindau disease with special reference to the urological features. Several members of the patient's family had also presented this hereditary disease. Careful follow-up of these patients permits early diagnosis of some of the characteristic tumors.
