SADI-S, state of the art. Indications and results in 2024: a systematic review of literature.

Updates Surg

December 2024

U.O.C. Chirurgia Endocrina e Metabolica, Fondazione Policlinico Universitario Agostino Gemelli IRCCS, Università Cattolica del Sacro Cuore, Rome, Italy.

Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy (SADI-S) was proposed as a simplification of the biliopancreatic diversion (BPD) procedure with duodenal switch (DS) in order to reduce operative time and postoperative complications rate, however, keeping effectiveness in weight loss and in comorbidities' resolution. We performed a systematic review of the literature with the aim of summarizing the current evidence of SADI-S clinical outcomes in order to assess its effectiveness and safety, and a total of 17 studies were included. Short- and mid-term follow-up results were reported, with a mean TWL loss ≥ 25% at 12 months and > 44% after 24 months, comparable to BPD-DS, also in reoperative surgery.

Article Synopsis
  • Trans-foraminal endoscopic discectomy (TELD) is a minimally invasive surgical option for lumbar disc herniation (LDH) that typically results in less postoperative low back pain and complications compared to traditional open surgery.
  • A study of 86 patients who underwent TELD found that those with preoperative disc height loss or "microinstability" had a higher chance of experiencing persistent back pain and less improvement in their condition after surgery.
  • The findings suggest that severe disc height loss may indicate a greater risk for poor recovery outcomes, emphasizing the importance of preoperative assessments in predicting surgery success.

The aim of this work was to validate the measurements of three physiological parameters, namely, body temperature, heart rate, and peripheral oxygen saturation, captured with an out-of-the-lab device using measurements taken with clinically proven devices. The out-of-the-lab specialized device was integrated into a customized mHealth application, e-CoVig, developed within the AIM Health project. To perform the analysis, single consecutive measurements of the three vital parameters obtained with e-CoVig and with the standard devices from patients in an intensive care unit were collected, preprocessed, and then analyzed through classical agreement analysis, where we used Lin's concordance coefficient to assess the agreement correlation and Bland-Altman plots with exact confidence intervals for the limits of agreement to analyze the paired data readings.

Dropped Head Syndrome: The Importance of Neurophysiology in Distinguishing Myasthenia Gravis from Parkinson's Disease.

Biomedicines

August 2024

Gardner Family Center for Parkinson's Disease and Movement Disorders, Department of Neurology, University of Cincinnati, Cincinnati, OH 45219, USA.

Article Synopsis
  • Dropped head syndrome (DHS) involves severe neck flexion due to neck muscle tone imbalance, often linked to neuromuscular conditions like myasthenia gravis (MG), while Parkinson's disease (PD) may present with a similar issue called antecollis without muscle weakness.
  • A case study of a 71-year-old man showed symptoms of neck flexion and Parkinsonism, including rigidity and reduced arm swing, but also revealed significant neck muscle weakness and confirmed MG with specific tests.
  • Successful treatment involved medications for both MG (pyridostigmine and plasma exchange) and PD (carbidopa/levodopa), emphasizing the importance of detailed clinical evaluations to address the complex interplay between these conditions

Nonsevere Hemophilia: The Need for a Renewed Focus and Improved Outcomes.

Semin Thromb Hemost

May 2024

Irish Centre for Vascular Biology, School of Pharmacy and Biomolecular Sciences, Royal College of Surgeons in Ireland, Dublin, Ireland.

People with nonsevere hemophilia (PWNSH) are phenotypically more diverse than those with severe hemophilia. Perceptions relating to a "nonsevere" phenotype have contributed to fewer research initiatives, fewer guidelines on optimal management, and a lack of standards for surveillance and clinical assessment for affected individuals. In many cases, episodes of abnormal bleeding could, if investigated, have led to earlier diagnosis.

Heart failure (HF) is a well-described final common pathway for a broad range of diseases however substantial confusion exists regarding how to describe, study, and track these underlying etiologic conditions. We describe (1) the overlap in HF etiologies, comorbidities, and case definitions as currently used in HF registries led or managed by members of the global HF roundtable; (2) strategies to improve the quality of evidence on etiologies and modifiable risk factors of HF in registries; and (3) opportunities to use clinical HF registries as a platform for public health surveillance, implementation research, and randomized registry trials to reduce the global burden of noncommunicable diseases. Investment and collaboration among countries to improve the quality of evidence in global HF registries could contribute to achieving global health targets to reduce noncommunicable diseases and overall improvements in population health.

Atrial fibrillation (AF), the most common cardiac arrhythmia, is associated with adverse CV outcomes. Vascular aging (VA), which is defined as the progressive deterioration of arterial function and structure over a lifetime, is an independent predictor of both AF development and CV events. A timing identification and treatment of early VA has therefore the potential to reduce the risk of AF incidence and related CV events.
