Purpose: Identification of patients' intended chemotherapy regimens is critical to most research questions conducted in the real-world setting of cancer care. Yet, these data are not routinely available in electronic health records (EHRs) at the specificity required to address these questions. We developed a methodology to identify patients' intended regimens from EHR data in the Optimal Breast Cancer Chemotherapy Dosing (OBCD) study.

Article Synopsis
  • Immune checkpoint inhibitors (ICIs) are effective cancer treatments but can lead to immune-related adverse events (irAEs) affecting multiple organs, particularly in the gastrointestinal and hepatic systems.
  • A review of recent literature on these toxicities emphasizes the need for gastroenterologists to identify and manage conditions like enterocolitis, hepatitis, and others, using strategies from relevant oncology guidelines.
  • The management of irAEs involves assessing severity, ruling out other causes, and employing treatments such as corticosteroids and immunosuppressants, but clinical guidelines are primarily based on retrospective data.

Background: Serum level of microRNA-122 (miR-122) has been reported as a sensitive diagnostic biomarker for detecting liver injury, comparable to the aminotransferases. Aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities are increased in other conditions, such as acute skeletal muscle injury (ASMI). We determined whether miR-122 is nonspecifically increased in patients suffering from ASMI.

Medical Decision-Making and Revascularization in Ischemic Cardiomyopathy.

Med Clin North Am

May 2024

Department of Cardiology, Kaiser Permanente San Francisco Medical Center, 2425 Geary Boulevard, San Francisco, CA 94115, USA; Clinical Trials Program, Division of Research, Kaiser Permanente Northern California, 2000 Broadway, Oakland, CA 94612, USA. Electronic address:

Ischemic cardiomyopathy (ICM) is the most common underlying etiology of heart failure in the United States and is a significant contributor to deaths due to cardiovascular disease worldwide. The diagnosis and management of ICM has advanced significantly over the past few decades, and the evidence for medical therapy in ICM is both compelling and robust. This contrasts with evidence for coronary revascularization, which is more controversial and favors surgical approaches.

Heterotopic ossification (HO) is a debilitating pathology where ectopic bone develops in areas of soft tissue. HO can develop as a consequence of traumatic insult or as a result of dysregulated osteogenic signaling, as in the case of the orphan disease fibrodysplasia ossificans progressiva (FOP). Traumatic HO (tHO) formation is mediated by the complex interplay of signaling between progenitor, inflammatory, and nerve cells, among others, making it a challenging process to understand.

Article Synopsis
  • The study investigates how changes in prostate-specific antigen (PSA) levels at 3 and 7 months of androgen deprivation therapy (ADT) relate to overall survival in patients with metastatic hormone-sensitive prostate cancer (mHSPC).
  • Results show that a complete response (CR) in PSA at both 3 and 7 months significantly correlates with improved overall survival compared to no response, with specific statistical measures supporting this association.
  • The findings suggest that monitoring PSA levels can help identify patients at higher risk of death, aiding in treatment decisions and the design of future clinical trials.

Purpose: There are currently no consensus guidelines on establishing metrics for investigational drug services (IDS). Because of the complexity of research protocols, it remains difficult for sites to track pharmacy productivity and create a baseline for IDS growth within the institution, as well as to perform benchmarking with peer institutions. The goal of this study was to help establish practical guidance for IDS metrics and site utility as applicable.

Article Synopsis
  • Systemic immunomodulatory agents are used to treat moderate-to-severe plaque psoriasis and psoriatic arthritis, but their use before surgery may raise the risk of infections and complications.
  • The study aimed to assess the risk of surgical site infections (SSI) in patients with autoimmune diseases who are on various immunomodulatory drugs and undergoing surgery.
  • Findings suggest that for low-risk surgeries, many immunomodulatory treatments, such as TNF-α inhibitors and methotrexate, can be safely continued, while for more complex surgeries, a careful evaluation is recommended before continuing their use.

Durable tolerance in kidney transplant recipients remains an important but elusive goal. We hypothesized that adding B cell depletion to T cell depletion would generate an immune milieu postreconstitution dominated by immature transitional B cells, favoring tolerance. The Immune Tolerance Network ITN039ST Research Study of ATG and Rituximab in Renal Transplantation was a prospective multicenter pilot study of live donor kidney transplant recipients who received induction with rabbit antithymocyte globulin and rituximab and initiated immunosuppression (IS) withdrawal (ISW) at 26 weeks.

Rationale and design of NUDGE-FLU-CHRONIC and NUDGE-FLU-2: Two nationwide randomized trials of electronic nudges to increase influenza vaccination among patients with chronic diseases and older adults during the 2023/2024 influenza season.

Am Heart J

June 2024

Department of Cardiology, Copenhagen University Hospital - Herlev and Gentofte, Copenhagen, Denmark; Center for Translational Cardiology and Pragmatic Randomized Trials, Department of Biomedical Sciences, Faculty of Health and Medical Sciences, University of Copenhagen, Copenhagen, Denmark; Steno Diabetes Center Copenhagen, Copenhagen, Denmark. Electronic address:

Background: Yearly influenza vaccination is strongly recommended for older adults and patients with chronic diseases including cardiovascular disease (CVD); however, vaccination rates remain suboptimal, particularly among younger patients. Electronic letters incorporating behavioral nudges are highly scalable public health interventions which can potentially increase vaccination, but further research is needed to determine the most effective strategies and to assess effectiveness across different populations. The purpose of NUDGE-FLU-CHRONIC and NUDGE-FLU-2 are to evaluate the effectiveness of electronic nudges delivered via the Danish governmental electronic letter system in increasing influenza vaccination among patients with chronic diseases and older adults, respectively.
