The many faces of pediatric hydatid disease: a pictorial review.

Pediatr Radiol

November 2024

Instituto Nacional de Salud del Niño San Borja, Av. Javier Prado Este 3101, Lima, 15037, Peru.

Hydatid disease, caused by the larval stages of Echinococcus species, poses a significant public health challenge, especially in resource-limited cattle-producing areas of South America. The number of cases in children under the age of 15 is nearly 16% of the total cases in South America according to the latest report of the Pan American Health Organization (PAHO). The presentation of the disease depends on the anatomic location and correlates with the parasitic life stage.

  • Freezing of gait (FOG) is a debilitating symptom of Parkinson's Disease and Progressive Supranuclear Palsy, leading to significant quality of life impairment, and its underlying mechanisms remain poorly understood.
  • A study involving three patients with Progressive Supranuclear Palsy-progressive gait freezing (PSP-PGF) showed no deterioration in gait and balance after spinal cord stimulation (SCS) over 13 months, alongside notable changes in brain activity patterns.
  • The findings indicate improved communication between brain and spinal circuits during SCS, suggesting potential advancements in treatments utilizing brain-spine interfaces for FOG patients.

Osteoarticular tuberculosis (TB) is an uncommon form of extrapulmonary TB that has the potential to damage joints and bones, generating long-term impairment. Mainly, the initial diagnosis of osteoarticular TB relies on clinical findings and imaging. When required, imaging can aim for less invasive tissue or fluid sampling for pathology, microbiology, and molecular biology analysis.

Background: To enhance the quality of care available for children with central nervous system (CNS) tumors across the world, a systematic evaluation of capacity is needed to identify gaps and prioritize interventions. To that end, we created the pediatric neuro-oncology (PNO) resource assessment aid (PANORAMA) tool.

Methods: The development of PANORAMA encompassed 3 phases: operationalization, consensus building, and piloting.

Global trends in CD4 count measurement and distribution at first antiretroviral treatment initiation.

Clin Infect Dis

November 2024

Centre for Infectious Disease Epidemiology and Research, School of Public Health, University of Cape Town, South Africa. CIDER, Level 3 Falmouth Building, Anzio Road, Observatory, 7925, South Africa.

Background: While people with HIV (PWH) start antiretroviral treatment (ART) regardless of CD4 count, CD4 measurement remains crucial for detecting advanced HIV disease and evaluating ART programmes. We explored CD4 measurement (proportion of PWH with a CD4 result available) and prevalence of CD4 <200 cells/µL at ART initiation within the International epidemiology Databases to Evaluate AIDS (IeDEA) global collaboration.

Methods: We included PWH at participating ART programmes who first initiated ART at age 15-80 years during 2005-2019.

There is limited access to molecular genetic testing in most low- and middle-income countries. The iHope program provides clinical genome sequencing (cGS) to underserved individuals with signs or symptoms of rare genetic diseases and limited or no access to molecular genetic testing. Here we describe the performance and impact of cGS in 247 patients from three clinics in Peru.

Article Synopsis
  • The study examined the impact of immunoparesis (IP) recovery on the prognosis of 113 newly diagnosed transplant-ineligible multiple myeloma (MM) patients who received a fixed treatment regimen and achieved complete or very good partial responses.
  • Results showed that patients who initially had IP and then experienced recovery during or after treatment had significantly longer progression-free survival (PFS) and overall survival (OS) compared to those who did not recover.
  • Additionally, among MRD negative patients, those with IP recovery also demonstrated improved PFS and OS, highlighting that IP recovery can enhance prognostic evaluations in combination with MRD status in this patient population.

Article Synopsis
  • Balloon atrioseptostomy is an emergency procedure used in congenital heart disease to improve blood flow and mixing in young patients, which is guided by either fluoroscopy or echocardiography.
  • A study conducted between 2018 and 2023 analyzed 36 patients who underwent the procedure, with a majority being male and over 60% diagnosed with transposition of the great vessels.
  • Both methods yielded a 100% success rate without complications, with echocardiography being more advantageous as it avoids radiation exposure and allows the procedure to be performed in the patient's crib.

Introduction: The progression of neurologic cavovarus (NCV) and equinocavovarus (NECV) foot deformities during growth can eventually lead to non-reducible severe talonavicular (TN) and subtalar (ST) joint rotational malalignments. This study aimed to describe a technique to correct severe, resistant inversion deformity of the ST joint complex as the foundational procedure in comprehensive reconstruction of multi-segmental NCV and NECV deformities in pediatric patients and to analyze mid-term radiologic, functional, and satisfaction outcomes.

Methods: This prospective study included children with severe NCV and NECV foot deformities who underwent TCNR between 2019 and 2022, with a minimum follow-up of 2 years.

Cancer Rep (Hoboken)

September 2024

Division of Hematological Malignancies, Dana-Farber Cancer Institute, Harvard Medical School, Boston, Massachusetts, USA.

Article Synopsis
  • Follicular lymphoma (FL) is the most common slow-growing non-Hodgkin lymphoma in the U.S. and Europe, but there's limited data from Latin America, particularly Chile.
  • The study involved 722 Chilean patients diagnosed with FL from 2000 to 2019, revealing a median diagnosis age of 62, with many patients presenting advanced disease and a significant number affected by bone marrow involvement.
  • Key findings show that adding rituximab to chemotherapy improves overall survival and response rate, while experiencing POD24 (progression of disease within 24 months) indicates much poorer survival outcomes.
