Article Synopsis
  • The MitraClip procedure for mitral valve repair is used in high-risk patients but often results in a sizable iatrogenic atrial septal defect (iASD).
  • Out of 2,722 patients who received the MitraClip, 30 (1%) required additional closure of the iASD shortly after the procedure due to complications like hypoxemia and heart failure.
  • The study found that most patients needing closure experienced significant health issues within 24 hours post-procedure, and while most were treated successfully, one patient unfortunately died before closure could be executed.

Although intraoperative anesthetic management of extensive encircling pulmonary vein isolation (PVI) is essential for the safe performance of this procedure, there is no standardized approach for the use of sedation and analgesia. Therefore, the present study aimed to clarify the optimal fentanyl dosage and timing of administration for the anesthetic management during PVI. A total of 364 patients with atrial fibrillation (AF) who underwent PVI at our institution between June 2017 and October 2020 were recruited.

February 2022

Department of Cardiovascular Biology and Medicine, Juntendo University Graduate School of Medicine, Tokyo, Japan; Japan Agency for Medical Research and Development-Core Research for Evolutionary Medical Science and Technology (AMED-CREST), Japan Agency for Medical Research and Development, Tokyo, Japan.

Article Synopsis
  • Frailty and sarcopenia are prevalent issues in elderly heart failure patients, with higher rates seen in women for frailty and in men for sarcopenia.
  • Both conditions significantly impact the risk of 1-year mortality in heart failure patients, although frailty specifically predicted mortality only in men.
  • The study found no significant gender interactions influencing the prognostic impact of frailty or sarcopenia, emphasizing that both conditions are detrimental for older patients regardless of sex.

Usefulness of Incorporating Hypochloremia into the Get With The Guidelines-Heart Failure Risk Model in Patients With Acute Heart Failure.

Am J Cardiol

January 2022

Department of Cardiovascular Biology and Medicine and; Japan Agency for Medical Research and Development-Core Research for Evolutionary Medical Science and Technology (AMED-CREST), Japan Agency for Medical Research and Development, Tokyo, Japan.

Although hypochloremia is strongly associated with adverse prognosis in acute heart failure (AHF), it is unknown whether incorporating hypochloremia into the preexisting risk model improves the model performance. We calculated the Get With The Guidelines-Heart Failure (GWTG-HF) risk score in 1,428 patients with AHF (derivation cohort) and developed 2 risk scores incorporating brain natriuretic peptide (BNP) into the GWTG-HF risk score (GWTG-BNP risk score) and incorporating both BNP and hypochloremia (GWTG-BNP-Cl risk score). Hypochloremia was defined as <98 mmol/L.
