In patients with chronic kidney diseases (CKD), high serum indoxyl sulfate (IS) levels correlate with cardiac fibrosis and hypertrophy and thus a critical risk factor for heart failure. The aim of this study was to determine the effects of IS on cardiac function and inflammasome pathway in a rat model of CKD. We assessed the physiological and pathological changes and measured biomarkers of fibrosis and hypertrophy in the hearts of Dahl salt-sensitive (DS), DS hypertensive (DH), and DH IS-treated rats (DH + IS).

Progression of left ventricular apical hypoplasia-like restrictive cardiomyopathy with severe pulmonary hypertension: Follow-up from fetal stage.

J Cardiol Cases

October 2021

Department of Pediatrics and Pediatric Cardiology, Saitama Medical Center, Saitama Medical University, 1981 Kamoda, Kawagoe-shi, Saitama 350-8550, Japan.

Article Synopsis
  • Left ventricular apical hypoplasia is a rare heart condition where the left ventricle (LV) is underdeveloped, often leading to unique heart shapes and dysfunctions.
  • A case study of a young boy shows that this condition can develop postnatally, contrary to the belief that its distinctive "banana-shaped" right ventricle (RV) is always congenital.
  • The Potts shunt surgery proved beneficial for this patient, improving pulmonary hypertension and stabilizing heart failure symptoms for three years.

Usefulness of random-start progestin-primed ovarian stimulation for fertility preservation.

J Ovarian Res

January 2022

Department of Obstetrics and Gynecology, Saitama Medical Center, Saitama Medical University, 1981 Kamoda, Kawagoe, Saitama, 350-3550, Japan.

Background: Progestin-primed ovarian stimulation (PPOS) has been used in infertility cases in recent years, and several reports have stated that it has oocyte collection results similar to those of gonadotropin-releasing hormone antagonist (GnRH-ant) protocol. For emergency fertility preservation, random-start ovarian stimulation is usually recommended. Therefore we compared the clinical outcomes of random-start PPOS with those of conventional random-start GnRH-ant protocols in fertility-preserving cases.

Vaginal cancer diagnosed during pregnancy presenting a therapeutic dilemma: A case report.

Gynecol Oncol Rep

November 2021

Department of Obstetrics and Gynecology, Saitama Medical Center, Saitama Medical University, 1981 Kamoda, Kawagoe, Saitama 350-8550, Japan.

Article Synopsis
  • Vaginal cancer is a rare condition, making up only about 2% of female reproductive cancers, and it is primarily treated with radiation or chemoradiotherapy, though early-stage cases may involve surgery.
  • A 38-year-old pregnant patient was diagnosed with stage I vaginal cancer after a tumor was found at 11 weeks of gestation and was surgically removed at 13 weeks.
  • Despite a treatment plan involving radical surgery post-abortion, the patient chose to continue the pregnancy, leading to complications; ultimately, a cesarean section was performed at 26 weeks to address the cancer recurrence.

Background: Using two static scans for I-meta-iodobenzyl-guanidine (I-MIBG) myocardial scintigraphy ignores the dynamic response from vesicular trapping in nerve terminals. Moreover, the long pause between scans is impractical for patients with Lewy body diseases (LBDs). Here, we optimized indices that capture norepinephrine kinetics, tested their diagnostic performance, and determined the differences in I-MIBG performance among disease groups.

Intravenous infusion route in maternal resuscitation: a scoping review.

BMC Emerg Med

December 2021

Japan Resuscitation Council, Maternal group, Tokyo Japan, 2-5-4 Yoyogi, Sibuya-ku, Tokyo, 151-0053, Japan.

Background: The concept that upper extremities can be used as an infusion route during cardiopulmonary resuscitation in pregnant women is a reasonable recommendation considering the characteristic circulation of pregnant women; however, this method is not based on scientific evidence.

Objective Of The Review: We conducted a scoping review to determine whether the infusion route should be established above the diaphragm during cardiopulmonary resuscitation in a pregnant woman.

Discussion: We included randomized controlled trials (RCTs) and non-RCTs on the infusion of fluids in pregnant women after 20 weeks of gestation requiring establishment of an infusion route due to cardiac arrest, massive bleeding, intra-abdominal bleeding, cesarean section, severe infection, or thrombosis.

In recent years, local governments in Japan have established a public financial support system for fertility preservation in pediatric, adolescent, and young adult cancer patients. Fertility preservation has become popular for patients with cancers included in the gonadal toxicity risk classification of the 2017 edition of the Guideline for Fertility Preservation in Children, Adolescents and Young Adult Cancer Patients from the Japan Society of Clinical Oncology. However, patients with cancer and non-cancer diseases that are not included in the Guideline's gonadal toxicity risk classification also often receive treatment that may affect fertility, but they are often denied the opportunity of fertility preservation because no public financial support is available for diseases not listed in the Guideline.

Background: although high-density lipoprotein has cardioprotective effects, the association between serum high-density lipoprotein cholesterol (HDL-C) and hypertension is poorly understood. Therefore, we investigated whether high and low concentrations of HDL-C are associated with high blood pressure (HBP) using a large healthcare dataset.

Methods: in a community-based cross-sectional study of 1,493,152 Japanese people (830,669 men and 662,483 women) aged 40-74 years who underwent a health checkup, blood pressures automatically measured at healthcare center were investigated in nine HDL-C groups (20-110 mg/dL or over).

Article Synopsis
  • Immune-checkpoint inhibitors (ICIs) are used for treating non-small cell lung cancer (NSCLC), with programmed death-ligand 1 expression as a common but insufficient predictor of their effectiveness.
  • This study evaluated tumor growth rate, specifically the initial rapidity of tumor progression (IRP), as a simpler predictor of ICI treatment response among NSCLC patients treated with drugs like nivolumab, pembrolizumab, or atezolizumab.
  • Results showed a negative correlation between IRP and time to treatment failure, indicating that faster initial tumor growth is linked to poorer therapeutic outcomes with ICIs.

We asked if sensation-seeking is linked to premorbid personality characteristics in patients with addictive disorders, or the characteristics follow the sensation-seeking activity. We interpreted the former as a state associated with normal rates of dopamine synthesis, and the latter as a trait of individuals with abnormally high rates of synthesis. We previously determined dopaminergic receptor density in striatum, and we now tested the hypothesis that an elevated dopaminergic condition with increased extracellular dopamine and receptor density follows increased dopamine synthesis capacity in highly sensation-seeking individuals, as measured by positron emission tomography of 18 men with tracer fluorodopa (FDOPA).
