Risk of gastric adenoma and adenocarcinoma in patients with familial adenomatous polyposis in Japan: a nationwide multicenter study.

J Gastroenterol

March 2024

The Committee of Hereditary Colorectal Cancer, Japanese Society for Cancer of the Colon and Rectum, Tokyo, Japan.

Article Synopsis
  • Patients with familial adenomatous polyposis (FAP) are at a higher risk of gastric neoplasms, including adenomas and cancer, and this study investigates their incidence over time.
  • A cohort of 443 FAP patients was analyzed, revealing that by age 50, 22.8% had gastric adenomas and 7.6% had gastric cancer, with the highest risk for adenomas occurring around age 65.
  • The study suggests that regular monitoring of the upper gastrointestinal tract in older FAP patients could improve early detection of gastric cancer, indicating a need for more frequent check-ups as patients age.

Background: Temporal trends in the management of acute coronary syndrome complicated with cardiogenic shock after the revision of guideline recommendations for intra-aortic balloon pump (IABP) use and the approval of the Impella require further investigation, because their impact remains uncertain.

Methods And Results: Using the Japanese Percutaneous Coronary Intervention (J-PCI) registry database from 2019 to 2021, we identified 12 171 patients undergoing percutaneous coronary intervention for acute coronary syndrome complicated with cardiogenic shock under mechanical circulatory support. The patients were stratified into 3 groups: (1) IABP alone, (2) Impella, and (3) venoarterial extracorporeal membrane oxygenation (VA-ECMO); the VA-ECMO group was further stratified into (3a) VA-ECMO alone, (3b) VA-ECMO in combination with IABP, and (3c) VA-ECMO in combination with Impella.

Background The identification of large-artery stiffness as a major, independent risk factor for cardiovascular disease-associated morbidity and death has focused attention on identifying therapeutic strategies to combat this disorder. Genetic manipulations that delete or inactivate the translin/trax microRNA-degrading enzyme confer protection against aortic stiffness induced by chronic ingestion of high-salt water (4%NaCl in drinking water for 3 weeks) or associated with aging. Therefore, there is heightened interest in identifying interventions capable of inhibiting translin/trax RNase activity, as these may have therapeutic efficacy in large-artery stiffness.

Article Synopsis
  • Complex factors, both genetic (like MMR gene mutations in Lynch syndrome) and environmental (such as alcohol consumption), may influence the risk and timing of colorectal cancer onset in patients.
  • A study involving 288 patients with Lynch syndrome found that alcohol drinkers had a significantly higher risk of developing colorectal cancer earlier compared to non-drinkers.
  • The increased risk was particularly notable in men, people with specific MMR gene mutations (MLH1 and MSH2), and those with tumors in the proximal colon, suggesting the need for alcohol reduction as a preventative measure.
