The number of patients eligible for transcatheter aortic valve replacement( TAVR) has expanded significantly since its initial approval in 2013, and a wealth of evidence has led to TAVR being considered for all patients with symptomatic severe aortic stenosis (AS), based on the overall judgment of the heart team. Clinical results of TAVR in moderate- and low-risk patients, as well as in chronic hemodialysis patients, are expected to emerge in the coming years. We believe that the heart team, including cardiovascular surgeons and cardiologists, needs to be further matured to consider "life-time management" of each patient from various perspectives and comprehensively.

Department of Neuropsychiatry, University of Toyama Graduate School of Medicine and Pharmaceutical Sciences, 2630 Sugitani, Toyama city, Toyama 930-0194, Japan; Research Center for Idling Brain Science, University of Toyama, 2630 Sugitani, Toyama city, Toyama 930-0194, Japan.

An anomalous pattern of structural covariance has been reported in schizophrenia, which has been suggested to represent connectome changes during brain maturation and neuroprogressive processes. It remains unclear whether similar differences exist in a clinical high-risk state for psychosis, and if they are associated with a prodromal phenotype and/or later psychosis onset. This multicenter magnetic resonance imaging study cross-sectionally examined structural covariance in a large at-risk mental state (ARMS) sample with different outcomes.

Article Synopsis
  • A 46-year-old man experienced an acute type A aortic dissection and received a total arch replacement using the frozen elephant trunk procedure.
  • Two months post-surgery, he developed back pain and was found to have a complication called distal stent graft-induced new entry (dSINE) via a CT scan.
  • Investigating this issue using computational fluid dynamics revealed that increased wall shear stress at the true lumen, rather than the FET site, might have contributed to the dSINE occurrence.

  • * It involved 312 PAC samples and 315 renin samples, finding that PAC values from CLEIA and liquid chromatography/mass spectrometry/mass spectrometry (LC-MS/MS) were highly correlated, while RIA showed more false positives.
  • * The results indicate that using PAC from CLEIA improves PA detection rates and that weak mineralocorticoids affect PAC measurements, notably in the traditional RIA method.

  • The new Japanese guidelines for primary aldosteronism introduce a "borderline range" for functional confirmatory tests, which is not commonly found in other international guidelines.
  • This study compared patients in the borderline group to definitive-positive and negative groups using data from a nationwide Japanese registry, analyzing results from 1,785 individuals based on their captopril-challenge test results.
  • The findings showed that the borderline group did not differ significantly from the negative group concerning target organ damage, though both groups had higher odds of chronic kidney disease and electrocardiographic abnormalities when compared to definitive-positive patients.

  • A retrospective multicenter study was conducted in Japan to assess the influence of preoperative neck pain on clinical outcomes after posterior decompression surgery for cervical ossification of the posterior longitudinal ligament (OPLL).
  • The study involved 90 patients followed for at least two years, who were grouped based on the presence or absence of preoperative neck pain, but the two groups showed no significant demographic or outcome differences.
  • Both groups improved post-surgery according to the Japanese orthopedic association (JOA) scores, but lower JOA scores and larger cervical angles in the no preoperative pain group were linked to higher chances of developing postoperative neck pain.
