Article Synopsis
  • Survivors of head and neck cancer (HNC) are more likely to develop second primary lung cancer (SPLC), primarily due to smoking as a common risk factor.
  • * A study analyzed 100 HNC patients undergoing lung surgery and compared them to 200 matched patients without HNC, revealing significant differences in nutritional status and complications.
  • * Results showed that HNC patients faced more malnutrition, had higher surgery complication rates, and lower 5-year survival rates compared to the control group, indicating a need for specialized care in this high-risk population.

Tardive dyskinesia is recognized as buccolingual dyskinesia, but also includes various involuntary movements, such as chorea, dystonia, myoclonus, and tremor. Tardive dyskinesia can be treated depending on the type of movement disorder present. Antipsychotics causing tardive dyskinesia should be reduced in dosage or should be discontinued.

Article Synopsis
  • Delirium is a major issue in ICUs and is linked to worse patient outcomes, yet there's limited information on how many patients are actually assessed for it and what tools are used.
  • A study involving 20 institutions examined the use of delirium assessment tools in ICU/HCU patients over a month, focusing on usage rates and delirium prevalence.
  • Results showed that 95% of institutions used tools like the ICDSC or CAM-ICU, with a 17.9% prevalence of delirium during hospital stays, and two-thirds of patients improved by discharge.

Occupational exposure of anticancer agents during their preparation has been recognized as a serious hazard. Closed system drug transfer devices (CSTDs) enable "safe" preparation of agents for medical personnel and ensure a safe hospital environment. However, artificial particles of infusion materials have been reported during CSTD use.

Clinical Applications of Meshed Multilayered Anatomical Models by Low-Cost Three-Dimensional Printer.

Plast Reconstr Surg

December 2021

From the Department of Plastic and Reconstructive Surgery, Osaka Medical College; and Department of Plastic and Reconstructive Surgery, Saiseikai Imabari Hospital.

In recent years, even low-cost fused deposition modeling-type three-dimensional printers can be used to create a three-dimensional model with few errors. The authors devised a method to create a three-dimensional multilayered anatomical model at a lower cost and more easily than with established methods, by using a meshlike structure as the surface layer. Fused deposition modeling-type three-dimensional printers were used, with opaque polylactide filament for material.

Background And Aim: Although an inverse relationship between current smoking and the development of ulcerative colitis (UC) has been shown in North America and Europe, evidence is limited in Asian countries, where the incidence of UC is rapidly increasing. This Japanese case-control study examined the association between active and passive smoking and risk of UC.

Methods: A self-administered questionnaire was used to obtain information on smoking and potential confounding factors in 384 cases with a diagnosis of UC within the past 4 years and 665 controls.

Background: Over the past decades, remarkable advancements in systemic drug therapy have improved the prognosis of patients with bone metastases. Individualization is required in external beam radiotherapy (EBRT) for bone metastases according to the patient's prognosis. To establish individualized EBRT for bone metastases, we investigated factors that affect the local control (LC) of bone metastases.

Purpose: A recent report by the American Association of Physicists in Medicine Task Group 75 and 180 provided imaging dose estimates for image-guided CyberKnife radiotherapy. However, to our knowledge, there have been no concrete demonstrations of imaging intervals that are directly linked to exposure dose. We hypothesized that setting a rational standard may be clearer through a balance of treatment accuracy and reducing imaging doses if the margin of the planned treatment volume is controlled through the imaging interval.

Dietary intake of vegetables, fruit, and antioxidants and risk of ulcerative colitis: A case-control study in Japan.

Nutrition

December 2021

Japan Ulcerative Colitis Study Group (Other members of the Study Group are listed in the Appendix).

Objectives: Oxidative stress is considered one of the etiologic factors involved in ulcerative colitis (UC), yet there is limited epidemiologic information regarding the relationship between antioxidant intake and the risk of UC. The aim of the present case-control study in Japan was to examine the association between intake of green and yellow vegetables, other vegetables, fruit, vitamin C, vitamin E, retinol, alpha-carotene, beta-carotene, and cryptoxanthin and UC risk.

Methods: A total of 384 cases within 4 y of diagnosis with UC and 665 controls were included in the study.

Objectives: Heparin resistance (HR), defined as a decrease in heparin responsiveness, can result in adverse events with prolonged duration of surgery. Although some clinical risk factors have been suggested, the relationship with the surgical diagnosis is unclear. The aim of present study was to elucidate the clinical predictors of HR including the surgical diagnosis.

Background: An unexpected recurrence of hepatocellular carcinoma (HCC) sometimes occurs in patients with hepatitis C virus (HCV) after treatment with direct-acting antivirals (DAAs). However, the characteristics of patients with HCC recurrence may differ depending on time after DAA treatment. We aimed to identify risk factors related to HCC recurrence according to time after DAA treatment.

The double-blind part of the COMFORT-PD (COMt-inhibitor Findings from Opicapone Repeated Treatment for Parkinson's Disease) study in Japanese levodopa-treated patients with Parkinson's disease and motor fluctuations found that both opicapone 25 and 50 mg were significantly more effective than placebo. This 52-week open-label extension study evaluated the long-term safety and efficacy of opicapone 50 mg tablets in patients who completed the double-blind part of the COMFORT-PD study. Safety was monitored via adverse events, laboratory testing, and physical, cardiovascular and neurological examinations.

Objectives: This placebo-controlled, randomized study evaluated the efficacy and safety of opicapone 25-mg and 50-mg tablets in Japanese levodopa-treated patients with Parkinson's disease and motor fluctuations.

Methods: Japanese adults (n = 437, age 39-83 years) with Parkinson's disease (United Kingdom Parkinson's Disease Society criteria) received opicapone 25-mg (n = 145), opicapone 50-mg (n = 145), or placebo (n = 147) tablets over the double-blind treatment period (14-15 weeks). The primary efficacy assessment was change in OFF-time; secondary efficacy assessments included OFF/ON-time responders (≥1 hour change from baseline), total ON-time, ON-time with and without troublesome dyskinesia, and Unified Parkinson's Disease Rating Scale.

Article Synopsis
  • Gastric endoscopic submucosal dissection (ESD) under heparin replacement for patients on anticoagulants has a historically high risk of delayed bleeding, which may have changed over time.
  • A study reviewed 32 patients from 2015 to 2017, revealing a delayed bleeding rate of 9.4%, with 12.5% in the warfarin group and none in the direct oral anticoagulant group.
  • Continued aspirin use was identified as a significant risk factor for delayed bleeding, indicating the need for careful management of patients in this scenario.
