Purpose: The aim of this study was to observe the effects of changing humeral tray thickness on the resultant of intraoperative glenohumeral joint loads using a load-sensing system (LSS).

Methods: An rTSA was performed on fresh frozen full-body cadaver shoulders by using an internal proprietary LSS on the humeral side. The glenohumeral loads (Newtons) and the direction of the resultant force applied on the implant were recorded during four standard positions (External rotation, Extension, Abduction, Flexion) and three "complex" positions of Activity Daily Life ("behind back", "overhead reach" and "across chest").

In rhinoplasty, diced cartilage with fascia (DC-F) is a commonly used technique to enhance dorsal projection and achieve a smooth dorsum. We have developed a modified technique by dissecting the thickness of the fascia along its width, called "Mini DC-F". This method allows for effective camouflaging of dorsal irregularities and provides a slight increase in dorsal height.

Training for a mass casualty incident: Conception, development, and implementation of a crew-resource management course for forward surgical teams.

J Trauma Acute Care Surg

August 2024

From the Department of Thoracic and Vascular Surgery (H.d.L., J.-P.A.), Department of Anesthesiology and Intensive Care (W.M.), Sainte Anne Military Hospital, Toulon; Department of Anesthesiology and Intensive Care (R.P., A.L.), Laveran Military Teaching Hospital, Marseille; Département Recherche Expertise Formation Aéromédicale (S.L., A.V.), French Armed Forces Biomedical Research Institute, Brétigny sur Orge; Department of Digestive and Endocrine Surgery (B.d.l.V.), Laveran Military Teaching Hospital, Marseille; French Military Medical Academy (A.L., P.B., P.P., J.-P.A.), École du Val-de-Grâce, Paris; Department of Anesthesiology and Intensive Care (P.P.), Percy Military Teaching Hospital, Clamart; and French Special Operations Forces Medical Component Command (P.P.), Villacoublay, France.

Background: Since 2021, the predeployment training of French FSTs has included a simulation-based curriculum consisting of organizational and human factors. The purpose of this article is to describe the development of a crew-resource management (CRM) training course dedicated for the forward surgical teams (FSTs) of the French Military Health Service.

Methods: The approach was based on three steps: (1) establishment of a conceptual framework of FSTs deployment; (2) development of an aircrew-like CRM training combining lectures, laboratory exercises, and situational training exercises to consider four fundamental "nontechnical" (cognitive and social) skills for effective and safe combat casualty care: (a) leadership, (b) decision-making, (c) coordination, and (d) situational awareness; (3) Implementation of teamwork evaluation tools.

Background And Purpose: To evaluate the reliability and accuracy of nonaneurysmal perimesencephalic subarachnoid hemorrhage (NAPSAH) on Noncontrast Head CT (NCCT) between numerous raters.

Materials And Methods: 45 NCCT of adult patients with SAH who also had a catheter angiography (CA) were independently evaluated by 48 diverse raters; 45 raters performed a second assessment one month later. For each case, raters were asked: 1) whether they judged the bleeding pattern to be perimesencephalic; 2) whether there was blood anterior to brainstem; 3) complete filling of the anterior interhemispheric fissure (AIF); 4) extension to the lateral part of the sylvian fissure (LSF); 5) frank intraventricular hemorrhage; 6) whether in the hypothetical presence of a negative CT angiogram they would still recommend CA.

Introduction: Medial opening wedge high tibial osteotomy (MOWHTO) is a surgical option for medial femorotibial osteoarthritis, which preserves articular cartilage by correcting the extra-articular deformities of the lower limbs that cause osteoarthritis. This is an increasingly popular treatment in a population with high functional demand, such as the French military population. The aim of this study was to evaluate the rate of return to work and military activities at 6 months postsurgery and at last follow-up in this military population following MOWHTO.

J Trauma Acute Care Surg

November 2023

From the Intensive Care Unit (P.E., N.H., H.d.M., P.-L.Q., M.C., E.D., P.G., E.M.), and Department of Neurosurgery (C.J., A.D.), Sainte Anne Military Hospital, Toulon; and French Military Health Service Academy Unit (A.D., E.M.), Ecole du Val-de-Grâce, Paris, France.

Background: The ABO blood system has been involved in the pathogenesis of several diseases, including coagulopathy and bleeding complications. In trauma patients, blood type A has been associated with acute respiratory distress syndrome, and recently, blood type O has been associated with all-cause mortality. The purpose of this study was to assess the association between ABO blood types and long-term functional outcomes in critically ill patients with severe traumatic brain injury (TBI).

  • * 32 patients were treated with a combination of Rituximab and Lenalidomide (R2) over a median follow-up of 22.6 months, but the treatment only achieved a 12.5% response rate.
  • * The results showed a median progression-free survival of 2.6 months and a median overall survival of 9.3 months, leading researchers to conclude that the R2 regimen is not advisable for high-risk patients in this group.

Introduction: There are little data regarding elective referral to the military specialist, especially considering common pathologies such as spinal diseases, which, in the French forces, involve military neurosurgeons. An overview of the management of the military patient referred to neurosurgery consultation, especially considering spinal diseases that both constitute an operational healthissue in the forces and appear of special interest. The objective was to describe the reasons, demographics, and care pathways, addressed to the neurosurgeon.

Retinal pigment epithelium (RPE) tear is a common complication in the context of neovascular age-related macular degeneration treated by intravitreal injections of antivascular endothelial growth factor drugs. There are some reports of such complications after trabeculectomy but none after nonpenetrating deep sclerectomy (NPDS). A 57-year-old man presented to our hospital with uncontrolled advanced glaucoma of his left eye.

Article Synopsis
  • A study evaluated the effectiveness of a handheld near-infrared spectroscope (NIRS) for identifying patients needing neurosurgical care in military medical settings, where immediate CT scans are not available.
  • The analysis included 37 adults with traumatic brain injuries (TBI), revealing that the NIRS was particularly useful in detecting intracranial hematomas, with a 100% negative predictive value for those requiring surgery.
  • Findings indicated that while the NIRS showed promise for rapid assessment, its lack of specificity means it should be combined with other clinical evaluations for better triage decisions.

Introduction: Unlike orthopedic or visceral surgeons, French military neurosurgeons are not permanently deployed on the conflict zone. Thus, craniocerebral war casualties are often managed by general surgeons in the mobile field surgical team. The objective of the study was to provide the feedback of French military surgeons who operated on craniocerebral injuries during their deployment in a role 2 surgical hospital without a neurosurgeon.

Microvesicles (MVs) have previously been shown to exert profibrinolytic capacity, which is increased in patients with septic shock (SS) with a favorable outcome. We, therefore, hypothesized that the plasmin generation capacity (PGC) could confer to MVs a protective effect supported by their capacity to lyse a thrombus, and we investigated the mechanisms involved. Using an MV-PGC kinetic assay, ELISA, and flow cytometry, we found that granulocyte MVs (Gran-MVs) from SS patients display a heterogeneous PGC profile driven by the uPA (urokinase)/uPAR system.
