Clin Transl Allergy

November 2024

Division of Allergy and Immunology, Department of Dermatology, Allergy and Venerology, Charité Universitätsmedizin Berlin, Berlin, Germany.

Article Synopsis
  • IgE-mediated food allergies are the most common type, causing quick and serious reactions that affect patients' and caregivers' lives.
  • Omalizumab is a humanized monoclonal antibody that binds to IgE, reducing allergic reactions, and has been approved by the FDA for treating these food allergies.
  • The GALEN ANACARE Consensus Statement supports omalizumab's use based on a systematic review and expert agreement, noting it is currently the only drug that can significantly reduce IgE-mediated food allergic reactions, although more evidence is needed for stronger guideline recommendations.

This European Academy of Allergy and Clinical Immunology (EAACI) guideline provides recommendations for the management of IgE-mediated food allergy and was developed using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. Following the confirmation of IgE-mediated food allergy diagnosis, allergen avoidance and dietary advice (with support of a specialised dietitian, if possible) together with the provision of a written treatment plan, education on the recognition of allergic symptoms and prescription of medication including adrenaline using an auto-injector are essential. Patients with significant anxiety and requirement for coping strategies may benefit from support from a clinical psychologist.

Characteristic symptoms of NSAID-exacerbated respiratory disease (N-ERD) include asthma, chronic eosinophilic rhinosinusitis with nasal polyposis, cysteinyl LT (CysLT) overproduction and NSAIDs hypersensitivity. Some N-ERD patients present with episodic treatment-resistant extra-respiratory symptoms (CysLT-associated coronary artery vasospasm, gastroenteritis, or skin rash). Even when using standard treatments for respiratory and extra-respiratory symptoms, including systemic corticosteroids and aspirin desensitization, it is difficult to control the clinical symptoms and severe type 2 inflammation involved with mast cells, eosinophils, ILC2s, and platelet activation.

Low-dose oral food challenges.

Pediatr Allergy Immunol

October 2024

Department of Allergy, Clinical Research Center for Allergy and Rheumatology, NHO Sagamihara National Hospital, Sagamihara, Kanagawa, Japan.

Article Synopsis
  • Infants under 1 year with food allergies typically need to avoid certain foods, but low-dose oral food challenges (LD-OFCs) may help safely introduce small amounts.
  • A study analyzed LD-OFCs for infants allergic to hen's egg, cow's milk, or wheat, noting mostly mild skin reactions and no severe incidents like anaphylaxis.
  • The findings suggest that infants can safely consume small doses of allergenic foods, indicating that complete elimination isn't always necessary when reintroducing these foods.

Recombinant Gad c 1 improves diagnostic efficacy of cod allergy but not horse mackerel allergy.

Pediatr Allergy Immunol

October 2024

Department of Allergy, Clinical Research Center for Allergy and Rheumatology, NHO Sagamihara National Hospital, Sagamihara, Kanagawa, Japan.

Article Synopsis
  • The study investigates the effectiveness of recombinant Gad c 1 (rGad c 1) in diagnosing allergies to cod and horse mackerel, considering its potential as a major allergen marker for fish allergy in children.
  • In this retrospective study involving 45 children with possible cod allergies and 38 with horse mackerel allergies, researchers analyzed both cod-specific and rGad c 1-specific immunoglobulin levels through oral food challenges (OFCs).
  • Findings reveal that while rGad c 1 is a more reliable marker for diagnosing cod allergy compared to regular cod-specific antibodies, it is less effective for diagnosing horse mackerel allergy, indicating that further research is needed to fully understand rGad c

Our previous cryogenic electron microscopy (cryoEM) analysis showed that the core structures of α-synuclein filaments accumulated in brains of patients diagnosed with dementia with Lewy bodies (DLB) and multiple system atrophy (MSA) patients are different. We analyzed the post-translational modifications (PTMs) in these filaments , and examined their relationship with the core filament structures and pathological features. Besides the common PTMs in MSA and DLB filaments, acetylation, methylation, oxidation and phosphorylation were frequently detected in MSA filaments, but not in DLB filaments.

Background: Cow's milk allergy (CMA) is the most complex and common food allergy in infants. Elimination of cow's milk from the diet and replacement with a specialized formula for infants with cow's milk allergy who cannot be breastfed is an established approach to minimize the risk of severe allergic reactions while avoiding nutritional deficiencies. Given the availability of multiple options, such as extensively hydrolyzed cow's milk-based formula (eHF-CM), aminoacid formula (AAF), hydrolyzed rice formula (HRF), and soy formula (SF), there is some uncertainty regarding which formula might represent the most suitable choice with respect to health outcomes.
